[Graft dysfunction, acute rejection and bronchiolitis obliterans in lung and heart-lung transplantation].

Three complications which influence both survival and quality of life in transplanted patients will be the object of this chapter. Graft dysfunction: this is a severe re-implantation oedema leading to inefficiency of the graft as regards haemostasis whether or not associated with haemodynamic complications. The liberation of free radicals and/or cytokines induced by ischemia-reperfusion of the graft plays an important role in the pathogenesis of this syndrome. Acute rejection: the mechanism is complex leading to the intervention of an immune response stimulated by the detection of allo-antigen

[A case of rheumatoid arthritis associated with pernicious anemia and bronchiolitis obliterans organizing pneumonia].

A case of rheumateid arthritis (RA) with pernicious anemia (PA) and wandering multiple patchy densities in bilateral lung fields is reported. A 72-year-old woman was hospitalized in February 1994, because of cough. She had already advanced RA (Class IV, Stage IV). She showed macrocytic and hyperchromic anemia as follows ; red-cell count (RBC), 176 x 10(4)/microliters; hemoglobin (Hb),7.2 g/dl; hematocrit (Ht), 21.0% ; MCV, 119.3 fl; and MCH, 40.9 pg. Chest roentgenogram revealed multiple patchy densities in bilateral lung fields and there was no response to the administration of antibiotic age

Bronchiolitis obliterans organizing pneumonia (BOOP): the cytological and immunocytological profile of bronchoalveolar lavage.

The cytological and immunocytological profile of bronchoalveolar lavage (BAL) was studied in 10 patients with idiopathic bronchiolitis obliterans organizing pneumonia (BOOP) and compared with the data in idiopathic pulmonary fibrosis (IPF) (n = 22), chronic eosinophilic pneumonia (CEP) (n = 9), and extrinsic allergic alveolitis (EAA) (n = 24). Lymphocyte subsets were enumerated using an immunoperoxidase slide assay. The BAL pattern in BOOP patients was characterized by several features: 1) colorful cell differentials with an increase in all cell types, most markedly in lymphocytes, and more mo

The importance of acquired diffuse bronchomalacia in heart-lung transplant recipients with obliterative bronchiolitis.

The results of heart-lung transplantation are improving with increasing experience in postoperative management, but obliterative bronchiolitis may still develop late postoperatively. We have performed 19 heart-lung transplants, with 1-month, 1-year, and 2-year actuarial survival rates of 95% +/- 5%, 84% +/- 8%, and 69% +/- 16%, respectively. Three early recipients died of bronchiolitis, and four patients who were operated on more than 2 years ago are currently being followed up with bronchiolitis. Since August 1988, 13 surviving recipients have undergone serial postoperative bronchoscopies and

[Bronchiolitis obliterans organizing pneumonia (BOOP). Characteristics and boundaries of an anatomo-clinical entity].

Bronchiolitis obliterans organizing pneumonia (BOOP) is defined by endoluminal obstruction of distal airspaces by a fibrous granulation tissue consisting of inflammatory cells, fibroblasts, and connective tissue. The morphogenesis of the process is the following: diffuse alveolar injury leading to basement membrane denudation and intra-alveolar exudate of fibrinogen, immunoglobulins, coagulation factors, fibronectin; intra-alveolar migration of interstitial fibroblasts through gaps of the epithelial basement membrane; intra-alveolar secretion of loose connective tissue (codistribution of colla

Intravenous cyclophosphamide and methylprednisolone for the treatment of bronchiolitis obliterans and interstitial fibrosis associated with crysotherapy.

A patient with rheumatoid arthritis receiving intramuscular gold developed severe pulmonary insufficiency. Histopathology revealed both bronchiolitis obliterans and interstitial fibrosis possibly related to gold therapy. The patient was successfully treated with IV cyclophosphamide and prednisone. During the course of therapy, hypogammaglobulinemia without detectable IgM developed.

Continuing respiratory problems three and a half years after acute viral bronchiolitis.

We reviewed the clinical progress of 81 children as part of a prospective study three and a half years after admission to hospital with acute viral bronchiolitis in infancy. Fifty six (69%) reported episodes of lower respiratory symptoms continuing over the preceding year, 25 (31%) had symptoms lasting for longer than two weeks on two or more occasions, 14 (17%) had symptoms for more than 100 days, and six (7%) required readmission to hospital with acute respiratory illness. Two years previously, these percentages had been 82%, 36%, 33%, and 13% respectively. Forty six (57%) children were said

Slowly progressive bronchiolitis obliterans. A case report with detailed pulmonary function studies.

