A previously healthy young male infant presented to the emergency department in a comatose state with clinical signs suggestive of opioid toxicity. Despite early mechanical ventilation and continuous naloxone infusion, neurological improvement was limited. Brain MRI confirmed toxic encephalopathy. We report a cascade synthesis of indole-fused benzodiazepines by the photocatalyzed addition of phenacyl radical, generated from α-acetoxy acetophenone, to 2-(3-methyl-1H-indol-1-yl)aniline, and subsequent cyclodehydration.
