Evaluation of a Novel, Bedside, Ultrasonographic Neuronavigation System Versus Free-Hand Ventricular Targeting: A Double-Blinded, Prospective, Paired Comparison Study.

BACKGROUND AND OBJECTIVES: External ventricular drain (EVD) placement is a common neurosurgical procedure. Inaccurate placement can lead to significant complications and neurological morbidity. We present a prospective, repeated-measures experiment evaluating a novel, ultrasonographic neuronavigation device designed to improve first-pass EVD accuracy and reduce complications. METHODS: A total of 25 volunteers ranging from first-year medical students to neurosurgical attendings with over 40 years of neurosurgical experience placed titanium wires imitating EVD stylets into 3-dimensional-printed

Rapidly Trading Down Depression's 3 Pillars to 5HT3-Receptors Through ECT or Psilocybin?

Depression astonishingly can be stopped instantly by electrotherapies or through some psychedelics like psilocybin. In explaining this, the traditional approaches to their antidepressant effects via "reset" models and orthosteric serotonin receptors has neglected the only serotonin channel 5HT3, which e.g. has emerged as being helpful for the neurotrophic translation for all anti-depressants and final synaptic effects. Psychedelics here are confronted with a panorama of also anti-depressant 5HT3-channels and a search for their part e.g. in the "3 pillars" reigning depression. Of these M1) mito

Association of intracranial arteriovenous malformation embolization with more rapid rate of perfusion in the peri-nidal region on color-coded quantitative digital subtraction angiography.

BACKGROUND: Hemodynamic alterations post-embolization of intracranial arteriovenous malformations (AVMs) may cause delayed edema/hemorrhage in brain parenchyma adjacent to the lesion. OBJECTIVE: To quantify and compare cerebral perfusion changes in the peri-AVM territory pre- and post-embolization using color-coded quantitative digital subtraction angiography (q-DSA). METHODS: Pediatric intracranial AVM embolization procedures performed over a 5 year period were included. DSA images of all patients were retrospectively assessed using syngo iFlow. Regions of interest (ROI) were selected on an

Vein of Galen Malformation with Pial Arteriovenous Fistula in a 12-month-old Child.

Pial arteriovenous fistulas are extremely rare vascular malformations of the brain (accounting for approximately 1.6% of vascular malformations of the brain) and are classified as Galenic and non-Galenic pial fistulas. We present a rare association of a mural-type vein of Galen malformation with a pial fistula involving the precentral branch of the right middle cerebral artery (MCA) in a 12-month-old infant. A 12-month-old preterm male infant, born vaginally with a delayed cry (birth weight 1.2 kg), presented with progressive macrocephaly, recurrent upper respiratory tract infections, developm

Occipital Pial AVM Rupture in a Young Adult: Dual Intranidal Aneurysms, Solitary Parasagittal SSS Drainage, and Hematoma-Corridor Microsurgical Cure.

Background and Clinical Significance: Focal hemorrhagic severity associated with posterior convexity pial brain arteriovenous malformation (AVM) cases can be exacerbated by hemodynamic stress focusing on focal areas of architectural weakness and by superficial venous outflow being restricted by non-redundant superficial venous drainage. This clinical case report exemplifies how bedside neurologic localization and angioarchitectural characteristics can inform the selection of microsurgical approaches for the treatment of ruptured AVMs that are directed at reducing hemorrhage recurrence risk thr

Anticoagulant Use during Subacute Asymptomatic Cerebral Cavernous Malformation Hemorrhage.

INTRODUCTION: Several cohort analyses and 2 meta-analyses show a lower risk of cerebral cavernous malformation (CCM) hemorrhage in patients taking anti-thrombotics than those without. Most of the reported patients were taking antiplatelet agents rather than anticoagulants, and little is reported on the use of anticoagulation in the setting of an acute CCM hemorrhage. CASE PRESENTATION: We present 3 patients from our practice requiring anticoagulation in the setting of CCM hemorrhage. Patient 1 (age: 61 years) was hospitalized with headache and right arm and leg numbness after a COVID-like ill

Engineered lymphatic stroma model applications in central nervous system leukemia.

Central nervous system (CNS) involvement frequently occurs in acute lymphoblastic leukemia (ALL), but many questions remain about how leukemia cells access, persist in, and exploit the CNS. The CNS is protected by the meninges, which are fluid-filled membranes that surround the brain. Within the meninges, meningeal lymphatic vessels, located within the dura mater, drain cerebrospinal fluid to the deep cervical lymph nodes for immunosurveillance, providing a potential pathway for leukemia cells to enter or exit the CNS. Here, we utilized tissue engineered models of the meningeal lymphatics and

Intra-hospital MRI transport in neurocritical patients with aneurysmal subarachnoid hemorrhage: complications and clinical impact with predominant continuation of care.

