Takayasu arteritis and large-vessel giant cell arteritis in Italian population. Comprehensive analysis from a single institutional cohort of 184 cases.

OBJECTIVE: To compare clinical and imaging characteristics in patients with Takayasu arteritis (TAK) and large vessel-giant cell arteritis (LV-GCA) in an Italian population. METHODS: We conducted a retrospective monocenter study comparing characteristics and outcomes of a cohort of 59 patients with TAK and a cohort of 127 patients with LV-GCA diagnosed between 1996 and 2016. Most of them (92%) were followed up for at least 24 months at Reggio Emilia Hospital (Italy). We also reviewed the literature discussing the results of the published manuscripts comparing LV-GCA to TAK RESULTS: LV-GCA pat…

Rare Systemic Vasculitides: Polyarteritis Nodosa and Takayasu Arteritis.

BACKGROUND Polyarteritis nodosa (PAN) and Takayasu arteritis (TA) are systemic medium- and large-vessel vasculitides associated with significant morbidity when diagnosis is delayed. Although described in the literature, reports directly demonstrating the impact of diagnostic timing on vascular outcomes remain limited. This report presents 2 rare and contrasting cases highlighting the importance of early recognition with timely vascular and immunosuppressive interventions, which can prevent irreversible ischemic complications, whereas delayed diagnosis can result in permanent structural damage,…

Giant cell arteritis in clinical practice: beyond GiACTA.

OBJECTIVES: Giant cell arteritis (GCA) therapy relies on high-dose glucocorticoids (GCs), which are associated with a high incidence of side effects and GCA relapses, highlighting the need for steroid-sparing agents such as tocilizumab (TCZ) and methotrexate (MTX). The aims of this study were to analyse GC side effects and to assess the steroid-sparing efficacy of TCZ and MTX in a real-life cohort of patients with GCA through the application of the Glucocorticoid Toxicity Index (GTI) version 2.0. METHODS: This retrospective cohort study included patients with a new diagnosis of GCA made in ou…

[Giant cell arteritis revealed by bilateral internuclear ophthalmoplegia].

INTRODUCTION: We report an original observation of giant cell arteritis (GCA) revealed by bilateral internuclear ophthalmoplegia due to parenchymal inflammatory involvement of the central nervous system. CASE REPORT: An 84-year-old female patient presented to our emergency department because of diplopia due to bilateral internuclear ophthalmoplegia. Cerebral MRI found a T2 hypersignal with enhancement after gadolinium injection in T1 sequence of the two medial longitudinal fascicles. Lumbar puncture was negative, biology only found elevated acute phase response proteins, PET scan showed large…

Ocular ischaemic syndrome due to giant cell arteritis.

Giant cell arteritis (GCA) is a large vessel vasculitis with devastating visual consequences if left untreated. Classically, GCA has a predilection for the short posterior ciliary arteries which supply the optic nerve head, causing sudden painless visual loss with the development of arteritic anterior ischaemic optic neuropathy (AAION). Other ocular manifestations of GCA include central retinal artery occlusion, oculomotor nerve palsy, posterior ischaemic optic neuropathy and ocular ischaemic syndrome (OIS). OIS is a rare clinical entity, typically stemming from severe, unilateral carotid arte…

Bilateral juvenile temporal arteritis: a case-based review.

Juvenile Temporal Arteritis (JTA) is a rare non-granulomatous vasculitis affecting the superficial temporal arteries, mostly in individuals under 45 years old. It is often misdiagnosed due to its benign nature and the absence of systemic symptoms. Herein, we present a case report of a 40-year-old woman who initially presented with painless nodules in the left temporal area. Following a biopsy, the patient developed additional nodules not only in the same temple but also on the contralateral side. Remarkably, these nodules underwent spontaneous regression without further treatment, highlighting…

Giant cell arteritis following SARS-CoV-2 infection.

Giant cell arteritis (GCA) is a vasculitis characterised by arterial inflammation, with permanent vision loss being a feared complication. The association between GCA and viral infections is well-known, but there is limited data regarding its relationship to COVID-19. A man in his 80s with a recent history of COVID-19 developed sudden vision loss, persistent headaches and jaw claudication. Ophthalmological findings raised the suspicion of a GCA diagnosis, which was confirmed by temporal artery biopsy. Treatment with prednisone stabilised vision and resolved systemic symptoms. Reports suggest a…

PARACENTRAL ACUTE MIDDLE MACULOPATHY IN GIANT CELL ARTERITIS.

PURPOSE: To describe a representative case and review the literature on paracentral acute middle maculopathy (PAMM) and giant cell arteritis (GCA). METHODS: A review of the English-language ophthalmic literature was performed using the search terms of PAMM, giant cell arteritis, and temporal arteritis. RESULTS: A 72-year-old woman with PAMM as the presenting ophthalmic manifestation of GCA was described with a review of the prior cases from the literature. It was found that there were 26 cases of PAMM in GCA. In 19 of 26 cases, PAMM was associated with no other fundus abnormalities and was o…

Central Retinal Artery Occlusion Associated with Takayasu Arteritis.

Takayasu arteritis is a chronic inflammatory vasculitis with granulomatous panarteritis particularly impacting large vessels including the aorta and its branches, especially the subclavian arteries, with clinical manifestation dependent on the involved artery. Sequelae of the active disease vary, including stenosis, occlusions, or aneurysmal dilatations of the large vessels. The prevalence of Takayasu arteritis is higher in the Asian population and in Japan, but quite low in the United States, varying from 0.9-8.4 per million people. Ocular manifestations are rare and lead to a delay in diagno…

Emergency Medicine Case Study on Giant Cell Arteritis.

The purpose of this case study on giant cell arteritis (GCA) is to ensure that emergency nurse practitioners (ENPs) are knowledgeable on this disease process to prevent serious complications such as permanent blindness in patients presenting to the emergency department (ED). The case study reviews emergency management of GCA, which includes diagnostic workup, pharmacological treatment, consults, disposition, follow-up care, and when to return to the ED. The classification criteria will be discussed for GCA from the American College of Rheumatology and the European Alliance of Associations for …

PARACENTRAL ACUTE MIDDLE MACULOPATHY IN GIANT CELL ARTERITIS.

PURPOSE: To report the occurrence of paracentral acute middle maculopathy (PAMM) in giant cell arteritis (GCA), describe its features and outcomes, and identify risk factors associated with PAMM in patients with GCA. METHODS: Review of medical records of patients with GCA who were examined in the Rothschild Foundation Hospital. Patients were divided into three groups: GCA with PAMM (Group 1), GCA with ophthalmic involvement but without PAMM (Group 2), and GCA without ophthalmic involvement (Group 3). We analyzed the data for age, sex, medical history, laboratory testing, visual acuity, and po…

Bilateral posterior scleritis due to giant cell arteritis.

A woman aged mid 70s with mild learning difficulty presented with eye symptoms, headache and shoulder pains. She was found to have bilateral posterior scleritis secondary to giant cell arteritis. Posterior scleritis is a potentially blinding condition, with ambiguous symptoms and signs, therefore diagnosis is commonly delayed. This patient was seen in General Practice as well as in Accident and Emergency, where diagnoses of acute angle closure glaucoma as well as iritis were rightfully considered. Her symptoms persisted for 11 days before ophthalmic opinion was sought. Thorough assessment of a…
