AAEM case report #28: monomelic amyotrophy.

Monomelic amyotrophy is a rare form of motor neuron disease usually presenting as painless asymmetric weakness and atrophy in the distal upper extremities of young adults. Only rarely are the legs involved and pyramidal findings are uncommon. Monomelic amyotrophy is most often observed in people of Japanese and Indian heritage and affects men almost exclusively. Most cases are sporadic. Laboratory testing is frequently normal or nonspecific except for electrophysiologic studies which typically demonstrate reduced compound muscle action potential amplitudes, fasciculations, and features consist

Effects of denervation and muscle ablation procedures on the canine pharyngo-oesophageal junction.

The resting and deglutitive pressures in the pharynx, pharyngo-oesophageal sphincter, and upper oesophagus were obtained from 32 healthy intact dogs. The dogs were divided into groups and submitted to either bilateral denervation or muscle ablation. Manometric studies were repeated two weeks and four weeks after operation. Bilateral division of the pharyngo-oesophageal nerve reduced the resting sphincteric pressure and the amplitude of the pressures of sphincteric relaxation and contraction after deglutition. Bilateral division of the recurrent laryngeal nerves or bilateral division of the cer

A fluoroscopic assessment of brachial plexus block by the supraclavicular approach: Have we been overmedicating?

BACKGROUND: Ultrasonography-guided supraclavicular brachial plexus block has demonstrated safety as compared with landmark or nerve stimulat … ps (p = 0.103). Horner syndrome (HS) was seen in 51.18% of patients. First, ipsilateral superficial cervical plexus block was also observed in 40.22% of patients. Among patients who developed both, ∼60% of patients (99/167) belonged to the 40-mL group. CONCLUSIONS: Optimal MEAV appears between 20 and 30 mL. Higher drug volumes are associated with more cephalad s …

Quality of life in patients treated by adjuvant radiotherapy for endometrial and cervical cancers: correlation with dose-volume parameters.

INTRODUCTION: Modern multidisciplinary cancer treatments aim at obtaining minimal influence on patients' quality of life (QoL). The purpose of this study was to assess QoL and correlate it with dose-volume parameters of organ at risks (OARs) in patients who received adjuvant radiotherapy for endometrial and cervical cancers. MATERIALS AND METHODS: We administered the EORTC QLQ-C30 and EN24 or CX24 questionnaires to 124 patients, 100 with endometrial cancer and 24 with cervical cancer treated with postoperative radiotherapy ± chemotherapy in regular follow-up. Bladder function, fecal incontine

[A case of neuralgic amyotrophy with antiganglioside antibody].

A 54-year-old-man experienced pain from his left shoulder to his left arm and had difficulty in lifting his arm after a febrile episode. Three weeks after the onset, he was admitted to our hospital. Neurological examination demonstrated weakness and atrophy of the left deltoid muscle. Deep tendon reflexes were normal and no pathological reflexes were elicited. CSF total protein was slightly increased. The occurrence rate of F-waves was decreased in the left upper limb. Magnetic resonance imaging (MRI) study of the cervical cord and brachial plexus with and without Gadolinium infusion showed no

Intraoperative Monitoring of the Diaphragm Function During Cervical Dorsal Root Entry Zone-Lesioning Illustration With a Case Report.

BACKGROUND AND IMPORTANCE: Microsurgical DREZotomy (MDT) involves lesioning the dorsal root entry zone at the cervical spinal level, carrying a potential risk of iatrogenic spinal cord injury. Misidentifying the C4 root can lead to impaired diaphragmatic contractions and respiratory compromise. This report is the first MDT procedure in our Central Asian region and introduces the simultaneous analysis of 3 ventilator waveforms-capnography, pressure time, and flow time-as a novel, rapid-response intraoperative neurophysiological monitoring for definitive C4 root identification. CLINICAL PRESENT

[Gunshot injuries of peripheral nervous system: the questions of classification and diagnostics].

