Gradual fibular transfer by ilizarov external fixator in post-traumatic and post-infection large tibial bone defects.

INTRODUCTION: Several reconstructive procedures have been used in management of large tibial bone defects including bone graft, bone transpo … auma or infection with severe soft tissue compromise, and small or poor tibial bone remnants making bone lengthening difficult. Exclusion criteria were patients with vascular or nerve injuries. The average age of the patients was 31.64 (± 6.5) years. Medial fibular transfer was done for all patients using Ilizarov at a rate of 0.5 mm twice daily. Iliac bone graft …

Family of Crouzon Syndrome Represents the Evolution of the Frontofacial Monobloc Advancement Technique: From Immediate Movement to Monobloc Distraction to Monobloc Bipartition Distraction.

Crouzon syndrome (CS) is an autosomal dominant disorder characterized by premature fusion of cranial sutures, midface and supraorbital ridge retrusion, exorbitism, and in some clinical scenarios strabismus, parrot-beaked nose, short upper lip and hypertelorbitism. Treatment of CS is overlapped with the beginning of craniofacial surgery and is grounded on morphologic and functional objectives. The authors reported on the outcomes and complications of family members (mother and 2 siblings) with CS, who were operated on by different techniques of frontofacial advancement and have attained skeleta

Outcome of pelvic support osteotomy with the Ilizarov method in the treatment of the unstable hip joint.

Instability of the hip joint in the young adult is a difficult problem. Patients with an unstable hip secondary to any aetiology usually have loss of bone from the proximal femur or shortening of the limb or both. In this study we report our results in the treatment of the unstable hip joint in young adults by pelvic support osteotomy using the Ilizarov method. From 1997 to 2004, 25 patients (17 females and 8 males) with an unstable hip joint were treated in the Orthopaedic department of Mansoura University Hospital, Egypt. Their mean age was 22.4 years (range: 19 to 35). The main complaints w

[Pott's puffy tumour: innovative technique in calvarial reconstruction].

BACKGROUND: Pott's puffy tumour is a rare complication of sinusitis. This osteomyelitis can affect the outer and inner tables of the frontal sinus. The treatment of Pott's puffy tumour combines medical and surgical approaches. Surgical approaches have traditionally been open, but endoscopic techniques have been adopted recently in select cases. The bony defect from debridement can be left alone, or closed with autografts or allografts. OBJECTIVE: To describe a technique for the reconstruction of a large skull vault after the debridement of extensive osteomyelitis of the anterior cranial vault

Desmoplastic Fibroma: A Rare Pathological Midshaft Femoral Fracture Treated With Resection, Acute Shortening, and Re-lengthening: A Case Report.

CASE: We report a rare case of desmoplastic fibroma (DF) of the midshaft femur presenting as a pathological fracture. This rare benign bone tumor was treated with an acute en bloc excision and femoral shortening over an intramedullary nail. Once union of the acute shortening had been achieved, further surgery was undertaken to lengthen the femur with the use of Intramedullary Skeletal Kinetic Distractors. At 3 years after fracture, our patient had achieved equal leg lengths, had normal knee function, and was disease free. CONCLUSIONS: DF resulting in pathological fracture of the midshaft femu

Temporal osteocutaneous island flaps for complete reconstruction of cleft palate defects.

In each of four patients, closure of a wide cleft palate was performed with a temporal osteocutaneous island flap (TOCI flap) based on the superficial temporal artery. These were fabricated island fasciocutaneous flaps composed of temporoparietalis fascia and galea with pericranium, surfaced with split skin grafts. Two flaps carried with them a segment of vascularized parietal bone (TOCI-I). In the other two flaps, the osseus component was supplied by placing an osteogenic powder in a pericranial pocket: a temporal artery "osteogenic" cutaneous flap (TOCI-II). In each case the flap closed the

[Maxillonasal dysostosis: Binder's syndrome].

It is known that the septum of the nose participates in sagittal growth of medial face. Teratogenic deficiency of vitamin K promotes premature ossification of the septum cartilage. It results in hypoplasia of maxillonasal complex, development of characteristic concave profile with the impressed and short nose with half the moon naries, infringement of an occlusion. The orthodontic treatment should be carried out from younger children's age. The surgical treatment is directed on elimination of bone-skeletal infringements of an occlusion, reconstruction of normal contours of middle face and leng

Gain-of-Function of FGFR3 Accelerates Bone Repair Following Ischemic Osteonecrosis in Juvenile Mice.

