First trimester isolated maternal hypothyroxinaemia: adverse maternal metabolic profile and impact on the obstetrical outcome.

BACKGROUND: Isolated maternal hypothyroxinaemia (IH) is defined as low maternal FT4 (<5th percentile) and normal thyroid-stimulating hormone … 0·003), in the first trimester (P = 0·004) and at the time of delivery (P = 0·001). At term, foetal breech presentation and caesarean section rate were significantly higher (P = 0·006 and P = 0·026, respectively) than in the euthyroid controls. A significant increase in macrosomia was also noted (P = 0·026). CONCLUSION: The prevalence of hypothyroxinaemia in early p …

Congenital knee dislocation: conservative management of a series of cases.

Analyzing the stagnation of cesarean delivery rates in the United States over the past 15 years.

Identifying key factors behind persistently high cesarean rates over the past 15 years is challenging due to inconsistent documentation of primary indications, overlapping risk factors, and evolving clinical standards. The modified Robson classification helps standardize comparisons across populations. Cesarean rates by modified Robson class were compared using National Vital Statistics data from 2009, when the cesarean rate peaked, to the most recent data available in 2023. In 6 of the 11 categories, the rate of cesarean decreased: nulliparous spontaneous, nulliparous induced, multiparous spo

Routine maneuvers in eutocic breech vaginal delivery at term: A prospective cohort study.

OBJECTIVE: To study neonatal and maternal outcomes associated with routine maneuvers in breech vaginal delivery at term. METHODS: This was a secondary analysis of the multicenter PREMODA observational prospective study in France and Belgium. We included women with vaginal breech delivery, excluding those who underwent maneuvers to resolve a dystocic delivery. Maternal data and characteristics of labor, in addition to neonatal and maternal outcomes, were recorded. We defined two groups according to mode of delivery; breech vaginal delivery with or without routine maneuvers, and we compared the

Reverse Breech Extraction Vs Head Pushing For Delivery Of Deeply Impacted Fetal Head In Emergency Caesarean Section.

BACKGROUND: Obstructed labour is considered a negligible component of maternal mortality in developed countries but it is a major mortality burden in developing countries. This study was done to compare maternal outcome associated with reverse breech extraction and vaginal head pushing method for the deeply impacted foetal head in emergency caesarean section. METHODS: It was done at the Department of Obstetrics and Gynaecology Military Hospital, Rawalpindi from May to Nov 2014. A total of 110 patients meeting our inclusion criteria were randomly divided into two groups, i.e., Group A (deliver

Experience and confidence in vaginal breech and twin deliveries among obstetric trainees and new specialists in Australia and New Zealand.

BACKGROUND: The number of vaginal breech and twin deliveries may be insufficient for adequate training. OBJECTIVE: To determine whether advanced trainees and new Fellows in obstetrics are gaining adequate experience and confidence in vaginal breech and twin deliveries. METHODS: An online survey was emailed to registered Royal Australia and New Zealand College of Obstetrics and Gynaecology (RANZCOG) advanced trainees and new Fellows (Years 1-5). This survey asked about their experience, confidence and whether they intend to perform vaginal breech and twin deliveries as specialists. RESULTS:

Intrapartum intervention rates and perinatal outcomes following successful external cephalic version.

OBJECTIVE: To determine intrapartum and perinatal outcomes following successful external cephalic version for breech presentation at term. STUDY DESIGN: This was a retrospective cohort study of outcomes following successful external cephalic version in 411 women at an Australian tertiary maternity unit between November 2008 and March 2015. The study cohort was compared with a control group of 1236 women with cephalic presentation who underwent spontaneous labor. Intrapartum intervention rates and adverse neonatal outcomes were compared between both groups. RESULTS: The success rate of extern…

To ECV or not to ECV? The current evidence base concerning external cephalic version.

External cephalic version (ECV) is the technique of attempting to turn a baby in the womb from a head-up to a head-down position. The practice is grounded on evidence that vaginal breech birth (VBB) presents greater short-term risks for babies than caesarean section (CS) (Hofmeyr et al 2011), but that labour and vaginal birth also offer benefits to both mothers and babies. Therefore, if we can turn babies to a head-down position, we can reduce the risks associated with both VBB and CS, and enable mother and baby to benefit from labour and birth.

Long-term neurologic outcome of infants born by cesarean section.

Prevention of neurologic injury to the fetus through skilled and attentive care during the peripartum period is designed to identify signs of fetal distress so that appropriate obstetric interventions can occur. The impact of mode of delivery on neurologic outcome varies depending on the clinical indication for cesarean delivery and the associated maternal and fetal conditions. This review summarizes current knowledge of the impact of mode of delivery on long-term neurologic outcome.

Factors associated with maternal morbidity in the Term Breech Trial.

OBJECTIVE: In the Term Breech Trial, the risk of maternal morbidity in women who delivered after planning for a caesarean section (CS) was not significantly different from those who delivered after planning for a vaginal birth. We undertook secondary analyses to determine factors associated with maternal morbidity among 2078 women. METHODS: By using multiple logistic regression analyses, we determined the effect of prelabour CS, CS during early labour, CS during active labour, vaginal birth, and other factors on maternal morbidity. For 1536 women delivered after labour, we determined the effe

[Incidence of and factors associated with primary cesarean section in Benin (Africa)].

Among 3225 deliveries in two Cotonou public maternity units, 244 primary cesarean section were observed during July 1988 to December 1989. Primary cesarean section incidence varied according to indications, maternal parity and maternity care units. Foetal distress (65.9%), malpresentation and cephalopelvic disproportion (22.1%) were the dominant organic risk factors for the primary cesarean section. Multivariate stepwise logistic regression technique indicated that the most important indications for the practice in Cotonou were mainly organic risk factors, maternal height and parity as well as

Traumatic parturitional intracranial hemorrhage.

Twenty-five cases of intracranial hemorrhage due to injury at birth, encountered over a seven-year period, are presented. CT scans showed many different pathways of spread of tentorial hemorrhage; the likelihood of an arterial source is emphasized. Recurrent or delayed bleeding was documented twice. Surgical evacuation of hematomas was necessary in 10 cases, seven involving hemorrhage into the posterior fossa. There were three deaths, one due to pulmonary hemorrhage complicating subtentorial subdural hematoma, another due to severe central parenchymal hemorrhage and a third because of postoper
