Resolving homology in the face of shifting germ layer origins: Lessons from a major skull vault boundary.

The vertebrate skull varies widely in shape, accommodating diverse strategies of feeding and predation. The braincase is composed of several flat bones that meet at flexible joints called sutures. Nearly all vertebrates have a prominent 'coronal' suture that separates the front and back of the skull. This suture can develop entirely within mesoderm-derived tissue, neural crest-derived tissue, or at the boundary of the two. Recent paleontological findings and genetic insights in non-mammalian model organisms serve to revise fundamental knowledge on the development and evolution of this suture.

Current challenges in the management of patients with sickle cell disease - A report of the Italian experience.

Sickle cell disease (SCD) is an inherited red blood cell disorder caused by a structural abnormality of hemoglobin called sickle hemoglobin (HbS). Clinical manifestations of SCD are mainly characterized by chronic hemolysis and acute vaso-occlusive crisis, which are responsible for severe acute and chronic organ damage. SCD is widespread in sub-Saharan Africa, in the Middle East, Indian subcontinent, and some Mediterranean regions. With voluntary population migrations, people harboring the HbS gene have spread globally. In 2006, the World Health Organization recognized hemoglobinopathies, incl

Main anatomical features of the calcarine sulcus: a 3D magnetic resonance imaging at 3T study.

OBJECTIVE: The purpose of our study was to determine the main anatomical features of the calcarine sulcus using a 3-T MRI. METHODS: Fifty human brains have been explored using an MRI 3-T in Doctors Center in Beirut (Lebanon). RESULTS: The calcarine sulcus was identified in 100% of cases. In most cases, it had a continuous aspect with several peaks. In all our specimens, the calcarine sulcus crosses the parieto-occipital fissure. The majority of their collateral branches and their connections with other sulci were located at the level of the calcarine sulcus properly. In the majority of speci

[Improvement of therapeutic environment--effort to establish the medical center for GID in a general hospital].

Treatment for gender identity disorder (GID) should be done in cooperation with psychologist, gynecologist, urologist, plastic surgeon and extramural scholars as a team treatment. And if possible, it should be a comprehensive treatment with supports of a social worker, a coordinator and an expert nurse. The ethical committee for individual GID patient treatment has already dissolved according to the third edition of the guideline. We call the therapeutic team Okayama University Hospital Gender Center. It treats not only GID patients, but also gender associated diseases such as disorders of sex

Quantum-sized gold nanoclusters: bridging the gap between organometallics and nanocrystals.

This Concept article provides an elementary discussion of a special class of large-sized gold compounds, so-called Au nanoclusters, which lies in between traditional organogold compounds (e.g., few-atom complexes, <1 nm) and face-centered cubic (fcc) crystalline Au nanoparticles (typically >2 nm). The discussion is focused on the relationship between them, including the evolution from the Au⋅⋅⋅Au aurophilic interaction in Au(I) complexes to the direct Au-Au bond in clusters, and the structural transformation from the fcc structure in nanocrystals to non-fcc structures in nanoclusters. Thiolate

[Antenatal diagnosis of a congenital granular cell tumor].

INTRODUCTION: Neuman's tumors also called congenital epulis or congenital gingival granulomatous tumors are rare and benign oral cavity tumors. They are usually discovered at birth. CASE REPORT: A 32 year-old pregnant female patient was followed for gravid diabetes well controlled. A maxillary tumor in the fetus was revealed by the third trimester US. A fetal MRI was prescribed, after discussion with the antenatal diagnostic center, to determine the localization, size, and nature of the tumor. It revealed a 3 cm long tumor with cystic like areas not communicating with the encephalus. A caesar

Imaging of giant cell tumor of bone.

Giant cell tumor (GCT) of bone is a benign but locally aggressive and destructive lesion generally occurring in skeletally mature individuals. Typically involving the epiphysiometaphyseal region of long bones, the most common sites include the distal femur, proximal tibia and distal radius. On radiographs, GCT demonstrates a lytic lesion centered in the epiphysis but involving the metaphysis and extending at least in part to the adjacent articular cortex. Most are eccentric, but become symmetric and centrally located with growth. Most cases show circumscribed borders or so-called geographical

The role of the obstetrician-gynecologist in emerging infectious diseases: monkeypox and pregnancy.

Early in June 2003, the Centers for Disease Control and Prevention (CDC) announced yet another unique infectious disease outbreak, the first evidence of community-acquired monkeypox in the United States. By July 8, 2003, a total of 71 cases had been reported to CDC from 6 states. When emerging infectious diseases are reported in the United States, particularly when these reports receive widespread media attention, obstetrician-gynecologists may be called upon to rapidly respond to queries from their patients and to address certain infectious disease risks within their clinical practices. Narcotic drug methohexital: synthesis by enantioselective catalysis.

The narcotic drug methohexital 1 contains two asymmetric carbon atoms and, thus, consists of four isomers, two diastereomeric pairs of enantiomers. The commercial drug is the so-called alpha-racemate, one pair of diastereomers only. A method was developed to prepare differently enriched mixtures of methohexital isomers without resorting to lengthy and expensive optical resolutions. A model reaction for the synthesis of methohexital is the palladium-catalyzed allylation of 1,5-dimethyl-barbituric acid 3, which is optimized and checked by molecular modeling. Catalysts with the best ligands are u

Avascular necrosis in children with acute lymphoblastic leukemia.

This study describes the prevalence and demographics of avascular necrosis (AVN) in children with acute lymphoblastic leukemia (ALL). With improving survival of ALL patients on modern chemotherapy regimens, an increasing number of children with AVN will be presenting to orthopaedists. From 1991 to 1996, 202 patients were treated for ALL at a major tertiary pediatric cancer referral center. Eight patients (4.0%) subsequently developed AVN at an average of 30.0 months after beginning chemotherapy. A total of 27 documented joints were involved, with an average of 3.4 joints affected per patient d

The antiphospholipid syndrome.