Two hundreds and sixty-seven patients with gunshot injuries of 394 nerves and plexus underwent clinical and electromyographic investigation and H-reflex evaluation as well. Based on the results of the data obtained, 3 types of nerves trunk injuries: neuroapraxia, axonothmesis and neurothmesis, were identified. The reduction of biopotential amplitude and synergic muscular activity, decrease of impulse conduction velocities (ICVeff--up to 30%), M-response amplitude (up to 50% and more) and motor units functioning were characteristic of neuroapraxia of nerve trunks. Axonothmesis of nerve trunks f

Bypass coaptation for cervical root avulsion: indications for optimal outcome.

OBJECTIVE: Previously, we reported bypass coaptation of the C3 and C4 anterior rami to the upper trunk of the brachial plexus for restoration of the muscles denervated as a result of C5 and C6 nerve root avulsion. This procedure is thought to be superior to the transfer of individual peripheral nerve fibers to the brachial plexus branches. Therefore, the benefits of the bypass coaptation procedures in the treatment of various root avulsions are presented. METHODS: Twenty-six patients were selected as suitable candidates for bypass coaptation procedures. They were divided into 3 groups: 1) Erb

Histopathologic study of the neuroma-in-continuity in obstetric brachial plexus palsy.

BACKGROUND: Operative treatment of traction lesions in obstetric brachial plexus palsy is still controversial. The authors analyzed the histopathology of neuroma-in-continuity of the upper trunk by study of the resected neuroma. METHODS: The neuroma-in-continuity of the upper trunk was studied histopathologically in 28 children with Erb palsy who had undergone resection of the neuroma and nerve reconstruction of the plexus at the age of 3 to 11 months. The authors recorded the distribution of myelinated motor nerve fibers and the proportions of collagen and regenerating nerve fibers traveling

Onset of complications following cervical manipulation due to malpractice in osteopathic treatment: a case report.

The aim of this study is to correlate cervical disc herniation with manipulation performed by a non-physician osteopath on a patient complaining of neck pain. The authors report a case in which a woman - treated with osteopathic spinal manipulation - developed cervical-brachial neuralgia following the cervical disc herniation. The patient then underwent surgery and was followed by physiotherapists. A clinical condition characterized by limitation of neck mobility, with pain and sensory deficit in the right arm and II-III fingers, still persists. The patient consulted the authors to establish w

[Pseudocyst of the neck after traumatic avulsion of spinal C6-T2 nerve roots--case report].

A case is presented of an avulsion injury of the left brachial plexus in a woman who--after surgical reconstruction of the plexus--developed a liquid fistula from a giant pseudocyst localized in this area. A prolonged persistence of the pseudocyst resulted in a severe pain syndrome and symptoms of vascular disorders in the upper limb. Since results of local treatment (punctures, pressure dressings) were unsatisfactory, the authors decided to close the liquid fistula by suturing the damaged dural sac by means of laminectomy. The necessity of particular caution when operating in the area of inte

[Carotid endarterectomy under locoregional anesthesia].

The purpose of carotid surgery is to prevent the incidence of stroke in patients with cerebrovascular disease. It's important, therefore, to find the most useful methods of cerebral protection and flow monitorization during carotid endarterectomy. It is well known that patient's clamping tolerance changes according to his anatomical and physiopathological conditions (contralateral carotid patency, stenosis, occlusion), and compensative circles efficiency (Willis). General anaesthesia reduces cerebral metabolic requests and allows a better pressure control, but requires difficult, expensive and

[Experimental study on delayed reinnervation of vocal cord adductors].

The adductor branch was anastomosed with the main branch of ansa cervicalis to reinnervate the vocal cord adductors in six dogs at the time of denervation and after varying intervals of chronic denervation. Some recovery of adduction was noted within ten-month interval. Laryngeal electromyography, tension tests and histologic studies of the adductor muscles were correlated with the recovery of adduction. Although adduction didn't recover after twelve-month interval, striations of the adductor muscles were normal, only slight fibrosis was observed.

Clinical diagnosis, testing, and electromyographic study in brachial plexus traction injuries.

A system for evaluation of patients with brachial plexus injury is based on careful observation, including the patient's history and a physical examination, a radiographic assessment, and electrodiagnosis. The clinical history and physical examination remain the cornerstone of this evaluation and their documentation is vital. Traction injuries, wherein there is a possibility of root avulsion, were examined myelographically and electromyographically at one month. Although they must be considered, the limitations of these methods of evaluation do not vitiate their usefulness. Modification of the
