Cost-Effectiveness Analysis of Finerenone for Treatment of Chronic Kidney Disease in Patients with Type 2 Diabetes from Japanese Payer Perspective.

INTRODUCTION: Type 2 diabetes (T2D) is a major cause of chronic kidney disease (CKD) in Japan, and there is an increasing treatment need for first- and second-line care in these patients. The addition of finerenone to current treatment modalities lowers the risk of CKD progression and cardiovascular events in patients with CKD and T2D from the Japanese payer perspective. This study investigated the cost-effectiveness analysis of adding finerenone to standard of care (SoC) versus SoC alone for the treatment of CKD in patients with T2D. METHODS: The FINE-CKD model validated to estimate the cost

Accounting for group differences in study retention in a randomized trial of specialized treatment for first episode psychosis.

BACKGROUND: Schizophrenia is a chronic disabling disorder for which current treatments are only partially effective. While the evaluation of novel interventions is a high priority, loss to follow-up is a major threat to validity. METHODS: Pattern mixture modeling is a statistical technique that incorporates information on patterns of retention that may bias comparisons between randomized treatment groups. This study used pattern mixture mixed model (PMMM) in the analysis of outcomes of a two-year cluster-randomized trial, the Recovery after an Initial Schizophrenia Episode-Early Treatment Pro

The determinants of individual health care expenditures in prison: evidence from Switzerland.

BACKGROUND: Prison health systems are subject to increasing pressures given the specific health needs of a growing and aging prison population. Identifying the drivers of medical spending among incarcerated individuals is therefore key for health care governance in prisons. This study assesses the determinants of individual health care expenditures within the prisons of the canton of Vaud, a large region of Switzerland. METHODS: We use a unique dataset linking demographic and prison stay characteristics as well as objective measures of morbidity to detailed medical invoice data. We adopt a mu

Development of Low-Cost Locally Sourced Two-Component Compression Bandages in Western Kenya.

INTRODUCTION: Compression therapy is well-established standard of care for chronic leg ulcers from venous disease and lymphedema. Chronic leg ulcers and lymphedema have a significant impact on quality of life, driven by pain, foul odor, and restricted mobility. Provision of layered compression therapy in resource-limited settings, as in Western Kenya and other regions of sub-Saharan Africa, is a major challenge due to several barriers: availability, affordability, and access to healthcare facilities. When wound care providers from an Academic Model Providing Access to Healthcare (AMPATH) healt

Penalized G-estimation for effect modifier selection in a structural nested mean model for repeated outcomes.

Effect modification occurs when the impact of the treatment on an outcome varies based on the levels of other covariates known as effect modifiers. Modeling these effect differences is important for etiological goals and for purposes of optimizing treatment. Structural nested mean models (SNMMs) are useful causal models for estimating the potentially heterogeneous effect of a time-varying exposure on the mean of an outcome in the presence of time-varying confounding. A data-adaptive selection approach is necessary if the effect modifiers are unknown a priori and need to be identified. Although

Assessing uncertainty in the burden of hepatitis C virus: Comparison of estimated disease burden and treatment costs in the UK.

Hepatitis C virus (HCV) is a major and growing public health concern. We need to know the expected health burden and treatment cost, and understand uncertainty in those estimates, to inform policymaking and future research. Two models that have been important in informing treatment guidelines and assessments of HCV burden were compared by simulating cohorts of individuals with chronic HCV infection initially aged 20, 35 and 50 years. One model predicts that health losses (measured in quality-adjusted life-years [QALYs]) and treatment costs decrease with increasing initial age of the patients,

Cost effectiveness of first-line oral antiviral therapies for chronic hepatitis B : a systematic review.

BACKGROUND: Chronic hepatitis B is a common, progressive disease, particularly when viral replication is detected. Oral antivirals can suppress viral replication and prevent or delay the development of cirrhosis and liver-related complications. OBJECTIVE: The aim of this study was to systematically review the quality of cost-effectiveness evidence on first-line treatment with entecavir (ETV) or tenofovir difumarate (TDF) for patients with chronic hepatitis B. METHODS: We searched electronic databases and retrieved articles published up to October 2011, in which the cost effectiveness of ETV

Systemic Gaps and Roadmap for Integrated Care for Post-tuberculosis Lung Disease in Ethiopia: A Narrative Review.

Post-tuberculosis lung disease (PTLD) is a major cause of chronic respiratory impairment globally, yet it remains a critically neglected complication in high-burden, low-resource settings. This narrative review synthesizes evidence on PTLD, with a specific focus on Ethiopia, to analyze the health system gaps and propose actionable solutions. We conducted a narrative review of published and grey literature on the clinical burden, socioeconomic impact, and health system challenges of PTLD, with a particular emphasis on Ethiopia. Evidence was selected for relevance to PTLD phenotypes, diagnosis,

Feasibility and cost-effectiveness of a hospital-based hepatitis B management model in China: the 'Hot Wave Project' experience.