Introduction Magnet Resonance Imaging (MRI) is essential for neurocritical care but requires intrahospital transport (IHT) in patients treated in an intensive care unit, which carries significant risks. This study assesses the risk profile and whether the benefits of MRI in aneurysmal subarachnoid hemorrhage (aSAH) outweigh the associated complications of IHT. Method In this retrospective study, all aSAH patients treated in our neurocritical care unit (NCCU) between 2016 and 2023 were screened. Data collection included baseline demographics, hemorrhage severity scores, aneurysm treatment strat

Effectiveness of low-molecular-weight heparin versus unfractionated heparin for venous thromboembolism prophylaxis after firearm-related penetrating brain injury.

OBJECTIVE: Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, is a major source of morbidity and mortality following firearm-related penetrating brain injury (PBI). Standard pharmacological prophylaxis against VTE includes the use of low-molecular-weight heparin (LMWH) or unfractionated heparin (UH). However, the comparative effectiveness of LMWH versus UH to prevent VTE in this population remains unclear. METHODS: A retrospective analysis was conducted using data from the Trauma Quality Improvement Program (2017-2019) to evaluate the effectiveness of LMWH ve

Noninvasive Method Using Mechanical Extensometer for the Estimation of Intracranial Compliance by Repeated Measures Agreement Analysis.

BACKGROUND: Intracranial compliance refers to the relationship between changes in volume and the resultant changes in intracranial pressure (ICP). This study aimed to assess the agreement of a noninvasive ICP waveform device for the estimation of compliance compared with invasive ICP monitoring employing three distinct methods. METHODS: We conducted a retrospective analysis of ICP waveform morphology recorded through both invasive (external ventricular drain) and noninvasive (mechanical extensometer) methods in adult patients with acute brain injury admitted to a neurointensive care unit betw

The acute management of intracerebral hemorrhage: a clinical review.

Intracerebral hemorrhage (ICH) is a devastating disease with high rates of mortality and morbidity. The major risk factors for ICH include chronic arterial hypertension and oral anticoagulation. After the initial hemorrhage, hematoma expansion and perihematoma edema result in secondary brain damage and worsened outcome. A rapid onset of focal neurological deficit with clinical signs of increased intracranial pressure is strongly suggestive of a diagnosis of ICH, although cranial imaging is required to differentiate it from ischemic stroke. ICH is a medical emergency and initial management shou

Extended replacement of aorta in chronic type A aortic dissection.

BACKGROUND: Chronic type A dissection is itself an indication for urgent surgery. Usually, replacement of the ascending aorta alone is adequate. Arch and descending aortas are replaced only in rare occasions-impending rupture, dilatation with diameter >5 cm. Extended replacement of the proximal aorta is associated with high morbidity and mortality. Staged repair is usually recommended by many centers. The disadvantages are the requirement for two stage operations, rupture of descending aorta in the interim period and patient denial to have the second stage. The authors use one stage operation

Safety of perioperative minidose heparin in patients undergoing brain tumor surgery: a prospective, randomized, double-blind study.

OBJECT: Thromboembolic phenomena (TEPs) continue to be a significant source of morbidity and mortality in patients undergoing neurosurgery. Although the efficacy of low-dose heparin in preventing TEPs in neurosurgical patients is well established, neurosurgeons are reluctant to use it perioperatively because of concern for increased bleeding complications. To clarify this issue, the authors used a prospective, randomized, double-blind design to evaluate the safety of minidose heparin treatment in patients undergoing surgery for supratentorial brain tumors. METHODS: One hundred three patients,

Improving Outcomes in Paediatric Patients With Posterior Fossa Tumours: The Rapid Assessment Pathway in Resource-Limited Settings.

INTRODUCTION: Posterior fossa tumours present substantial clinical challenges due to their deep anatomical location and proximity to vital neurovascular structures. Limited awareness and diagnostic delays remain major obstacles to early detection of paediatric brain tumours. For the last two decades, the 5-year overall and event-free survival of children with posterior fossa tumours has been doubled due to the improvement in the diagnostic tools and the advances in the surgical techniques approaching total or near-total resection. MATERIALS AND METHODS: This retrospective comparative cohort s
