Validation of a sampling method and liquid chromatography mass spectrometry analysis method for measurement of fentanyl and five other illicit drugs.

With the increased provision of services by health authorities and community organizations allowing supervised inhalation of illicit substances comes concerns about the potential for secondhand exposure to the substances being used, whether in the adjacent community or to workers at the sites. In order to address community concerns surrounding secondhand illicit substance exposure and better protect harm reduction workers, a validated sampling and LC-MS/MS analysis method was developed for 6 illicit drugs: fentanyl, heroin, methamphetamine, cocaine, etizolam, and bromazolam. It was found that

Hygiene and sanitation-related socio-ecological contexts and constraints of communities in districts with persistent trachoma, Southern Ethiopia.

BACKGROUND: Persistence and recrudescence of trachoma remain major challenges delaying progress towards elimination of the disease in many countries, including Ethiopia. Limited evidence exists regarding factors contributing to the persistence and recrudescence of trachoma globally. This study highlights the importance of understanding socio-ecological characteristics of communities within persistent and recrudescent districts. AIM: To explore the socio-ecological contexts and constraints of communities in districts with persistent trachoma. SETTING: Two districts were selected from South Omo

Identification of unvaccinated children, Madagascar.

PROBLEM: Madagascar has a high number of unvaccinated (zero-dose) children. In 2023, an estimated 74% received any dose of the diphtheria-tetanus-pertussis (DTP) vaccine. In southern Madagascar, service gaps, poverty, drought and limited access leave many children unreached by routine immunization. APPROACH: Within the Sorohy ny Aretina Manaova Ezaka Vaksiny initiative, a community-based enumeration and vaccination-status verification was conducted in 16 districts in August 2024-June 2025 under health ministry authorization. A total of 3975 community health workers (CHWs) were recruited and t…

Ivermectin dosing based on physical appearance.

Ivermectin is a safe and effective microfilaricide which has been shown to have a beneficial impact on some onchocercal eye lesions. Current requirements for the distribution of ivermectin are that patients be weighed, and that their dose be determined according to their weight. This requirement increases the cost of distributing the drug to communities in need of treatment and, when scales breakdown, may lead to the suspension of distribution. Data are presented on a simple, alternative method of dose assessment based on the physical appearance of the patient. Fifteen assessors achieved an ov

Healthcare workers' perceptions and experiences of communicating with people over 50 years of age about vaccination: a qualitative evidence synthesis.

BACKGROUND: Infectious diseases are a major cause of illness and death among older adults. Vaccines can prevent infectious diseases, including against seasonal influenza, pneumococcal diseases, herpes zoster and COVID-19. However, the uptake of vaccination among older adults varies across settings and groups. Communication with healthcare workers can play an important role in older people's decisions to vaccinate. To support an informed decision about vaccination, healthcare workers should be able to identify the older person's knowledge gaps, needs and concerns. They should also be able to sh

Barriers and Enabling Factors for Taking PrEP: The Lived Perspectives of Female, Male and Transgender Sex Workers in Thailand.

Despite Thailand's goal of ending AIDS by 2030, pre-exposure prophylaxis (PrEP) uptake remains very low among sex workers (SWs) as they are effectively excluded from programming. This qualitative study explores PrEP pereceptions, decision-making and long-acting injectable (LAI)-PrEP potential among 113 female, male and transgender SWs across seven high-burden provinces. Utilizing 13 focus group discussions and thematic analysis guided by the Health Belief Model, findings revealed uneven and limited PrEP awareness, with many female SWs encountering PrEP for the first time. Key barriers identifi

Coverage of, and compliance with, mass drug administration under the programme to eliminate lymphatic filariasis in India: a systematic review.

India's mass drug administration (MDA) programme to eliminate lymphatic filariasis (PELF) covers all 250 endemic districts, but compliance with treatment is not adequate for the programme to succeed in eradicating this neglected tropical disease. The objective of our study was to systematically review published studies on the coverage of and compliance with MDA under the PELF in India. We searched several databases-PubMed/Medline, Google Scholar, CINAHL/EBSCO, Web of Knowledge (including Web of Science) and OVID-and by applying selection criteria identified a total of 36 papers to include in t

Interventions to prevent needle stick injuries among health care workers.

Needle stick injuries (NSIs) are frequently reported as occupational injuries among health care workers. The health effects of a NSI can be significant when blood-to-blood contact occurs from patient to health care worker. The objective of this study was to evaluate whether the number of NSIs decreased among health care workers at risk in one Dutch academic hospital after introduction of injection needles with safety devices in combination with an interactive workshop. In a cluster three-armed randomized controlled trial, 23 hospital divisions (n=796 health care workers) were randomly assigned

Acceptability of screening for substance use among children at Ugandan health facilities.

BACKGROUND: Substance use among children and adolescents is an emerging public health concern, with studies from some settings reporting substantial levels of alcohol and other substance use among young people. However, in many low- and middle-income countries, including Uganda, the role, feasibility, and implications of routine substance use screening in primary healthcare settings remain poorly understood. Validated tools such as the CRAFFT (Car, Relax, Alone, Forget, Friends, Trouble) screening instrument have been used in other contexts, but evidence on their acceptability among children,