Bone collapse, bone deformity, and a long treatment period are major clinical problems associated with juvenile ischemic osteonecrosis (JIO). Accelerating the process of bone repair in JIO is expected to shorten the treatment duration and better maintain morphology. We previously indicated that both bone formation and resorption were accelerated following distraction osteogenesis-mediated limb lengthening in genetically engineered mutant mice with a gain-of-function mutation in fibroblast growth factor receptor 3 (FGFR3) gene (i.e., Fgfr3 mice). The purpose of this study was to investigate the

Magnetic resonance imaging evaluation of bone marrow changes in obstructive sleep apnoea syndrome in adults.

PURPOSE: The aim of this study was to evaluate bone marrow reconversion in patients with obstructive sleep apnoea syndrome (OSAS) by means of magnetic resonance imaging (MRI). To our knowledge, no explicit correlation between OSAS and such parameter has been documented in the literature. MATERIALS AND METHODS: During a 35-month period, 33 patients with a clinical diagnosis of OSAS, obese but without restrictive or obstructive ventilatory defects, were evaluated with MRI during wakefulness in all patients with T1-, PD- and T2-weighted sequences in the sagittal and axial plane within 1 week aft

Salvage of failed resection of the distal ulna. Case report.

A 45-year-old woman completely lost the ability of active supination of the forearm after a Darrach resection for malunited fracture of the distal radius. A three-component reconstruction was performed to stabilise the distal stump of the ulna and prevent convergence between the two forearm bones. The procedure combined advancement lengthening osteotomy of the ulna, longitudinal intramedullary tenodesis of the extensor carpi ulnaris tendon, and dorsal transfer of the pronator quadratus through the interosseous space. Four months after the salvage procedure she again had full active supination

Rehabilitation of patients after lower limb replantations by the bone distraction method.

The length of the lower extremities was equalized by the bone distraction method in four patients after replantation of lower legs. A Volkov-Oganesian or Ilizarov distraction apparatus was applied on the replanted lower leg after corticotomy of the tibia and osteotomy of the fibula, and distraction was carried out over 6 to 11 months in small, measured doses. The lower extremities were lengthened from 4 to 8 cm. The lengths of the lower extremities were fully equalized in two patients; they were partially equalized in the other two patients. This made it possible to achieve successful rehabili

Vertical 3D Bone Reconstruction with Simultaneous Implantation: A Case Series Report.

Tooth loss generally leads to a corresponding loss of supporting bone structures, jeopardizing correct implant placement. Bone augmentation procedures facilitate reconstruction of the alveolar contours but lengthen treatment time by about 4 to 9 months. The aim of this case series report is to describe the short-term results of the combination of three-dimensional bone augmentation using the shell technique in conjunction with simultaneous implantation. A total of 10 patients who underwent autologous bone augmentation using the shell technique with simultaneous implantation were retrospectivel

Pilon fractures of the distal tibia.

Pilon fractures are relatively rare fractures of the distal metaphysis of the tibia resulting from axial and/or rotational forces. They involve varying degrees of metaphyseal disruption, articular damage, and malleolar displacement. When severe, these injuries represent a tremendous challenge to the orthopaedic trauma surgeon. Traditional treatment of high-grade pilon fractures by closed means has led to unsatisfactory results. Open reduction and internal fixation in accordance with AO/ASIF principles has greatly improved the outcome in these often disabling fractures. Surgical technique invol

Classification and management of congenital abnormalities of the femur.

Sixty-nine patients with congenital abnormality of the femur were reviewed. Their manifestation of femoral dysplasia ranged from an intact femur approximately 60 per cent of the length of the normal leg to a subtotal absence of the femur in which only the femoral condyles remained, often with a congenital fusion of the knee joint. Two groups were defined: Group I consisted of those with congenital hypoplastic femur in which the hip and knee could be made functional and where, in some patients at least, leg equalisation was possible; Group II consisted of those with true proximal focal femoral

Treatment strategies for osteochondromas in the distal ulna - a multicentre comparative cohort study.

INTRODUCTION: Osteochondromas in the distal ulna of patients with hereditary multiple osteochondromas can cause deformity, radiocapitellar joint incongruency, pain and impaired function. This study compared the long-term outcomes of two different treatment strategies in neighbouring academic hand centres. METHODS: One centre used conservative treatment with secondary reconstruction, excising only symptomatic lesions and performing ulnar lengthening for significant shortening. The other centre carried out prophylactic excision of both symptomatic and asymptomatic distal ulnar osteochondromas u

Rate of Osteonecrosis After Femoral Lengthening with Intramedullary Lengthening Nails.

BACKGROUND: There has been historical concern that the use of intramedullary nails could present undue risk of osteonecrosis of the femoral head due to compromise of the femoral blood supply. Intramedullary lengthening nails are rapidly becoming commonplace in lengthening procedures. As such, the primary objective of this study was to analyze the rates of osteonecrosis following femoral lengthening. The secondary objective was to characterize general trends in femoral lengthening. METHODS: This retrospective cohort study evaluated patients who had undergone femoral lengthening at a single ins
