Clinical analysis of combined revascularization in treating ischemic Moyamoya disease in adults.

OBJECTIVE: The aim of this study was to investigate the clinical efficacy of superficial temporal artery-middle cerebral artery anastomosis (STA-MCA)+encephalo-duro-myo-synangiosis (EDMS) in treating ischemic Moyamoya disease (IMD) in adults. METHODS: A total of 30 adult patients with IMD were selected to be included in the study; they underwent STA-MCA+EDMS and were followed up for 3 months to 2 years. The digital subtraction angiography findings, modified Rankin scale (mRs) score, and complications of all the patients were compared. RESULTS: Thirty patients successfully completed the surge

"Y-stent retriever": a new rescue technique for refractory large-vessel occlusions?

Long-awaited positive trial data have shown the efficacy of endovascular treatment in patients with ischemic stroke who arrive at the hospital within the first 6 hours with large-vessel occlusion of the anterior circulation. With the introduction of stent retrievers (SRs) for mechanical thrombectomy, efficient and safe large-artery recanalization treatment can be achieved. However, sometimes there are patients who do not attain complete flow restoration following attempts with traditional maneuvers. The authors present the case of a 57-year-old man with acute ischemic stroke due to an M1 embol

How to Clamp and Bypass if There Is Single Artery Supply to the Head and That Contains Severe Stenosis?

Takayasu arteritis manifests with arterial occlusions and aneurysms. Revascularization is sometimes challenging, especially when carotid arteries are affected. In this report, we present a carotid artery revascularization technique in a patient who was admitted with orthostatic and postprandial transient ischemic attacks, resulting in a diagnosis of bilaterally occluded subclavian and vertebral arteries, occluded left common carotid artery, and severely stenosed right common carotid artery. Clamping of the right carotid artery was a challenge; however, our technique provides a neurologically s

Perioperative Complications and Long-Term Outcomes After Bypasses in Adults with Moyamoya Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Surgical revascularization for adults with moyamoya disease (MD) includes direct, indirect, or combination bypasses. It is unclear which provides the best outcomes. We sought to determine the best surgical management for adults with MD by comparing perioperative complications and long-term outcomes among 3 bypass types. METHODS: Literature databases were searched for articles reporting revascularization bypass outcomes for adults with MD. A pooled analysis of all qualified studies and meta-analysis using only studies reporting direct comparisons of 2 bypass types were performed. O

[Four cases of fusiform posterior inferior cerebellar artery aneurysms successfully treated by trapping and occipital artery-PICA bypass].

Aneurysms located on the proximal portion of the posterior inferior cerebellar artery (PICA) are rare, and even rarer are fusiform aneurysms in this location. Therefore the principles of surgical management are poorly understood and still subject to debate. The management plan for these lesions is based on the proper understanding of the PICA anatomy, and particularly the origin of important perforating arteries. As many anatomic variations of PICA can be observed and the perforator's origin is sometimes in complex anatomical relations with the aneurysm, the management has to be individualized

Meningeal management for optimal revascularization from middle meningeal artery.

Numerous surgical techniques have been developed and refined for the treatment of moyamoya disease. Among the indirect techniques of revascularization, encephaloduroarteriosynangiosis has been recognized as effective in promoting revascularization and reversing symptomatology. Neovascularization occurs between the donor artery, either the superficial temporal artery or the occipital artery, and the underlying ischemic cortex. Additionally, the middle meningeal artery and its dural branches have also been shown to contribute to collateral blood supply. In this report the authors describe an int

Encephalo-myo synangiosis (EMS) in the vertebrobasilar occlusive disease. Case reports.

Two cases of vertebro-basilar insufficiency treated with infratentorial (IT) Encephalo-Myo-Synangiosis (EMS) using the pedicled occipitalis muscle flap, are presented. IT EMS was carried out for revascularization of the vertebro-basilar system. None of these cases has suffered from either carotid or vertebro-basilar insufficiency during seven years follow-up period. As far as we know this is the first report of successful IT EMS for vertebro-basilar insufficiency.

Critical Care Nursing in Moyamoya Disease: Complexities and Considerations.

Moyamoya disease is a rare progressive cerebrovascular disorder involving stenosis of major intracranial arteries and the formation of fragile collateral vessels. Patients face lifelong risks of ischemic and hemorrhagic stroke. Surgical revascularization is the definitive treatment, aimed at restoring cerebral perfusion and reducing neurologic injury. Postoperative care is among the most complex in neuroscience nursing. Critical care nurses play a vital role in managing cerebral perfusion, conducting frequent neuro assessments, regulating blood pressure, and preventing seizures. Success requir

Association between angiographic and clinical outcomes after STA-MCA bypass in adult moyamoya disease.

BACKGROUND AND PURPOSE: As an angiographic outcome, postoperative collateral formation (PCF) is commonly used to evaluate the effect of STA-MCA bypass in moyamoya disease (MMD), but whether it can reliably reflect clinical outcomes is still unclear. We investigated the association between PCF and clinical outcomes in adult MMD. METHODS: All STA-MCA bypass procedures performed from January 2013 to December 2019 were screened in this prospective cohort study. Patients who acquired presurgical and follow-up catheter angiography were included. The clinical outcomes consisted of symptom improvemen

Clinical importance of the superficial temporal artery in neurovascular diseases: A PRISMA-compliant systematic review.

The superficial temporal artery (STA) plays a very important role in neurovascular diseases and procedures. However, until now, no comprehensive review of the role of STA in neurovascular diseases from a neurosurgical perspective has ever been published. To review research on the clinical importance of STA in neurovascular diseases, a literature search was performed using the PubMed database. Articles were screened for suitability and data relevance. This paper was organized following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. According to the l

Subarachnoid Hemorrhage Attributable to Bilateral Aplastic or Twiglike Middle Cerebral Artery.

BACKGROUND: Aplastic or twiglike middle cerebral artery (MCA), a congenital anomaly related to the abnormal development of the MCA, is rare. It is usually unilateral; bilateral lesions are extremely rare. An aplastic or twiglike MCA may lead to cerebral ischemic stroke, intracranial hemorrhage, and subarachnoid hemorrhage (SAH). In patients with ischemic stroke due to hemodynamic stress, extracranial-intracranial (EC-IC) bypass surgery is indicated. As hemodynamic stress may also elicit SAH, it is important to select the appropriate treatment. EC-IC bypass surgery was successful in a patient w

Current management of vertebral basilar occlusive disease.

The current medical and surgical management of vertebral basilar occlusive disease has been reviewed, with the emphasis on the lack of definitive data proving the absolute efficacy of one form of therapy. In addition, the renewed interest in medical and surgical therapy of the vertebral system has been displayed. More experience and detailed study should be directed towards the final solution of the best treatment for vertebral basilar insufficiency. The extracranial vascular procedure of choice for correcting a vertebral stenosis appears to be a vertebral to common carotid artery anastomosis.
