Maternal indicators to monitor hospitals in the Rede Cegonha: a proposal.

The Rede Cegonha strategy was launched in 2011 by the federal government to improve pregnancy, delivery, postpartum care, and child development in the first two years of life, reduce maternal and child mortality, and expand women's sexual and reproductive rights. We propose seven new maternal indicators to improve obstetric care evaluation in the next assessment cycle. The new indicators are the use of Robson's classification to monitor cesarean rates, use of magnesium sulfate in cases of preeclampsia/eclampsia, pregnant women's use of calcium supplements, blood transfusions and hysterectomy i…

Death after adenotonsillectomy secondary to massive pulmonary embolism.

Tonsillectomy is one of the most common surgical procedures performed in the United States. Although relatively safe, there is a small risk of post-operative mortality. The majority of deaths come from airway compromise or hemorrhage. The authors present a case of a 32-month-old child who underwent routine adenotonsillectomy for sleep disordered breathing and chronic pharyngitis who was found unresponsive and pulseless in his bed on the morning of postoperative day 2. The cause of death determined by post mortem autopsy was massive pulmonary embolism (PE). PE is a rare event in children and ha

Gangrenous small bowel obstruction secondary to congenital internal herniation: a case report.

Congenital internal herniation is a rare condition presenting as recurrent abdominal pain or acute intestinal obstruction. In cases in which bowel incarceration or strangulation develop, rapid progression to bowel ischemia, necrosis or perforation is inevitable. Mortality in such cases has been reported to be as high as 50%. Despite advances in imaging modalities, arriving at a pre-operative diagnosis of a congenital internal herniation remains a challenge. We report such a case where imaging was unsuccessful in determining the cause of intestinal obstruction in a 3 year old child. Congenital

Are child passengers bringing up the rear? Evidence for differential improvements in injury risk between drivers and their child passengers.

Since nearly half of children fatally injured in automobile crashes were restrained, optimizing occupant protection systems for children is essential to reducing morbidity and mortality. Data from the Partners for Child Passenger Safety study were used to compare the differential injury risk between drivers and their child passengers in the same crash, with a focus on vehicle model year. A matched cohort design and conditional logistic regression model were used in the analyses. Overall, injury risk for drivers was higher than for children, but the risk difference was largest for the oldest mo

Toxic shock syndrome in burns: diagnosis and management.

Toxic shock syndrome (TSS), a toxin-mediated disease, is the most common cause of unexpected mortality in children with small burns. It is a diagnosis that is often missed because of non-specific signs and an ability to mimic other childhood illnesses. Any child with a pyrexia greater than 38.9 degrees C, a rash, or a sudden change in clinical condition within a few days of a burn injury should be monitored closely for TSS. If there is co-incident hyponatraemia or lymphopaenia, or if there is any deterioration in clinical condition, the child should be managed with anti-staphylococcal and stre

Outlook for the child with a cephalocele.

Specific information on the outcome for a child with a cephalocele can be difficult to find and interpret. To update outcome information for the child with a cephalocele, the investigators compared outcome of 34 infants from their institution with that of those in previously published series. For the infants from the investigators' institution, overall mortality was 29% and was confined to infants with posterior defects, which was consistent with other published series. Additional major congenital abnormalities were present in nearly half the infants, and these were an important factor in cont

Destructive pulmonary disease due to mixed anaerobic infection.

Pulmonary infection is one of the commonest infections in childhood and is responsible for considerable mortality and morbidity, especially in Nigeria. Most infections are caused by Staphylococcus pyogenes, Streptococcus pneumoniae, and Haemophilus influenzae, though occasionally other organisms such as Klebsiella pneumoniae, coliform, and salmonella are responsible. Rarely does one encounter mixed infection with Clostridium welchii, coliform, and anaerobic streptococci organisms which are the causative organisms in the case reported here. The child presented with features of a bronchopneumoni

[Shaken Baby Syndrome: fact or fiction in Portugal?].