A case of idiopathic bronchiolitis obliterans in a 40-year-old man is presented. The chest radiograph showed miliary nodules, which on histological examination were found to consist of granulation tissue in and around bronchioles causing narrowing or obliteration of the airway lumen. The pulmonary function tests indicated a milk degree of airway obstruction, ventilation/perfusion inequality, hypoxemia which increased during exercise, and a well-preserved diffusion capacity suggesting absence of significant damage to the interstitium of the lung. Radiospirometry suggested that the functional de…

Bronchiolitis Severity Scores for Predicting Mechanical Ventilation Needs in Children With Respiratory Syncytial Virus.

BACKGROUND: Respiratory syncytial virus (RSV) bronchiolitis is a common cause of respiratory failure requiring pediatric intensive care unit (PICU) admission in young children. Predicting the need for mechanical ventilation remains challenging. We evaluated the usefulness of bronchiolitis severity scores for predicting mechanical ventilation after PICU admission. METHODS: We conducted a single-center retrospective study of children with RSV bronchiolitis admitted to our PICU between April 2021 and July 2023. Inclusion criteria were age < 24 months, no underlying respiratory disease, and no pr

Prioritising Recommendations for Conversion to Living Mode in the Australasian Bronchiolitis Guideline: A Modified Delphi Study.

BACKGROUND: The Australasian Bronchiolitis Guideline (ABG) provides evidence-based recommendations for managing one of the most frequent reasons for infant hospitalisation. As traditional guidelines age, they suffer from lack of relevance when new evidence is available but not incorporated. To address this limitation a guideline, or parts thereof, can be converted into a "living" mode, where new evidence is regularly incorporated into recommendations, enabling faster translation to practice. This study aimed to prioritise ABG recommendations for conversion to a living mode. METHOD: Members of

Global trends and advances in pediatric bronchiolitis treatment in the last two decades: a bibliometric analysis.

BACKGROUND: Bronchiolitis, a leading respiratory disease in infants, causes substantial global morbidity. This is the first study to employ bibliometric analysis to explore global trends and progress in pediatric bronchiolitis treatment, addressing a previously unexamined gap. METHODS: A comprehensive literature search was conducted in the Web of Science Core Collection on January 31, 2025, encompassing publications between 2004 and 2024. The bibliometric analysis utilized VOSviewer, CiteSpace, and the R package "bibliometrix" for data visualization, network analysis, and the identification o

Optimizing the Management of Pediatric Bronchiolitis: A Multi-cycle Clinical Audit at a Regional Hospital in Oman.

Background Bronchiolitis is a major cause of lower respiratory tract infection and hospital admission among infants. Although international guidelines consistently recommend supportive care as the primary management approach, significant variation in clinical practice persists, often resulting in avoidable investigations, treatments, and healthcare expenditure, contributing to unnecessary costs and potential patient harm from avoidable interventions. Objective The objective of this study is to evaluate the effectiveness of a multi-cycle clinical audit in improving adherence to evidence-based b

High Flow Nasal Cannula Use in Bronchiolitis: Predicting Need for Early Escalation of Care.

OBJECTIVES: High-flow nasal cannula (HFNC) use in bronchiolitis has increased substantially with wide variability, likely due to the lack of objective means of predicting clinical outcomes. Our objective was to identify features associated with care escalation for infants started on HFNC in the Emergency Department (ED), thereby assisting in disposition of patients and optimizing the utilization of limited resources. METHODS: This is a retrospective cohort study from 3 free-standing children's hospitals of infants younger than or equal to 12 months with bronchiolitis who were initiated on HFN

Multi-modal analysis of vascular remodelling in human obliterative bronchiolitis after lung transplantation.

Chronic lung allograft dysfunction (CLAD) is the leading cause of late morbidity and mortality after lung transplantation. Bronchiolitis obliterans syndrome (BOS), a major CLAD phenotype, is characterized by obliterative bronchiolitis (OB). While microvascular perturbations are described in BOS, their cellular and molecular characteristics remain unclear. Intact human CLAD lungs (n=3) underwent hierarchical phase-contrast tomography (HiP-CT) for three-dimensional vascular mapping. In OB-affected airways vs. controls (n=4 each), endothelial subtypes were characterized by multiplex immunofluores