BACKGROUND: Chronic hepatitis B virus (HBV) infection remains a major burden. The "Hot Wave Project," a hospital-wide initiative launched in 2022, aims to enhance HBV care by integrating patient education with systematic screening, referral, diagnosis, treatment, and follow-up. METHODS: We conducted a comparative analysis of hospital-wide HBV care metrics between 2021 (preintervention) and 2024 (postintervention). An interrupted time series (ITS) analysis was conducted on monthly data to evaluate the causal impact of the intervention. A decision-analytic Markov model was used to simulate the

Collaborative Care for Older Adults with low back pain by family medicine physicians and doctors of chiropractic (COCOA): study protocol for a randomized controlled trial.

BackgroundLow back pain is a prevalent and debilitating condition that affects the health and quality of life of older adults. Older people often consult primary care physicians about back pain, with many also receiving concurrent care from complementary and alternative medicine providers, most commonly doctors of chiropractic. However, a collaborative model of treatment coordination between these two provider groups has yet to be tested. The primary aim of the Collaborative Care for Older Adults Clinical Trial is to develop and evaluate the clinical effectiveness and feasibility of a patient-

Community and Academic Synergy for Cancer Survivorship Care Delivery Enhancement (Project CASCADE): a study protocol for a pragmatic, stepped-wedge, cluster-randomized trial in Texas primary care community health centers.

BACKGROUND: In the United States, over 18 million individuals are living with cancer. The majority of these cancer survivors also manage other chronic conditions and receive care from multiple specialists, including oncology, cardiology, and primary care clinicians. However, it remains unclear who holds primarily responsibility for coordinating their care across specialties. Because of its generalist nature, primary care is uniquely suited to deliver whole-person and coordinated care for all conditions for cancer survivors. However, primary care teams experience many challenges delivering high

Patient Perspectives on Leveraging Long-Acting Injectable Antiretroviral Therapy Visits to Improve Chronic Disease Care.

Long-acting injectable (LAI) antiretroviral therapy (ART) for treatment of HIV requires more frequent healthcare interactions than standard care, offering opportunities to enhance monitoring and management of comorbid chronic conditions among people with HIV (PWH). We conducted in-depth interviews with PWH to explore perspectives on LAI-ART uptake and readiness to switch from oral ART, as well as opportunities to integrate chronic disease care into routine LAI-ART visits. Participants who were virally suppressed and had one or more chronic medical conditions were eligible. Transcripts were ana

The science of neonatal high-frequency ventilation.

Despite improvements in respiratory care, ventilator-induced lung injury remains an important cause of morbidity and mortality in neonates who require assisted ventilation. Animal data clearly demonstrate that high-frequency ventilation can be used successfully to reduce lung injury in experimental models of acute lung injury. These models and human research show that the efficacy of high-frequency ventilation is dependent on optimizing functional residual capacity and avoiding lung overinflation. When used with a strategy that promotes lung recruitment, high-frequency ventilation effectively

Telemedicine satisfaction in children with neurological impairment: the role of disease severity, nutritional status, and travel burden.

BACKGROUND: Telemedicine is increasingly used in the follow-up of children with neurological impairment to improve access to specialized care and reduce the burden associated with frequent hospital visits. However, data on caregiver satisfaction and the factors influencing perceived quality of telemedicine services in this population remain limited. Therefore, this study aimed to assess caregiver satisfaction with telemedicine consultations delivered as part of a hybrid model combining routine in-person follow-up and video consultations, and to explore its association with nutritional status a

Unveiling the enigma of acute kidney disease: predicting prognosis, exploring interventions, and embracing a multidisciplinary approach.

Acute kidney disease (AKD) is a critical transitional period between acute kidney injury and chronic kidney disease. The incidence of AKD following acute kidney injury is approximately 33.6%, and it can occur without identifiable preceding acute kidney injury. The development of AKD is associated with increased risks of chronic kidney disease, dialysis, and mortality. Biomarkers and subphenotypes are promising tools to predict prognosis in AKD. The complex clinical situations in patients with AKD necessitate a comprehensive and structured approach, termed "KAMPS" (kidney function check, advoca

The transitional pain service: a narrative review on the approach to managing chronic post-surgical pain.

We conducted a narrative review of the Transitional Pain Service (TPS) to (1) synthesize current evidence on its effectiveness and (2) identify key prerequisites for future TPS implementation. Chronic post-surgical pain (CPSP) is a significant and potentially preventable postoperative complication following major surgery. Established at Toronto General Hospital in 2014, the TPS represents a novel multidisciplinary model designed to mitigate CPSP risk and reduce opioid dependence. A comprehensive search of OVID Medline, supplemented by manual hand-searching, was performed using terms related to