Shaken Baby Syndrome (SBS) is an unquestionable and current worldwide problem, as it is considered one of the most severe forms of child abuse, with significant morbidity and mortality among children under one year of age. In Portugal the actual incidence of SBS is still unknown and probably undervalued and underdiagnosed, but international literature reports incidences between 14 and 40.5 per 100.000 children a year in several different countries. It is classically considered SBS to consist of shaking a child repeatedly and violently, inducing a cervical whiplash like movement. Due to the chi

The Utility of Psoas Muscle Index for the Detection of Sarcopenia and its Association with Severity of Alcoholic Liver Cirrhosis: Western India Perspective.

INTRODUCTION: Alcoholic liver cirrhosis (LC) is a severe condition that can lead to malnutrition and negatively impact a person's quality of life. Malnutrition can also increase the risk of functional impairment, mortality, and poor response to stressors like infections and surgeries. Addressing malnutrition in these patients is essential for improving their health outcomes and overall well-being. Current prognostic scoring systems do not consider malnutrition, and sarcopenia is a refractory complication associated with alcoholic LC. MATERIALS AND METHODS: The study aimed to quantify sarcopen

Transarterial chemoembolization in unresectable hepatocellular carcinoma--assessing the factors affecting the survival: An audit from a tertiary care center in northern India.

INTRODUCTION: Hepatocellular carcinoma (HCC) is the most common primary malignant tumor of the liver and third most common cause of cancer-related mortality. According to Barcelona Clinic Liver Cancer (BCLC) staging system, transarterial chemoembolization (TACE) is considered a recommended option for intermediate HCC. OBJECTIVES: The purposes of our study were to evaluate the various factors affecting the outcome of TACE, to study the efficacy of TACE by evaluating the imaging response by European Association for the Study of the Liver (EASL) necrosis criteria, and to study the survival of pa

Cerebral salt wasting syndrome in a child with central diabetes insipidus following surgery for recurrent craniopharyngioma: A case report.

Surgical treatment of craniopharyngiomas in children can produce disorders related to water and sodium such as central diabetes insipidus (CDI) and cerebral salt wasting syndrome (CSWS). The combination of both in children is unusually reported in the literature and is associated with high mortality. The management of CSWS is based on fluid therapy. Fludrocortisone is useful in children with CSWS who do not respond to fluid management. The objective of the paper is to describe the case of 6 years and 10 months old male child with hypopituitarism secondary to a craniopharyngioma surgery perform

An infant with acral peeling of skin: A curious case of congenital syphilis.

Congenital syphilis (CS) is a vertically transmitted infection caused by the spirochete Treponema pallidum. It is seen rarely due to proper antenatal screening. Signs and symptoms appear within the first 2 years of life in early CS and after 2 years in late CS. Failure to diagnose and treat CS in its early stages can result in higher morbidity and mortality. Skin manifestations can guide toward the diagnosis of CS at an early stage. Here, we report a 2-day-old neonate who presented with acral peeling of skin along with respiratory distress and hepatosplenomegaly. Clinical suspicion of CS was m

Unilateral pulmonary venous atresia: A rare cause of recurrent hemoptysis.

Hemoptysis is a rare but distressing complaint in children. Pulmonary venous atresia (PVA) is a rare cause of recurrent hemoptysis in children. We report a 3-year-old boy with recurrent hemoptysis for 2 years. The child had tachycardia, tachypnea, tender hepatomegaly, and left-sided decreased air entry with crepitations. Coagulation and Koch's workup was negative. X-ray of the chest showed a small left hemithorax. Computed tomography with angiography showed left unilateral PVA with pulmonary artery hypoplasia and dysplastic left lung. The child has been posted for left pneumonectomy. Although

[Respiratory failure in cystic fibrosis: management in pediatric intensive care unit, lung transplantation recommendation].

