Progressive myoclonic epilepsy, nerve deafness and spinal muscular atrophy.

A boy of Finnish descent developed nerve deafness at six years of age, action myoclonus two years later, generalized myoclonic seizures when 16 years old and muscular atrophy at the age of 17 years. Bulbar palsy caused his death from inhalational pneumonia when he was 19 years old. Autopsy disclosed no significant changes in the cerebral cortex, thalamus, striatum, Purkinje cells or dentate nucleus. The most striking histological finding was degeneration of motor neurones in cranial nerves and anterior horns of the spinal cord, with neuroaxonal dystrophy of nucleus gracilis and cuneatus. While…

Pure word deafness after bilateral primary auditory cortex infarcts.

In pure word deafness, comprehension and repetition of speech are impaired, but reading, writing, and spontaneous speech are preserved. Pure word deafness is distinguished from generalized auditory agnosia by the preserved ability to recognize environmental sounds. We examined a patient with pure word deafness associated with bilateral infarctions of the primary auditory cortex, who could use auditory affective intonation to enhance comprehension. The primary auditory cortex seems to be essential for comprehending speech, but comprehension of nonverbal sounds and affective prosody may be media…

Personality system and sudden deafness: a comparative psychological study.

The psychic structure of 30 patients suffering from sudden deafness and of 35 patients suffering from otosclerosis was tested by the FPI. Additionally the intelligence was tested by the MWT-B. It could be shown that deafness on one ear does not cause any significant change of personality. Patients with sudden deafness and additional organic diseases did not show any psychic changes or these changes are due to a basic disease. Patients suffering from idiopathic sudden deafness can be differentiated into two groups: the first group consists of patients with no psychic disease; these patients sho…

Temporal bone histopathology in deafness due to cryptococcal meningitis.

This paper reports on a patient who survived an attack of cryptococcal meningitis eight years prior to his death. A bilateral sensorineural hearing loss had been noted a short time before the patient was admitted to the hospital, and was the only complication after he recovered from the disease. Histopathologic study of the temporal bones showed a similar pattern of pathology in both ears, the most striking finding being a severe loss of spiral ganglion cells in Rosenthal's canal, and of cochlear nerve fibers in the osseous spiral lamina and internal auditory meatus. The vestibular nerve was m

[Deafness due to renal failure. Clinicopathological study (author's transl)].

In this series of 54 cases of chronic renal failure, the largest number of curves showed a curve of a presbyacousia type (3/4 of the total). This hypoacousia may be related to the length of time for which the renal pathology has existed and, parat from group 0 (patients treated without haemodialysis nor grafting), may be correlated with the curve of the speed of nervous conduction. This latter correlation is particularly close in patients with a strictly isolated renal disease. The aetiology of this deafness would appear to be related to the premature aging provoked by chronic renal failure. T…

Reversal of deafness after renal transplantation in Alport's syndrome.

Six patients (five men and one woman) with Alport's syndrome underwent successful renal transplantation (four received kidneys from cadaver donors and two received allografts from living, related donors). One patient who had received a cadaver kidney had substantial hearing improvement and the others had stabilization of hearing. Hearing loss in Alport's syndrome is progressive. The reversal of deafness in one of our patients and stabilization in the others made us wonder whether an inherited enzymopathy had been reversed, which then mitigated the deafness.

Deafness, ichthyosiform erythroderma, corneal involvement, photophobia and dental dysplasia.

Early childhood deafness, congenital non-bullous ichthyosiform erythroderma, corneal involvement, photophobia, chronic blepharoconjunctivitis, hypotrichosis, anhidrosis, hyperkeratosis of the nails and dental dysplasia were the symptoms observed in the 19-year-old girl described in this paper. The literature comprises reports on 8 patients with this syndrome, which McKusick (1975) listed as no. 24215.

[Round window membrane pathology in sudden deafness (author's transl)].

Exploration of the middle ear was performed on 15 patients with sudden deafness that had failed to improve after intravenous infusion of Eupaverin and Rheomacrodex. Three patients had abnormalities of the round window membrane, in 2 there was a perilymphatic fistula and in 1 an abnormal bulging of the membrane with no transmission of perilymphatic fluid movement. These 3 patients presented with a history of exertion or stress prior to the onset of symptoms. Surgical closure ofthe 2 fistulae led to improvement of hearing, and perilymphatic decompression in the third patient improved the tinnitu…
