Common sports injuries seen by the primary care physician. Part II: Lower extremity.

Sports medicine is the science of caring for the medical and surgical needs of athletes and their injuries. Injuries of the upper extremity were dealt with in Part I in a previous article. Part II deals with injuries of the lower extremity. Trochanteric bursitis and hamstring strains are treated with rest, rehabilitation, and correction of training errors. Patellofemoral pain syndromes require accurate diagnosis and usually a rehabilitative program. Injuries to the medial collateral ligament are very common, but can be associated with tears of the meniscus and cruciate ligaments. The latter tw…

Technique and results of Achilles tendon detachment and reconstruction for insertional Achilles tendinosis.

BACKGROUND: Insertional Achilles tendinosis is a clinical entity that commonly occurs with other posterior heel disorders such as retrocalcaneal bursitis, Haglund deformity, intratendinous ossification and pretendinous bursitis. Complete detachment and reconstruction of the Achilles tendon was evaluated as a method of treatment for this condition. METHODS: Seventy-five patients (81 heels) were treated over a 5-year period for chronic insertional Achilles tendinosis. These were divided in two groups: a nondetached group (26 patients, 31 heels, average age 55 years) included all patients with d…

Culture-negative osteoarticular infection caused by Legionella sainthelensi.

Legionella non-pneumophila species are uncommon causes of human infection and more particularly in extra-pulmonary sites. Osteoarticular involvement remains exceptional and represents a major diagnostic challenge, as routine diagnostic tools are mainly designed to detect L. pneumophila and culture requires specific enriched media. We report a case of severe upper-limb osteoarticular infection caused by Legionella sainthelensi following corticosteroid injection, involving bursitis, deep abscesses and synovitis. Identification was challenging and ultimately achieved through molecular analysis of…

Disorders of the Peritrochanteric and Deep Gluteal Space: New Frontiers for Arthroscopy.

Arthroscopic techniques for the hip joint have evolved into endoscopic methods for extra-articular disorders. These endoscopic strategies provide a less invasive alternative to open procedures for traditionally recognized forms of pathology. Endoscopy has defined new disorders amenable to surgical correction and has redefined some of these existing disorders. The peritrochanteric and deep gluteal regions represent 2 of the most currently active areas of exploration. Peritrochanteric problems include trochanteric bursitis, full-thickness and partial-thickness tears of the abductors including th…

[Greater trochanteric pain syndrome].

Greater trochanteric pain is one of the common complaints in orthopedics. Frequent diagnoses include myofascial pain, trochanteric bursitis, tendinosis and rupture of the gluteus medius and minimus tendon, and external snapping hip. Furthermore, nerve entrapment like the piriformis syndrome must be considered in the differential diagnosis. This article summarizes essential diagnostic and therapeutic steps in greater trochanteric pain syndrome. Careful clinical evaluation, complemented with specific imaging studies and diagnostic infiltrations allows determination of the underlying pathology in…

Living with frozen shoulder. 'Here are the risks. I want the injection'. An interpretative phenomenological analysis.

BACKGROUND: Frozen shoulder is a disabling condition characterised by severe pain and loss of shoulder movement and may affect up to 5% of the population. Qualitative research documents debilitating pain and how treatment to reduce pain is a priority for people diagnosed with frozen shoulder. Corticosteroid injections are a principal treatment to reduce the pain of frozen shoulder, however little is known about the patient experience. OBJECTIVES: This study aims to address this gap in the knowledge by exploring the lived experience of people with frozen shoulder who have undergone an injectio

Imaging of Soft Tissue Infections.

Although superficial infections can often be diagnosed and managed clinically, physical examination may lack sensitivity and specificity, and imaging is often required to evaluate the depth of involvement and identify complications. Depending on the area of involvement, radiography, ultrasound, CT, MR imaging, or a combination of imaging modalities may be required. Soft tissue infections can be nonnecrotizing or necrotizing, with the later having a morbid and rapid course. Infectious tenosynovitis most commonly affects the flexor tendon sheaths of the hand, characterized by thickened and enhan

Differentiating Achilles Insertional Calcific Tendinosis and Haglund's Deformity.

Calcification of the posterior portion of the calcaneus has numerous terms that refer to this pathology. Given the number of names, there can be confusion when discussing the different pathologies involving calcification at the insertion of the Achilles tendon at the calcaneus. Two of the diagnosis that can be confused with each other are Haglund's deformity and Achilles insertional calcific tendinosis. This article discusses how these 2 entities are differentiated clinically and how their surgical management is different.

Capsular Injury and Inflammation.

The capsular and ligamentous structures of the glenohumeral joint are important for stability of the shoulder. These structures are best evaluated by MR imaging. Familiarity with normal and abnormal appearance of the capsular structures of the shoulder is important to ensure that important pathology is not overlooked. Injury to the capsular structures can occur in the setting of trauma and most commonly involves the inferior glenohumeral ligament and axillary pouch. Adhesive capsulitis is a common inflammatory condition with characteristic imaging features that should be considered in the abse

Management of Snapping Scapula Syndrome.

Snapping scapula syndrome is a rare condition caused by the disruption of the gliding articulation between the anterior scapula and the posterior chest wall. The etiology of snapping scapula syndrome is multifactorial, and contributing factors include scapular dyskinesis, bursitis from repetitive use or trauma, and periscapular lesions. Although the majority of cases are initially treated with nonoperative modalities, recalcitrant snapping scapula syndrome can warrant surgical management. This report describes a 34-year-old amateur weight lifter with a 1-year history of increasing pain and ful…

Primary frozen shoulder: brief review of pathology and imaging abnormalities.

BACKGROUND: Primary frozen shoulder (FS) is a painful contracture of the glenohumeral joint that arises spontaneously without an obvious preceding event. Investigation of the intra-articular and periarticular pathology would contribute to the treatment of primary FS. REVIEW OF LITERATURE: Many studies indicate that the main pathology is an inflammatory contracture of the shoulder joint capsule. This is associated with an increased amount of collagen, fibrotic growth factors such as transforming growth factor-beta, and inflammatory cytokines such as tumor necrosis factor-alpha and interleukins
