Improvement of P-wave dispersion is associated with a lower incidence of atrial fibrillation after cardiac resynchronization therapy.

BACKGROUND: P-wave dispersion (PWD) is a useful predictor of paroxysmal atrial fibrillation (AF). The effect of cardiac resynchronization therapy (CRT) on PWD and the prognostic implications of the improvement in PWD remain undefined. The aim of the study was to explore the clinical significance of the improvement of PWD after CRT. METHODS: Electrocardiographic studies were performed before and three months after CRT in 81 patients (57 men and 24 women; age (60.5 ± 11.2) years) with standard CRT indication but no history of AF. A significant improvement of PWD (PWD responder) was defined as a…

[Successful cardiac resynchronization therapy in a patient with heart failure and ischemic mitral regurgitation: importance of septal flash].

We describe the case of a 76-year-old man with a history of ischemic heart disease and functional mitral regurgitation who over the previous six months had experienced worsening of functional class (NYHA III/IV) under optimal medical therapy, without ischemic symptoms and with negative ischemic tests. Mitral valve annuloplasty was considered. As the patient presented left bundle branch block on the surface ECG, cardiac resynchronization therapy (CRT) was also considered. There was, however, severe biventricular dysfunction and moderate to severe pulmonary hypertension, which are considered pre…

Incidence of paroxysmal atrial tachycardias in patients treated with cardiac resynchronization therapy and continuously monitored by device diagnostics.

AIMS: Little is known about the incidence of paroxysmal atrial tachycardias (PAT) in patients with heart failure (HF). The availability of cardiac resynchronization therapy (CRT) devices with extended diagnostics for AT enables continuous monitoring of PAT episodes. The aim of the study was to assess the incidence over time of PAT in HF patients treated with CRT. METHODS AND RESULTS: Consecutive patients in NYHA functional class III or IV despite optimal drug therapy, QRS duration > or = 130 ms, left ventricular ejection fraction < or = 35%, and left ventricular end-diastolic dimension > or =…

Upgrade and de novo cardiac resynchronization therapy: impact of paced or intrinsic QRS morphology on outcomes and survival.

BACKGROUND: Cardiac resynchronization therapy (CRT) improves outcomes in patients with left bundle branch block (LBBB), but the benefits of CRT in patients with other QRS morphologies or previous pacing are uncertain. OBJECTIVE: The purpose of this study was to describe outcomes in patients with prior right ventricular pacing and non-LBBB morphologies. METHODS: We studied 505 patients who underwent de novo CRT (n = 338) or CRT upgrade (n = 167). De novo patients were categorized by underlying QRS morphology: LBBB (67%), right bundle branch block (RBBB; 11%), intraventricular conduction delay…

Large response to cardiac resynchronization therapy in a patient with segmental paradoxical systolic expansion identified by strain imaging.

An 84-year-old man with non-ischaemic cardiomyopathy underwent cardiac resynchronization therapy (CRT) based on the presence of drug-refractory heart failure, depressed left ventricular ejection fraction (25%), and wide QRS complex (160 ms). Longitudinal tissue velocity revealed no significant dyssynchrony (23 ms in Yu index and 35 ms in opposing wall delay). However, longitudinal tissue Doppler strain revealed unique appearances in apical four-chamber and long-axis views. The anterior and inferior septum at basal and mid-levels had reversed strain (becoming positive), indicating paradoxical s…

Combined identification of septal flash and absence of myocardial scar by cardiac magnetic resonance imaging improves prediction of response to cardiac resynchronization therapy.

BACKGROUND AND AIMS OF STUDY: Septal flash (SF) describes early inward motion of the ventricular septum in patients with left bundle branch block (LBBB), and correction corresponds to increased response to cardiac resynchronization therapy (CRT). SF has traditionally been assessed by echocardiography. We sought to determine if cardiac magnetic resonance (CMR) imaging could identify SF and if the additional assessment of scar would improve the ability of CMR to predict CRT response. METHODS: Fifty-two patients with LBBB and heart failure underwent prospective CMR scanning prior to CRT implanta…

Lead replacement due to non-infectious reasons in patients with cardiac resynchronization therapy in long-term follow-up.

BACKGROUND: Traditional cardiac resynchronization therapy (CRT) is an effective treatment for patients with heart failure. OBJECTIVE: The study aimed to determine the long-term durability of CRT leads, factors predisposing to lead replacement, and the outcomes of lead replacement in CRT patients. METHODS: The study population comprised 1059 consecutive patients who received conventional CRT implants at a university hospital between 2002 and 2019. RESULTS: During a median follow-up of 1661 days (interquartile range: 815-2792), a total of 324 leads were replaced in 251 patients (23.7%) for no…

Sex differences in left bundle branch pacing for cardiac resynchronization therapy: novel procedural insights into pacing lead location.

