Diabetic autonomic neuropathy.

Diabetic autonomic neuropathy (DAN) affects each tissue, organ, system and the whole body, and presents with a diverse clinical picture. Originating from endocrine factors, this neurological disease may cause symptoms, whose differential diagnosis needs a good knowledge of the whole internal medicine. DAN is strongly involved in the development of diabetic foot, ulceration and amputation. The life threatening consequences of cardiac and patient-frustrating sequels of other types of DAN is more difficult to estimate. In this chapter the different clinical aspects of DAN will be discussed, accor

Orthostatic hypotension. Causes, evaluation, and management.

Chronic orthostatic hypotension is caused by a variety of disorders. Frequently patients withdraw from social interactions, are prone to adverse drug reactions and inappropriate diagnoses, and are bed-bound by the time of diagnosis. Applying basic principles of cardiovascular physiology and pharmacology usually permits these patients to lead active lives and to live longer. Much of the management is based on common sense and knowledge of the basic pathophysiology of the disorder and depends on thorough patient education and close monitoring of blood pressure in many of the activities of daily

Study of sympathetic skin response in patients with COVID-19 infection.

OBJECTIVES: Many articles hypothesized the potential role of autonomic nervous system in the pathogenesis and outcome of COVID-19 infection. Several studies reported both central and peripheral nervous system involvement in COVID-19 as well. Up to our knowledge, there is no study evaluating whether this virus could invade the autonomic nervous system affecting its function adversely. Sympathetic skin response (SSR) has long been used as a method of evaluating the autonomic nervous system. Regarding the importance of the autonomic nervous system in hemostasis and wide consequences of COVID-19 i…

Can novel non-invasive autonomic tests help discriminate between pure autonomic failure and multiple system atrophy?

BACKGROUND: Pure autonomic failure (PAF) and multiple system atrophy (MSA) are rare disorders causing severe autonomic failure. Their initially similar clinical presentation may lead to years of diagnostic difficulties. Improving the differentiation would have an important impact on patients and families in view of better prediction of disease progression. OBJECTIVE: To investigate whether several new non-invasive autonomic tests are beneficial in discriminating between PAF and MSA. METHODS: Patients and controls underwent two tests examining the autonomic innervation of the skin (Sudoscan a

[Signs and symptoms of autonomic dysfunction in dysphonic individuals].

PURPOSE: To verify the occurrence of signs and symptoms of autonomic nervous system dysfunction in individuals with behavioral dysphonia, and to compare it with the results obtained by individuals without vocal complaints. METHODS: Participants were 128 adult individuals with ages between 14 and 74 years, divided into two groups: behavioral dysphonia (61 subjects) and without vocal complaints (67 subjects). It was administered the Protocol of Autonomic Dysfunction, containing 46 questions: 22 related to the autonomic nervous system and had no direct relationship with voice, 16 related to both…

Dysautonomia in fatal familial insomnia as an indicator of the potential role of the thalamus in autonomic control.

Fatal familial insomnia (FFI) is characterized by insomnia, dysautonomia, disruption of circadian rhythms, and motor dysfunction. The typical neuropathological findings in FFI are severe neuronal depletion in the mediodorsal (MD) and anteroventral nuclei of the thalamus. The interaction between the thalamus and central autonomic control mechanisms is poorly understood. The central autonomic areas include the anterior cingulate and insular cortices; amygdala, paraventricular nucleus, dorsomedial nucleus, and lateral hypothalamic area; periaqueductal gray; parabrachial nucleus; ventrolateral med

[Gastrointestinal motility and autonomic nerve dysfunction].

Gastrointestinal motility is greatly influenced by both the autonomic nervous system (ANS) and the enteric nervous system (ENS). Dysfunction of ANS and/or ENS produces various kinds of dysmotility from the esophagus to the colon. Generalized autonomic dysfunction, often seen in diabetics, causes abnormal peristaltic waves in the esophagus, abnormal electrical activity of the stomach, delayed gastric emptying and delayed intestinal transit. Localized disorders of the enteric nervous system is seen in patients with achalasia and Hirschsprung's diseases. Functional disorders, without evidence of …

Commentary: are postconcussive symptoms and exercise testing appropriate tools to identify autonomic impairment after mild traumatic brain injury?

OBJECTIVE: After mild traumatic brain injury (mTBI), autonomic nervous system dysfunction is thought to contribute to exercise intolerance and self-reported postconcussive symptoms (e.g. dizziness, lightheadedness, brain fog) but little has been done to establish this relationship in the literature. METHODS: Through recent literature review, it appears that few studies have assessed both autonomic function and exercise intolerance, and for those that have utilized varying methodologies making comparison across studies difficult. Some emerging research has identified potential impairment withi…

Insights into the background of autonomic medicine.

Knowledge of the physiology underlying the autonomic nervous system is pivotal for understanding autonomic dysfunction in clinical practice. Autonomic dysfunction may result from primary modifications of the autonomic nervous system or be secondary to a wide range of diseases that cause severe morbidity and mortality. Together with a detailed history and physical examination, laboratory assessment of autonomic function is essential for the analysis of various clinical conditions and the establishment of effective, personalized and precise therapeutic schemes. This review summarizes the main as…

[Autonomic Dysfunction in Guillain-Barré Syndrome].

Guillain-Barré syndrome is an acute-onset, immune-mediated neuropathy characterized by humoral immune attacks on peripheral motor and sensor … on motor, sensory, and autonomic nerves or from an immune reaction against antigens specific to the autonomic nervous system that occurs concurrently with attacks on motor and sensory nerves. This paper summarizes the clinical semiology and pathological and physiological findings of autonomic dysfunction in Guillain-Barré syndrome and discusses its pathophysiology by comp …

Autonomic involvement in hereditary transthyretin amyloidosis (hATTR amyloidosis).

PURPOSE: Hereditary transthyretin amyloidosis (hATTR amyloidosis) is a progressive disease primarily characterized by adult-onset sensory, motor, and autonomic neuropathy. In this article, we discuss the pathophysiology and principal findings of autonomic neuropathy in hATTR amyloidosis, the most common methods of assessment and progression, and its relation as a predictive risk factor or a measure of progression in the natural history of the disease. METHODS: A literature search was performed using the terms "autonomic neuropathy," "dysautonomia," and "autonomic symptoms" in patients with he

[Dysautonomia in Guillain-Barré syndrome].

About 20% of all GBS patients have symptoms of dysautonomia: labile hypertension, orthostatic hypotension, sinustachycardia or sinus arrest. This rate rises to 75% in patients with tetraplegia. Proprioceptive loss predicts dysautonomia independently from the severity of weakness. It is frequently responsible for dysautonomia. The afferent limb of cardiovascular regulation contains more myelinated fibers than the sympathetic and parasympathetic efferences, which determine the common classification of dysautonomia. The frequence of mixed sympathetic and parasympathetic hyperactivity is hard to e

[A new ocular sign of autonomic dysfunction].

The author discovered a new sign of vegetative dysregulation, asymmetry of ocular glisten. This symptom can exist separately or combine with other ocular vegetative symptoms. Its mechanism lies in increased or decreased reflection of the eye surface tissues as a result of more or less intensive tonicity of the smooth muscular fibers of the retrobulbar fat or due to changes in the intraocular pressure. The symptom may be used in practice for diagnosis of vegetative dysfunction, primarily vegetative regulatory asymmetry, of pathological alterations in vegetative structures, for topical diagnosis
