We present a case with brain abscess associated with entrapment of the lateral ventricle appearing more like remarkable brain edema in the temporo-occipital lobe than ventricular dilatation. A 72-year-old man suffering from headache and vomiting visited our clinic. CT and MRI showed brain abscess in the right parieto-occipital lobe, associated with ventriculitis. Lumbar puncture also revealed purulent meningitis. Both symptoms and CSF findings improved after administration of antibiotics. The improved condition continued for two months after admission, but disturbed consciousness and left hemi…

In 33 year-old man an aneurysm of a pheripheral branch of the right middle cerebral artery was disclosed by angiography 8 days after extirpation of a parietotemporal brain abscess. The aneurysm was absent in the preoperative angiogram. Since the site of the aneutysm coincided with the operative field, it is sure that the etiology of the aneurysm was related to operative trauma and/or arteritis. Although inflammatory or mycotic factor could not be excluded from its etiology, it seems reasonable that traumatic factor was major one. The aneurysm was found to have disappeared eventually in the ang…

We here describe the case of a kidney transplant patient from Mali who presented with multiple complex seizures followed by a facial paresis revealing a brain abscess due to Nannizziopsis obscura species complex. This fungal species is usually implicated in reptile skin infections and rarely disseminated infections. Very rarely, it can infect human beings suffering from immunodepression, causing various kinds of complications ranging from soft tissue abscesses to solid organ abscesses. This case highlights the potential wide range of pathogens involved and underlines the need for greater vigil…

UNLABELLED: Paediatric brain abscess (BA) is a rare but life-threatening condition, and large-scale epidemiological studies are limited. We aimed to characterize the temporal trend of pediatric BA in China during 2016-2023. We conducted a nationwide study using the FuTang Update Medical Records (FUTURE) database, which includes face-sheets of discharge medical records from 33 tertiary children's hospitals across China. Case-identification strategies were validated, and the optimal strategy was applied to identify BA cases. Demographic information, predisposing factors, complications, and clini…

Nocardiosis is a rare bacterial infection caused by a wide array of species of the genus Nocardia. These microbes are found ubiquitously in soil and cause infection mainly when immunocompromised hosts inhale these bacteria or from direct inoculation through breaks in the skin. Once infection is established, Nocardia spp. have a predilection to spread to the central nervous system, causing brain abscesses. Here, we present a rare case of a solitary Nocardia veterana brain abscess in a patient without known immunodeficiency who was successfully treated with an 8-week course of antibiotics. Overa…

In situ hybridization (ISH) staining of bacterial 16S ribosomal RNA (rRNA) is an alternative to standard histological stains (eg, Gram, Warthin-Starry), and may improve the diagnosis of bacterial brain abscesses. To evaluate the utility of 16S rRNA ISH, a 10-year retrospective cohort was assembled from a large academic medical center. Results of histological stains, cultures, and 16S rRNA sequencing were extracted from reports, and new Gram and 16S rRNA ISH stains were performed. Histologically identifiable bacteria were present in 40/63 (63%) cases and 38/57 (67%) were associated with positiv…

BACKGROUND: Infections attributed to Helcococcus kunzii are rarely documented, especially in relation to brain abscesses. This study aims to report the first documented case of a brain abscess associated with tympanitis caused by H. kunzii in China, alongside a comprehensive review of the existing literature.
CASE PRESENTATION: We detail the case of a 54-year-old female patient with a history of hypertension, who was diagnosed with tympanitis complicated by a brain abscess. An urgent occipital lobectomy was performed, during which pus was collected for culture analysis. Three distinct colony …

Fungal infections are an important cause of morbidity and mortality after lung transplantation. While Aspergillus sp. is the most common implicated organism, rare and emerging molds are increasingly reported. We report a case of Rasamsonia argillacea (formerly Geosmithia argillacea) brain abscess in a lung transplant recipient and highlight the unique characteristics associated with this unusual pathogen.

BACKGROUND: Mortality associated with delayed diagnosis of brain abscess is high. A high index of suspicion in conjunction with the use of neuroimaging is important for the early diagnosis of brain abscess. Early use of appropriate antimicrobial and neurosurgical care improves outcomes.
METHODS: We report a fatal case of an 18 year old female with a huge brain abscess who was misdiagnosed as a case of migraine headache over a 4-month period in a referral hospital.
REPORT: An 18-year-old female with a missed history of recent furuncles occurring in the right frontal part of her head and right…

BACKGROUND: Cerebral mucormycosis is an infectious disease of the brain caused by fungi of the order Mucorales. These infections are rarely encountered in clinical practice and are often misdiagnosed as cerebral infarction or brain abscess. Increased mortality due to cerebral mucormycosis is closely related to delayed diagnosis and treatment, both of which present unique challenges for clinicians.
CASE SUMMARY: Cerebral mucormycosis is generally secondary to sinus disease or other disseminated disease. However, in this retrospective study, we report and analyze a case of isolated cerebral muc…

BACKGROUND: Brain abscesses are uncommon but life-threatening in extremely preterm (EP, Gestational Age < 28 weeks) infants. The information of long-time follow-up is rare, but very few cases presented almost intact neural function after injury.
CASE PRESENTATION: We report the clinical course and the outcome of a 27-week preterm infant with multiple brain abscesses. The brain abscesses were detected by cranial magnetic resonance imaging (MRI) and were treated with surgical aspiration twice and a 7-week course of intravenous antibiotics. The patient had two episodes of seizure like activities…

We report the case of a 65-year-old male who presented with a 1-week history of right periorbital pain and progressive visual loss. He had a history of ulcerative colitis and was taking oral corticosteroids and mesalazine. Neurological and radiological examination demonstrated a rare case of invasive fungal rhinosinusitis that began with orbital apex syndrome. Initial endoscopic sinus surgery was performed and fungal culture identified Aspergillus fumigatus. Although antifungal treatment was started empirically before the operation, the patient had improved orbital pain but continued to have d

BACKGROUND: Brain abscesses are relatively rare, but they are a potentially life-threatening condition. Predictive factors for poor outcome are a young age and the presence of multiple abscesses. We report a case of a 15-month-old girl with cyanotic congenital heart disease who developed multiple brain abscesses caused by Streptococcus intermedius. The patient was treated with a combination of surgical aspiration and antimicrobial therapy without apparent neurological sequelae. To the best of our knowledge, this is the youngest such patient to have been reported in the literature. We explore t…

We present the case of a 3-year-old boy who was diagnosed with cerebral abscesses due to Aspergillus nidulans infection on day 28 of induction chemotherapy for acute lymphoblastic leukemia. He responded well to treatment with voriconazole and caspofungin, making a full recovery. There are very few cases of invasive aspergillosis reported in children during induction chemotherapy for acute leukemia and A. nidulans is rare in the absence of chronic granulomatous disease.

Hereditary haemorrhagic telangiectasia (HHT) is a rare inherited autosomal-dominant vascular dysplasia involving multiple organs. Brain abscess is an uncommon and potential fatal complication. We report a case of HHT caused by a novel ENG mutation who initially presented as brain abscess. The patient, with a family history of epistaxis, presented with fever, headache and right-sided haemiparesis. Upon examination, brain MRI showed a contrast-enhanced abscess on the left fronto-parietal region. Open brain drainage was performed and pus culture yielded Actinomyces meyeri. The chest image reveale…