BACKGROUND: Females have been reported to benefit more than males from biventricular pacing for cardiac resynchronization therapy (CRT). However, whether such sex differences also exist in left bundle branch pacing (LBBP) for CRT and the underlying mechanisms remain unclear. OBJECTIVE: This study aimed to investigate sex differences in LBBP-CRT, with a particular focus on procedural characteristics. METHODS: Patients with heart failure and left bundle branch block who had CRT indications and achieved successful LBBP were retrospectively analyzed. Sex differences in procedural performance and…

Associations of adipose tissue depots with cardiac resynchronization therapy response and clinical outcomes: A CRT-HF Clinic substudy.

BACKGROUND: The region of adipose deposition is an important determinant in the outcomes of patients with heart failure (HF). However, the impact of regional adiposity on HF patients undergoing cardiac resynchronization therapy (CRT) remains unclear. METHODS: A retrospective cohort analysis was conducted on 95 patients from a single-center study, assessing post-CRT outcomes. Multi-slice body composition measurements of chest computed tomography before CRT placement were used for adipose quantification. Patients were stratified based on subcutaneous adiposity, intramuscular adiposity, and hepa…

Personalized cardiac resynchronization therapy guided by real-time electrocardiographic imaging for patients with non-left bundle branch block.

BACKGROUND: Patients with heart failure and a non-left bundle branch block (non-LBBB) QRS pattern have a limited response to biventricular pacing (BVP). OBJECTIVE: A personalized cardiac resynchronization therapy (CRT) implantation approach guided by real-time electrocardiographic imaging (ECGi) was studied. METHODS: Twenty patients with left ventricular ejection fraction (LVEF) ≤ 35%, QRS duration ≥ 120 ms, and non-LBBB [13 (65%) with right bundle branch block and 7 (35%) with intraventricular conduction delay] were recruited. During CRT implantation, right atrial, right ventricular, corona…

Cardiac resynchronization therapy-defibrillator implantation with shock lead placement in the left bundle branch area: a case report.

BACKGROUND: Cardiac resynchronization therapy (CRT) with biventricular pacing is a well-established therapy. Left bundle branch area pacing (LBBAP) is a safe technique providing physiological pacing, and LBBAP-optimized CRT (LOT-CRT) has been shown to provide better electrical resynchronization than traditional CRT. However, there are few reports on shock lead placement in the left bundle branch area (LBBA) during CRT-defibrillator (CRT-D) implantation. CASE SUMMARY: A 76-year-old woman with heart failure from dilated cardiomyopathy presented with left bundle branch block pattern (QRS duratio…

Electrocardiographic predictors of clinical outcomes in nonischemic cardiomyopathy patients with left bundle branch area pacing cardiac resynchronization therapy.

BACKGROUND: Paced QRS morphology may vary during left bundle branch area pacing (LBBAP) per the pacing location. It remains unclear whether electrocardiographic changes observed during LBBAP can predict clinical outcomes. OBJECTIVE: We aimed to assess correlation between characteristics of paced QRS on the electrocardiogram and clinical outcomes in heart failure patients with nonischemic cardiomyopathy. METHODS: Of 79 consecutive heart failure patients receiving LBBAP, 59 patients were included in this prospective study after exclusions. LBBAP was performed using Medtronic 3830 lead. Patient

Cardiac resynchronization therapy guided by interventricular conduction delay: How to choose between biventricular pacing or conduction system pacing.

BACKGROUND: Biventricular pacing (BIV) is the gold standard for cardiac resynchronization therapy (CRT). Thirty percent of patients do not respond to CRT. Conduction system pacing (CSP) represents a viable alternative. Interventricular conduction delay (IVCD), as electrical desynchrony marker, is a CRT response predictor. The aim of this study was to determine the incidence of CRT responders by selecting the best approach between BIV and CPS based on intraoperative IVCD measurement in patients with HFrEF and LBBB. METHODS: Ninety-six patients were randomly assigned in a 1:1 ratio to either a …

Horizontal QRS axis predicts response to cardiac resynchronization therapy in heart failure patients with left bundle branch block.

BACKGROUND: Electrocardiogram criteria for left bundle branch block (LBBB) inadequately predict left ventricular electrical dyssynchrony, complicating cardiac resynchronization therapy (CRT) candidate selection. OBJECTIVE: This study aimed to investigate the predictive value of the horizontal QRS axis for CRT response in heart failure (HF) patients with LBBB patterns. METHODS: The direction and magnitude of the horizontal QRS axis were calculated using the net amplitudes in leads V2 and V6. CRT response was defined as a ≥10% increase in left ventricular ejection fraction and at least 1 New Y…
