Antibody reactivity to mycobacterial 65 kDa heat shock protein: relevance to autoimmunity.

Reactivity to the mycobacterial 65 kDa heat shock protein (HSP 65) has been implicated in the pathogenesis of adjuvant arthritis in the rat, and may be involved in the pathogenesis of rheumatoid arthritis or other autoimmune diseases in humans. Accordingly this study sought quantitative or qualitative differences in the antibody reactivity to HSP 65 between normal controls, patients with the multisystem autoimmune diseases, rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) and patients with the mycobacterial infections, tuberculosis (TB) and leprosy. Levels of antibodies to reco

The spondyloarthropathies.

The spondyloarthropathies represent a heterogeneous group of arthropathies including ankylosing spondylitis, reactive arthritis with Reiter syndrome as one subtype, arthritis associated with inflammatory bowel disease, arthritis associated with psoriasis, and juvenile spondyloarthritis. These conditions are linked by several common features: inflammatory arthritis involving the back, a high prevalence of HLA-B27, frequent tendon/ligament insertion inflammation, and several common extra-articular manifestations (iritis, skin lesions). Although not uncommon, these types of arthritis are more dif

Scored antibody reactivity determined by immunoblotting shows an association between clinical manifestations and presence of Borrelia burgdorferi sensu stricto, B. garinii, B. afzelii, and B. Valaisiana in humans.

An immunoglobulin G immunoblot was developed with antigenic extracts of Borrelia burgdorferi sensu stricto, B. garinii, B. afzelii, and B. valaisiana genospecies and was reacted with sera from patients with neuroborreliosis, acrodermatitis, and Lyme arthritis. A detailed analysis of the reactivities of the protein bands was performed, and a two-step scoring procedure was selected to determine the preferential reactivity of sera to one particular genospecies. The discriminative potential of 5 proteins (12-kDa, 16-kDa, 18-kDa, OspA, and 66-kDa proteins) was used as a rapid first-step scoring met

[Peloid therapy in various temperature modes in rehabilitation of patients with seronegative spondylarthritis].

Fifty patients with ankylosing spondylarthritis (AS) and 21 patients with reactive arthritis, Reiter's disease received peloid therapy. AS patients with a prevalent spinal lesion received mud applications (38-40 degrees), those with a prevalent joint lesion - applications of colder mud (22-24 degrees C), those free of inflammation in the joints and periarticular tissues - mud of 38-40 degrees C temperature. In reactive arthritis and Reiter's disease mud applications with temperature 22-24 degrees are indicated.

Yersinia antibodies in inflammatory joint diseases.

The occurrence of IgM, IgG and IgA class Yersinia antibodies was studied at the beginning of an inflammatory joint disease and one year later in 354 adult patients using an ELISA technique. The control groups consisted of age and sex matched healthy persons living in the same geographical area as the patients, and of 64 patients with chronic rheumatoid arthritis. Yersinia antibodies of any Ig class were found in 9.0% of all the patients at the beginning of the disease, in 4.0% of the healthy controls and in 1.6% of the patients with chronic rheumatoid arthritis. Patients with ankylosing spondy

Expression of MHC class II molecules on human monocytes is regulated independently from each other after phagocytosis of bacteria.

Reactive arthritis is usually self-limiting polyarthritis which develops in HLA-B27 positive individuals after certain gastrointestinal or urogenital infections. The pathogenesis of reactive arthritis is unknown but T cells seem to have a crucial role. Most of the antigen-specific T cells isolated from the synovial fluid have been MHC class II restricted. The role of antigen presentation in the pathogenesis of reactive arthritis has been studied relatively little. In this work the authors studied the effect of arthritis-triggering bacterium (Yersinia enterocolitica O:3) on the expression of MH

Arthritis in childhood human immunodeficiency virus infection predominantly associated with human leukocyte antigen B27.

AIM: Children with human immunodeficiency virus (HIV) infection usually present with recurrent and unusual infections. Although reported among adults, arthritis as a predominant presentation in children with HIV is rare. Reactive arthritis is considered to be the most common musculoskeletal manifestation in adults with HIV infection. However, in children, septic or HIV-related arthritis has been described. We report four children having arthritis with HIV disease and their long-term outcome; out of which three had human leukocyte antigen (HLA)-B27-related arthritis. It is important to be aware

Post-yersinial arthritis in Cleveland, England.

