Ventricular activation and repolarization in response to physiological and conventional pacing using ultra-high-frequency electrocardiography.

BACKGROUND: Physiological pacing targeting the cardiac conduction system is increasingly being adopted as an alternative to conventional right ventricular (RV) pacing for the treatment of bradyarrhythmias, although its effects on ventricular repolarization remain underexplored. OBJECTIVE: This study evaluates depolarization and repolarization responses to different pacing techniques using ultra-high-frequency electrocardiograms (UHF-ECGs). METHODS: Temporary pacing was performed at different cardiac areas in 178 patients with bradycardia. Depolarization was assessed via QRS duration (QRSd),

[History of cardiac resynchronization therapy : 30 years of electrotherapeutic management for heart failure].

The first permanent biventricular pacing system was implanted more than 30 years ago. In this article, the historical development of cardiac resynchronization therapy (CRT), starting with the pathophysiological concept, followed by the initial "proof of concept" studies and finally the large prospective-randomized studies that led to the implementation of CRT in heart failure guidelines, is outlined. Since the establishment of CRT, both an expansion of indications, e.g., for patients with mild heart failure and atrial fibrillation, but also the return to patients with broad QRS complex and lef

Left Bundle Branch Pacing: A Paradigm Shift in Physiological Pacing for Patients With Atrioventricular Block and Preserved Left Ventricular Systolic Function, A Systematic Review and Meta-analysis.

His-Purkinje conduction system pacing (HPCSP) via His bundle pacing (HBP) and Left Bundle Branch Pacing (LBBP) offer a physiological approach to pacing by restoring normal ventricular activation. This meta-analysis compares the feasibility, outcomes, and success rates of HBP and LBBP in patients with atrioventricular block (AVB) and preserved left ventricular function. A systematic search identified studies comparing LBBP with HBP in AVB patients with preserved systolic function. Primary outcomes included QRS duration, success rates, pacing threshold, and improvement in R-wave amplitudes. Seco

Bundle Branch Reentrant Ventricular Tachycardia Treated with Catheter Ablation in a Patient with Myotonic Dystrophy.

BACKGROUND Bundle branch reentrant ventricular tachycardia (BBRVT) is a rarely encountered ventricular tachycardia (VT) and is classically associated with advanced heart diseases. Importantly, the tachycardia is readily curable with catheter ablation. Without suspicion of BBRVT and recording of the His-Purkinje system, it is hard to diagnose accurately. Myotonic dystrophy (MD) is the most common neuromuscular disease in adults and is known to have a risk of development of BBRVT. Here, we report a case of BBRVT in an MD patient with normal cardiac configuration with typical clinical and electro

The electrocardiogram characteristics and pacing parameters of permanent left bundle branch pacing: a systematic review and meta-analysis.

PURPOSE: Recent advances in conduction system pacing have led to the use of left bundle branch pacing (LBBP), which has potential advantages over His bundle pacing (HBP). For example, LBBP engages the electrical activation through the left bundle branch, produces ventricular electrical synchrony, and avoids the weakness of HBP such as lead instability, higher threshold, and early battery depletion. This pacing modality has been considered an attractive mode to achieve normal physiological pacing. However, as a new technology, LBBP is still in the stage of clinical exploration and lacks adequat

Slow pathway elimination using antegrade conduction improvement with fast atrial pacing during AVNRT radiofrequency ablation: a proof-of-concept study.

BACKGROUND: Radiofrequency (RF) ablation of slow pathway (SP) is usually performed in sinus rhythm while monitoring the occurrence of a slow junctional rhythm (JR). JR although sensitive, is not specific for elimination of SP conduction. Our objective was to prospectively evaluate feasibility and safety of SP elimination using fast atrial rate pacing (FAP) during RF delivery. METHODS: Consecutive patients admitted for atrioventricular nodal re-rentrant tachycardia (AVNRT) ablation were included. The rate of proximal coronary sinus (CS) pacing was set to a value constantly yielding antegrade S

[Electrotherapy of supraventricular tachycardia (atrial fibrillation/atrial flutter)].