Admission to the ICU for respiratory failure of a child with cystic fibrosis is a telltale sign of the severity of the disease. Bronchopulmonary exacerbation, pneumothorax and hemoptysis are the primary causes, for which respiratory assistance is indispensable in these life-threatening situations. Non-invasive ventilation (NIV) has enabled significant progress in improving patient survival. The modalities of NIV must be tailored to both the patient and the cause of respiratory failure. Invasive ventilation, on the other hand, should be a treatment of last resort, because often associated with

Laparoscopic mesh repair of a Bochdalek diaphragmatic hernia with acute gastric volvulus in a pregnant patient.

The advantages of minimally invasive therapy can be utilised in the surgical disorders of pregnant patients. To our knowledge, there has not been a previous report describing laparoscopic management of diaphragmatic hernia (with mesh) in pregnancy. A 23-year-old pregnant (second trimester) woman was admitted with vomiting, epigastric pain, oliguria and dyspnoea of one month duration. Investigations revealed posterolateral diaphragmatic hernia of Bochdalek with gastric volvulus. Successful laparoscopic mesh-plasty of the diaphragmatic hernia was performed without mortality or morbidity to both

[Pulse electrophoresis of serogroup A streptocci isolated in Moscow].

Pulse electrophoresis of group A streptococci (GAS) isolated in 1998 - 2004 in Moscow from patients during outbreaks of tonsillitis in child institutions, military unit and also from sporadic cases in children and adults was performed. During analysis of 48 strains 16 pulse electrotypes were recognized. These data allow to consider that in different child institutions (and other groups) accumulation and spreading of most adapted to such environment variants of GAS are take place and population structure of GAS in such institutions is unique on molecular and other characteristics. [Infantile myofibromatosis of the tongue - a case-report].

PATIENT: We report a case of infantile myofibromatosis of the tongue in a 5 month old female child. The parents reported discovering the tumor in the musculature of the tongue and the tumor had largely grown in size in the space of 4 weeks. The child's health was not impaired at any time. After a specimen was removed for a histological examination, an infantile myofibromatosis was diagnosed. DISCUSSION: The occurrence of these tumors in neonates and young children in the oral cavity and in particular in the tongue is rarely reported. Development of the benign tumor disease depends on the form

Acute diarrhoeal disease in India and Indonesia.

Acute diarrhoea is still a leading cause of child mortality and morbidity, second only to pneumonia as a killer of children, in India and Indonesia. Untreated diarrhoea precipitates malnutrition and is often the underlying cause of marasmus and kwashiorkor. Shigella and salmonella dysenteries are responsible for about 60% of all cases in Indonesia and India. These bacillary agents respond well to trimethoprim. Amoebiasis responds well to metronidazole. Most cases can be managed in the home, even if the exact cause is unknown, by giving liquids or a simple rehydration drink. Diarrhoea can be pr…

Social Determinants of Childhood Mortality: Systematic Review and Meta-Analysis With Complementary Narrative Synthesis of Cultural Factors.

OBJECTIVE: To identify social determinants associated with childhood mortality and describe cultural factors. METHODS: We conducted a systematic review and meta-analysis of observational studies following MOOSE guidelines. MEDLINE, Embase, ProQuest, Wiley, OpenGrey, Google Scholar, and LILACS were searched through October 2024. Childhood mortality was defined as death before age five. Comparable social determinants were pooled quantitatively, while cultural factors were synthesized narratively. RESULTS: Thirty-two studies were included in the qualitative synthesis and 18 in the meta-analysis

The impact of measles immunization campaigns in India using a nationally representative sample of 27,000 child deaths.

India comprises much of the persisting global childhood measles mortality. India implemented a mass second-dose measles immunization campaign in 2010. We used interrupted time series and multilevel regression to quantify the campaign's impact on measles mortality using the nationally representative Million Death Study (including 27,000 child deaths in 1.3 million households surveyed from 2005 to 2013). 1-59 month measles mortality rates fell more in the campaign states following launch (27%) versus non-campaign states (11%). Declines were steeper in girls than boys and were specific to measles
