OBJECTIVES: Cervical intra-epithelial neoplasia is treated by local ablation or lower morbidity excision techniques. Choice of treatment depends on the severity of the disease. The objective of this review was to assess the effects of alternative surgical treatments for cervical intra-epithelial neoplasia.
SEARCH STRATEGY: We searched the Cochrane Gynaecological Cancer Group trials register and MEDLINE up to July 1997.
SELECTION CRITERIA: Randomised and quasi-randomised trials of alternative surgical treatments in women with cervical intra-epithelial neoplasia.

The perinodal cryomodification procedure ablates the surrounding tissue of atrioventricular node, as a consequence, eliminates atrioventricular nodal reentrant tachycardia (AVNRT) and reduces the ventricular response during atrial fibrillation. We performed the procedure in 4 patients: 2 patients with AVNRT, 1 patient with atrioventricular reciprocating tachycardia utilizing a left side accessory pathway and atrioventricular node dual pathways, and 1 patient with tachycardic atrial fibrillation. Fifty eyes with stage 3 plus retinopathy of prematurity (ROP) were treated with cryotherapy. The avascular retina (AR) only was treated in 15 eyes with a mean extent of 8.3 hours of stage 3 ROP; the ridge extraretinal fibrovascular proliferation complex (REFP) was treated in 17 eyes with a mean extent of 7.5 hours; and both locations (BL) were treated in 18 eyes with a mean extent of 7.7 hours. A new cryoprobe for cyclocryocoagulation was investigated in a clinical trial. A group of 15 patients was treated with one step coagulation at 120 degrees C; a control group of 15 patients was treated with a CO2-cooled cryoprobe using circular coagulation in the usual manner. Statistical evaluation showed a significant drop in IOP in both groups after cryocoagulation. The authors present the results of treatment in 51 patients with vitreous haemorrhage in the course of diabetic retinopathy by application of a transconjunctival scleral cryoapplication. In the result of clinical observations it was concluded that this method was successful in the majority of the patients.

Sinoatrial reentry is an uncommon cause of paroxysmal supraventricular tachycardia. This paper presents a case of supraventricular tachycardia, refractory to medical therapy, in which the sinus node formed part or all of the reentrant circuit. The mechanism of the arrhythmia was confirmed by catheter mapping during electrophysiological study and by intraoperative epicardial mapping. Cryosurgical ablation of the right atrium in the region of the sinus node has led to cure of her arrhythmia and emergence of a stable ectopic atrial pacemaker rhythm.

The preliminary experience of one surgeon, using pneumatic retinopexy for 20 consecutive cases of rhegmatogenous retinal detachment, was previously reported. The collaborative experience of six surgeons using pneumatic retinopexy for 100 cases is now reported. This series includes cases with pseudophakia, aphakia, macular detachment, macular breaks, vitreous hemorrhage, trauma, and old detachments. Freezing injuries were produced on the eyelids and lacrimal drainage systems in ten rhesus monkeys. The instrumentation produced both single and double freezing cycles at tissue temperatures of -30 and -60 degrees C. The extent of injury and the course of repair were evaluated by clinical observation and by histologic examination of tissue specimens removed at intervals up to three months after freezing. Fifty consecutive patients with metastatic carcinoma who underwent cryohypophysectomy were studied. Of these, 26 had breast cancer, 19 had prostatic cancer, one had malignant melanoma, one had cancer of the kidney, and three had metastatic adenocarcinoma from an unknown primary tumour. After cryohypophysectomy, excellent pain relief was obtained in 48% of patients, good or acceptable pain relief was obtained in 40%, and poor or no relief in 12%. In 5 cases of glaucoma (2 from trauma, 2 from narrowed drainage angles, 1 secondary to lens extraction), cyclocryotherapy was used to control intraocular pressure. In all cases the intraocular pressure decreased, with the usual result being a cosmetic and painless but blind eye.

A 50-year-old black man with uncontrolled open-angle glaucoma developed a flat anterior chamber after cyclocryotherapy. Because of poor visualization secondary to a cataract, ultrasonography was used to confirm the diagnosis of a large choroidal detachment. After suprachoroidal drainage and reformation of the anterior chamber, the patient has done well.

Cryotherapy is most useful in treating mild and moderate dysplasia of the ectocervix and symptomatic benign chronic cervicitis. It has almost replaced electrocauterization of the cervix and has reduced the need for conization. Cytology and colposcopy are essential in the selection of patients for this procedure and in evaluating their response to therapy.