Four cases of post-yersinial reactive arthritis are described. All patients presented with an acute lower limb arthropathy with features of an associated enthesopathy. Two patients had restriction of axial skeletal movements. Systemic features were prominent in three, including weight loss and malaise. Mean age of onset was 34 years. Three patients gave a history of antecedent diarrhoea within the previous month. Three patients had raised titres to Y enterocolitica type 0:3 (ranging from 1 in 320 to 1 in 2560) and one had raised titres to Y pseudotuberculosis type 2 at 1 in 640. Two of the thr

Yersinia enterocolitica and Yersinia pseudotuberculosis.

Yersinia enterocolitica can cause enteritis, right lower-quadrant pain mimicking appendicitis, reactive arthritis, and erythema nodosum. This organism is transmitted through food, animal contact, and contaminated blood products. Patients with iron excess are at a higher risk for serious infection. This article describes the history, microbiology, virulence factors, epidemiology, clinical manifestations, diagnosis, and therapy of Y. enterocolitica and Y. pseudotuberculosis. In addition, the immune response of those developing reactive arthritis following infection with Y. enterocolitica is disc

[Blastocystis hominis as a rare arthritogenic pathogen. A case report].

Blastocysts hominis, a protozoon considered to be a nonpathogen intestinal commensal, is now discussed to be a cause of intestinal infection under certain circumstances, e.g., immunosuppression. There are two published cases of Blastocysts infection complicated by arthritis which was classified as "reactive" in one case, "infectious" in the other. We report a third case: A 46-year-old female patient developed a chronic diarrhea and oligoarthritis some days after returning from a trip to Senegal. Arthritis was refractory against treatment with NSAID and corticosteroids. Finally, a 3-week course

Depressed lymphocyte transformation by yersinia and Escherichia coli in yersinia arthritis.

Lymphocyte transformation responses were studied in 21 patients with acute yersinia infection followed-up after the acute infection for up to one year. Eight patients had reactive arthritis caused by yersinia. The responses to the causative serotype of yersinia were significantly lower (p less than 0.05) in patients with arthritis than in those without arthritis. Stimulation with Escherichia coli gave lower responses than with yersinia, but with E. coli the difference between arthritic and nonarthritic groups was more significant (p less than 0.02). The responses to yersinia and E. coli were n

Synovial fibroblasts and synovial macrophages from patients with rheumatoid arthritis and other inflammatory joint diseases show chromosomal aberrations.

Chromosomal aberrations were investigated in nuclei extracted from synovial tissue and first-passage synovial fibroblasts (P-1 SFB, 98% enrichment) or macrophages (P-1 Mphi) from patients with rheumatoid arthritis (n=10). The findings were compared with those in other rheumatic diseases (osteoarthritis, n=14; reactive arthritis, n=1), as well as with those in chronic obstructive pulmonary disease (n=8). Controls were paired peripheral blood lymphocytes from arthritic patients, synovial tissue or SFB/Mphi from joint trauma/normals (n=9), and peripheral blood monocytes from normal donors (n=10).

Rheumatic manifestations of human immunodeficiency virus.

Since July 1983, various rheumatic, musculoskeletal, or other immune disorders characterized by dysregulation have been associated with HIV and AIDS. Infections occur, but with a lower frequency than expected in a patient population with a disorder primarily characterized by significant cellular immune deficiencies. Reactive arthritis, psoriatic arthritis, acute nonspecific arthritis, Sjögren syndrome, and inflammatory myositis have been reported. The initial reports of "acute painful joints," however, have not maintained prominence in the literature. A review of the literature over the past s

HLA-B27 associated reactive spondyloarthropathies in a Dutch military hospital.

Forty-two male patients with reactive spondyloarthropathies in a Dutch military hospital are described with mean age of onset 21.9 years. Peripheral arthritis or sacroiliitis was present in all, eye symptoms in 21 (50%), and genitourinary disease in 15 (35.7%). Evidence of antecedent sexually acquired or enterocolitic infection was found only in three (7.2%). HLA-B27 antigen was detected in 34 (81%) of 42 patients. Additional data suggest that reactive spondyloarthropathies are the most common form of inflammatory arthritis in young men.