Contrary to patients with the WPW-syndrome and AV nodal reentry in whom radiofrequency catheter ablation is the therapy of first choice for cure from these arrhythmias instead of life-long medical therapy, pharmacologic therapy of atrial fibrillation and atrial flutter remains the treatment of choice in these patients. If, however, atrial fibrillation with rapid atrioventricular conduction is medically intractable and associated with severe haemodynamic impairment, these patients may be offered catheter ablation of AV conduction by radiofrequency energy. Compared to DC ablation, this approach

Catheter ablation of the atrioventricular junction and activity responsive pacing. Effect on refractory atrial fibrillation with hypertrophic cardiomyopathy.

A 50-year-old man had chronic atrial fibrillation that was refractory to conventional therapy. He was intolerant of amiodarone, but successfully managed by transcatheter atrioventricular junction ablation. Activity-initiated rate-responsive ventricular pacing resulted in a fourfold greater increase in cardiac output with exercise compared to fixed rate pacing.

[Total or partial electrical interruption of the atrioventricular pathway. Evaluation of clinical results and indications].

After checking on the efficacy and innocuity of catheter ablation of the His bundle in an experimental study in the sheep, 22 patients with supraventricular tachycardia resistant to an average of 5.5 +/- 1.7 antiarrhythmic drugs per patient underwent this procedure. The most common arrhythmias were atrial fibrillation or flutter (poorly tolerated in 14 cases), and reciprocating tachycardia (8 cases, including 3 with accessory atrioventricular pathways). In 3 of the 5 patients with intranodal tachycardia, an initial attempt was made to modify one of the two AV nodal conduction pathways. Althoug

Delayed termination of re-entrant atrioventricular nodal tachycardia.

Termination of atrioventricular nodal (AVN) re-entrant tachycardia by one or two induced premature beats generally occurs within one tachycardia cycle from the last premature beat. Two cases are described in which programmed stimulation during sustained re-entrant AVN tachycardia caused delayed termination in the second or third tachycardia cycle following the extrastimuli. The site of block was the antegrade pathway in one case and the retrograde pathway in the other. The most likely mechanism was induced second degree block in one limb of the tachycardia circuit. Delayed termination provided

Unusual mechanism in the initiation of the paroxysmal supraventricular tachycardia in a patient with WPW syndrome.

A heretofore unreported unusual mechanism in the initiation of the paroxysmal supraventricular tachycardia (PSVT) in a patient with WPW (Wolff-Parkinson-White) syndrome was observed using His bundle recordings. The patient initially had some degree of AV conduction disturbance at the level of the AV node. A premature atrial impulse initially activated the ventricle exclusively through the accessory pathway and the same impulse re-excited the ventricle via the AV nodal-His axis after finishing the pure pre-excitation with marked prolongation of the AH and the HV intervals. After finishing this

Paroxysmal supraventricular tachycardia with unusual induction. Concealed reentry or automaticity?

In a patient with documented paroxysmal supraventricular tachycardia, earlier atrial extrastimuli consistently induced His-ventricle (H-V) block and "atrioventricular junctional" beats, which were always followed by an echo or paroxysmal supraventricular tachycardia. In "atrioventricular junctional" beats, a V wave was preceded by an H deflection with the same intervals as that of the sinus beat. As for the underlying mechanism of paroxysmal supraventricular tachycardia, two possibilities were considered: (1) concealed atrioventricular nodal reentry, and (2) "triggered automaticity."

[Problems of rehabilitation during permanent cardiac stimulation].

The importance of certain features of the classification of diseases of the His system for the purpose of planning and conducting permanent cardiac stimulation (PCE). Stability of the conduction disturbance is essential in classifying low stroke volume syndromes as due to sporadic, frequent or habitual disturbance, and the frequency of stimulation should be geared to that of the syndrome. Rehabilitation during PCE also has its psychological side. Indeed, this almost always surpasses the physical side in importance and is dependent on the patient's job, social and economic standing, and the siz
