Wrist fractures.

The benefits of a well-reduced and well-healed wrist fracture are predictable. This knowledge has prompted the development of myriad treatment methods and fixation devices. The choice of open versus closed reduction and the devices used to maintain that reduction could most often be determined with a well-defined and reproducible closed reduction maneuver. With this maneuver, the integrity of the critical volar ulnar corner of the radius is determined as is articular stepoff, metaphyseal comminution, and distal radionular joint (DRUJ) stability. Reconstruction of the subluxed or dislocated DRU

Multiple trauma-induced, secondary osteoarthritis, knee Arthroplasty and technology: Conventional approach to an unconventional scenario.

A 49-year-old man was involved in a high-energy motor vehicle accident. Haemodynamic instability with multiple long bone fractures of lower limbs was the hallmarks of the injury. Closed fragmented fracture of left femur shaft and open displaced supracondylar fracture of the contralateral femur. Closed comminuted high-grade fracture of the Tibia plateau (Schatzker VI) was diagnosed bilaterally. 'Orthopaedic damage control' was initiated with bilateral 'cross knee' external fixation, followed by conversion to open reduction internal fixation of all fracture sits at 8 days later. The patient unde

A severely displaced metaphyseal fracture of the proximal humerus with dislocation of the shoulder in a child.

Fracture of the proximal humerus metaphysis with coexistent dislocation of the shoulder in children is a rare injury. The injury often occurs as a consequence of high velocity trauma. Most fractures of the proximal humerus commonly associated with the epiphysis in children can be treated with closed reduction. We presented a case of 5-year-old girl who sustained this type of fracture- dislocation of the shoulder. Open reduction and internal fixation with multiple smooth K-wires was performed. At two years follow-up, the patient was pain free and regained full range of motion.

Incarceration of the radial head associated with a radial head fracture, radiocapitellar dislocation, and proximal radioulnar translocation.

We present a case report of an individual who sustained a locked proximal radioulnar joint fracture-translocation after a mechanical fall. After a failed attempt at closed reduction, intraoperative exploration revealed that the dislocated and fractured radial head was incarcerated along the medial aspect of the coronoid. After open reduction and internal fixation, the patient went on to uneventful healing and return of adequate function. To our knowledge, this is the first case report documenting an incarcerated fracture-translocation along the radiocapitellar and proximal radioulnar joints th

Anterior elbow dislocation with recurrent instability.

Anterior elbow dislocations are often associated with severe soft tissue lesions. The mechanism of dislocation is usually an anteriorly directed blow on the proximal ulna after a fall on a flexed elbow. Simple anterior elbow dislocations are traditionally treated by closed reduction followed by a short period of immobilisation. Fracture-dislocations are usually unstable after reduction and necessitate surgical management. The authors report a case of acute anterior dislocation with a concomitant radial head fracture that was complicated by recurrent instability. The literature is reviewed to d

Is Surgical Navigation Useful for Treating Zygomatic Arch Fractures?

OBJECTIVE: To report the case of a 58-year-old man with a zygomatic arch fracture, which was well localized and reduced using a surgical navigation system. METHODS: In this clinical report, the authors suggest intraoperative surgical navigation systems are useful diagnostically and for localizing sites of zygomatic arch fractures. RESULTS: The patient underwent successful closed reduction of zygomatic arch fractures using a surgical navigation system. CONCLUSIONS: Surgical navigation is a useful tool for identifying the locations of zygomatic arch fractures and for guiding closed reduction.

The isolated medial humeral epicondyle fracture treated nonoperatively: does fracture displacement change over time?

This study explores the change in the position of the fragment in isolated, displaced, medial humeral epicondyle fractures in children. In this series, 34 patients (mean age 11.0 years) were treated nonoperatively by cast immobilization with no attempt at closed fracture reduction. A statistically significant association was found between the position of the fracture fragment at the time of initial fracture compared with 3 weeks after fracture (P=0.015). Twenty-six of 34 patients (76.5%) showed spontaneous improvement in fragment position. These findings support the practice of treating signif…

A novel single-portal arthroscopic technique for the management of pediatric humeral lateral condylar fractures.

BACKGROUND: Surgical intervention is indicated for significantly displaced lateral humeral condyle fractures (LHCFs) in children. Arthroscopic-assisted closed reduction represents a minimally invasive alternative; however, its widespread adoption has been limited by the technical challenges inherent in both pediatric fracture management and elbow arthroscopy. This study introduces a simplified technique utilizing a single proximal anterolateral portal for arthroscopic-assisted reduction, which has shown promising efficacy and safety. METHODS: A retrospective analysis was performed on 18 pedia

Effectiveness and cost-effectiveness of surgery versus casting for elderly patients with Displaced intra-Articular type C distal Radius fractures: protocol of a randomised controlled Trial with economic evaluation (the DART study).