Ten years ago we described a group of patients with a combination of clinical features associated with the presence of antiphospholipid antibodies. These features included a tendency to both arterial and venous thrombosis, livedo reticularis, recurrent abortions and occasional thrombocytopenia. This striking clinical constellation was originally named the anticardiolipin syndrome and is now more appropriately called the antiphospholipid syndrome (APS). Although our early studies were centered on systemic lupus erythematosus in which up to a third of patients demonstrated features of the syndro

Apoptosis induced in Burkitt's lymphoma cells via Gb3/CD77, a glycolipid antigen.

Gb3/CD77 is a glycolipid antigen, specifically expressed on two different B-cell populations, Burkitt's lymphoma and a subset of tonsillar B-lymphocytes located in germinal centers, which could be the normal counterpart of Burkitt cells. Both Gb3/CD77(+) populations have recently been shown to enter programmed cell death (apoptosis) readily. Here we show that verotoxin, also called Shiga-like toxin, which is known to bind to the carbohydrate moiety of Gb3/CD77, induces cell death in Gb3/CD77(+) Burkitt's lymphoma cells, not only by inhibiting protein synthesis as classically described but also

Leprosy: new insight into an ancient disease.

Patients with leprosy may be classified into two clinical and histopathologic categories. At one end of the spectrum, patients with tuberculoid leprosy have few skin lesions in which organisms can rarely be identified. At the other end of the spectrum, patients with lepromatous leprosy have numerous skin lesions containing myriad bacilli. Because immunologic resistance is associated with this spectrum, the study of leprosy provides a unique opportunity to gain insight into immunoregulatory mechanisms in man. In addition, serodiagnosis to identify early cases and prevention by vaccination are a

The tricho system: hypertrichosis, radiation, and cancer.

Five women in their 6th to 8th decades were treated for facial hypertrichosis in Erie, Pennsylvania, 30 to 40 years ago. The epilations were performed with an X-ray device called "Tricho System," which was manufactured, sold, and proclaimed safe by its physician-inventor. The beauty shop operator who administered these treatments may or may not have received "instruction" by the company in the operation of the equipment. At varying intervals, each of the 5 women developed radiodermatitis and basal cell and/or squamous cell carcinoma at the sites of epilation. All presented to the plastic surgi

The Multicultural turn in psychotherapy: A philosophical and ethical reckoning.

The multicultural and social justice counseling (MSJC) movement has become a defining paradigm in psychotherapy, shaping ethics, training, and practice. While this evolution reflects important strides toward equity and inclusion, it also introduces philosophical tensions, particularly concerning ideological conformity, epistemological overreach, and the marginalization of empirically supported treatments such as cognitive behavioral therapy (CBT). This critical position paper examines how Freirean and decolonial frameworks have transformed multiculturalism from a supplemental lens into a domin

The implantable cardioverter-defibrillators at the end of life: a double-edged sword of a life-saving technology.

Implantable cardioverter-defibrillators (ICDs) are critical for preventing sudden cardiac death, but their function poses a significant challenge in palliative care, where goals shift from life prolongation to comfort. A substantial percentage of patients receive painful, futile shocks in their final days, causing significant distress. This review synthesizes evidence on the impact of these shocks and underscores the gap between clinical practice and patient-centered care. Key barriers to timely ICD deactivation include clinician discomfort, patient misconceptions, and systemic flaws like frag

Revolutionizing Cardiac Anesthesia: A Comprehensive Review of Contemporary Approaches Outside the Operating Room.

This review article provides a comprehensive examination of the evolution of cardiac anesthesia, emphasizing contemporary approaches beyond the traditional operating room (OR) setting. Tracing the historical roots of cardiac anesthesia from its inception in the mid-20th century, the narrative explores the significant paradigm shift driven by technological advancements and changing procedural approaches. The review highlights the emergence of non-OR environments, such as hybrid operating rooms, catheterization laboratories, and electrophysiology labs, as integral spaces for cardiac intervention

The brain simulates actions and their consequences during REM sleep.

Vivid dreams mostly occur during a phase of sleep called REM 1-5 . During REM sleep, the brain's internal representation of direction keeps shifting like that of an awake animal moving through its environment 6-8 . What causes these shifts, given the immobility of the sleeping animal? Here we show that the superior colliculus of the mouse, a motor command center involved in orienting movements 9-15 , issues motor commands during REM sleep, e.g. turn left, that are similar to those issued in the awake behaving animal. Strikingly, these motor commands, despite not being executed, shift the inter

Invasion of spontaneous germinal centers by naive B cells is rapid and persistent.

In autoreactive germinal centers (GC) initiated by a single rogue B cell clone, wild-type B cells expand and give rise to clones that target other autoantigens, known as epitope spreading. The chronic, progressive nature of epitope spreading calls for early interventions to limit autoimmune pathologies, but the kinetics and molecular requirements for wild-type B cell invasion and participation in GC remain largely unknown. With parabiosis and adoptive transfer approaches in a murine model of systemic lupus erythematosus, we demonstrate that wild-type B cells join existing GCs rapidly, clonally

"This Won't Hurt a Bit": Is There a Role for Music in Bedside Procedures?

Music has played a long and storied role in clinical healing. However, the integration of music into clinical practice has been slow to gain traction, despite a recent meta-analysis demonstrating association of music interventions with clinically meaningful improvements in health-related quality of life. There is growing evidence that music has an active role in reducing patient pain and anxiety as well as affecting physiologic parameters, such as heart rate and blood pressure, in an ICU setting. Past studies have shown that incorporation of music into procedures in the operating room, radiolo