INTRODUCTION: Current literature is inconclusive about the optimal treatment of elderly patients with displaced intra-articular distal radius fractures. Cast treatment is less invasive and less expensive than surgical treatment. Nevertheless, surgery is often the preferred treatment for this common type of distal radius fracture. Patients with a non-acceptable position after closed reduction are more likely to benefit from surgery than patients with an acceptable position after closed reduction. Therefore, this study aims to assess non-inferiority of functional outcomes after casting versus su

Mid- and long-term outcomes of surgical treatment for distal tibial physeal fractures.

BACKGROUND: Distal tibial fractures are among the most common injuries in childhood. The treatment of distal tibial physeal fractures presents significant challenges for orthopedic surgeons, and potential complications are a major concern. The aim of this study is to evaluate the mid- and long-term outcomes of patients who underwent surgery for a distal tibial physeal fracture. METHODS: This retrospective study included 46 patients who underwent surgery for a distal tibial physeal fracture between 2008 and 2022. Patients were evaluated based on the trauma that caused the fracture, the type of

The influence of the timing of surgery on soft tissue complications and hospital stay. A review of 84 closed ankle fractures.

BACKGROUND AND AIMS: Open reduction and internal fixation of an extensively swollen ankle may lead to wound closure problems, blistering, wound edge necrosis and infection. Accordingly, internal fixation should be accomplished either before or after the period of critical soft tissue swelling. The object of the study was to investigate if the timing of surgery had any influence upon soft tissue complications and hospital stay. PATIENTS AND METHODS: The clinical course of the first 6 postoperative weeks of 84 closed ankle fractures treated by open reduction and internal fixation were reviewed.

Supracondylar fractures of the humerus in children. A modified technique for closed pinning.

Complications in the treatment of displaced supracondylar fractures of the humerus (DSFH), particularly cubitus varus, are a persistent problem. A prospective study was conducted on 20 children with DSFH, using a modified technique to reduce and pin the fracture. All fractures were treated with closed reduction and percutaneous pinning. After the fracture was internally fixed, intraoperative anteroposterior roentgenograms of each distal humerus were compared, using Baumann's angle. The reduction was accepted if Baumann's angle on the fractured extremity was within 4 degrees of that on the norm

Open Reduction Techniques for Supracondylar Humerus Fractures in Children.

Supracondylar humerus fractures are the most common elbow fractures in children. Displaced supracondylar humerus fractures that are associated with neurologic and/or vascular injuries are treated with timely reduction through closed techniques. When closed techniques fail, reduction by open methods is indicated. Controversy exists as to which surgical approach yields the best outcomes in terms of cosmetic and functional results, while minimizing postoperative complications. Open reduction, when indicated, has been shown to yield good outcomes when closed reduction methods fail.

Open postero-lateral dislocation of the left elbow secondary to assault: A case report.

INTRODUCTION AND IMPORTANCE: Simple closed lateral elbow dislocation is rare, often associated with neurovascular injury and closed reduction may be difficult. Open complex postero-lateral dislocation has not been previously reported. CASE PRESENTATION: We present a case of open left posterior lateral elbow dislocation secondary to assault with a cutlass causing an avulsion fracture of the lateral epicondyle and common extensor origin tendon which prevented congruent reduction until the avulsed fragment was fixed with a bone screw. CLINICAL DISCUSSION: The patient had hemorrhagic shock from

Vertical femoral neck fractures in young adults: a closed fixation strategy using a transverse cancellous lag screw.

Vertical femoral neck fractures (Pauwels type III classification) in young adults generally occur as a consequence of high-energy trauma and are frequently seen in association with multiple injuries. Considering the controversies regarding the optimal fixation for this fracture, our aim was to evaluate the clinical outcome of a closed fixation strategy for vertical femoral neck fractures in young adults using two parallel and one transverse cancellous lag screws. This was a single-surgeon, prospective study including 20 young adults with average age of 38.75 years (range 18-59 years) with a hi

Return to Sports After Closed Reduction of Acute Traumatic Posterior Sternoclavicular Joint Dislocations: A Systematic Review.

BACKGROUND: Acute traumatic posterior sternoclavicular (SC) joint dislocation is a serious injury given its potential to cause cardiovascular and airway compromise that typically will require emergent closed reduction. There are limited data on the rate of return to sports (RTS) after this injury pattern when treated in a closed fashion. PURPOSE: To systematically review the literature and evaluate (1) the rate of RTS after closed reduction of posterior SC dislocation and (2) the timeline for RTS after closed reduction of posterior SC dislocation. Nasal Fractures: The Role of Primary Reduction and Secondary Revision.

The nasal bones are among the most commonly fractured bones in the facial skeleton. Proper management of nasal trauma acutely is important in minimizing secondary deformities and impaired function with nasal airway obstruction. Septal hematoma, if present, should be drained right away. Acutely closed nasal reduction and limited septoplasty can be performed. Unrecognized septal fracture may play a role in the failure of closed nasal reduction of fractured nasal bones. Complex nasoorbitoethmoid fractures are approached openly and treated with rigid fixation. Primary use of open rhinoplasty in an
