Generalized Anxiety Disorder and Major Depressive Disorder in Pregnant and Postpartum Women: Maternal Quality of Life and Treatment Outcomes.

OBJECTIVES: Comorbid generalized anxiety disorder (GAD) and major depressive disorder (MDD) in perinatal women is often under-diagnosed, resulting in suboptimal treatment and leading to significant maternal dysfunction. We describe a prospective, longitudinal study of the course, treatment outcomes, and quality of life (QoL) in pregnant and postpartum women with MDD and anxiety disorders. METHODS: Two separate cohorts of women were recruited through the Reproductive Mental Health Program, Women's and Children's Hospital, Vancouver, British Columbia, for pharmacotherapy of depressed mood. One

Increased rate of parental postpartum depression and traumatization in moderate and late preterm infants is independent of the infant's motor repertoire.

BACKGROUND: Moderately and late preterm infants represent a considerable and increasing proportion of infants cared for in neonatal departments worldwide. Parents of preterm infants are at risk of postpartal depression (PPD) and posttraumatic stress disorder (PTSD), and preterm infants are at risk of developmental impairment. AIM: This study aimed to assess (1) the incidence of parental PPD and PTSD in moderate to late preterm infants in comparison to full-term infants and (2) the influence of infants' motor repertoire assessed by Prechtl's general movements and illness severity on parental P

Estradiol modulates anhedonia and behavioral despair in rats and negative affect in a subgroup of women at high risk for postpartum depression.

In an effort to address inconsistencies in the literature, we tested a cross-species estrogen withdrawal model of postpartum depression (PPD) with a series of rodent experiments and a prospective, naturalistic human study. All rats were ovariectomized prior to experimentation. The first rat experiment examined the effects of low- and high-dose estradiol administration and withdrawal on lateral-hypothalamic self-stimulation, a behavioral index of anhedonia, in experimental (n=7) and vehicle-only control animals (n=7). The second rat experiment examined the effects of high-dose estradiol withdra

Perinatal depressive symptoms in HIV-infected versus HIV-uninfected women: a prospective study from preconception to postpartum.

OBJECTIVE: Depression is common among HIV-infected women, predicts treatment nonadherence, and consequently may impact vertical transmission of HIV. We report findings from a study evaluating preconception, pregnancy, and postpartum depressive symptoms in HIV-infected vs. at-risk, HIV-uninfected women. METHODS: We examined the prevalence and predictors of elevated perinatal (i.e., pregnancy and/or postpartum) depressive symptoms using a Center for Epidemiological Studies-Depression (CES-D) scale score of ≥16 in 139 HIV-infected and 105 HIV-uninfected women (62% African American) from the Wome

The association between pregnancy processes, preterm delivery, low birth weight, and postpartum depressions--the need for interdisciplinary integration.

Pregnancy and peripartum/perinatal periods are characterized by significant biologic as well as psychosocial processes and changes that influence the 2 individuals at focus (mother and fetus), as well as their interactions with the immediate environment. Multiple intertwined pathologic pregnancy processes (hormonal, biologic, stress and other mental occurrences) may lead to fetal distress, preterm delivery (PTD), low birth weight (LBW), and other delivery complications as well as to postpartum disorders. PTD and LBW in particular have been demonstrated to be associated with significant mortali

Socioeconomic Status and Postpartum Depression Risk After the Dobbs v Jackson Women's Health Organization Decision, Based on State Trigger Laws.

IMPORTANCE: The 2022 Supreme Court decision in Dobbs v Jackson Women's Health Organization resulted in immediate abortion bans or severe restrictions in 22 US states. While mental health consequences of restricted abortion access have been suggested, their distribution across socioeconomic strata remain unclear for postpartum depression (PPD) among Medicaid populations. OBJECTIVE: To assess associations of state-level abortion bans enacted after Dobbs with the incidence of PPD, focusing on socioeconomic status (SES) differences. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort st

Association between household income and postpartum depression among pregnant women with adequate or inadequate family support: a prospective cohort study.

Household income (HI) or family support may have a buffering effect on postpartum depression (PPD) during pregnancy. However, no study has clarified the impact of family support on the association between HI and PPD. This study aimed to analyze the association between HI and PPD and evaluate the effect of family support on this association. Community-dwelling pregnant women recruited between July 2019 and July 2022 were included in this study. Data on HI and other factors were obtained using self-administered questionnaires. PPD was assessed using the Japanese version of the Edinburgh Postnata

Maternal and neonatal ICU admissions and postpartum outcomes: A cross-sectional observational study on anxiety, depression, and maternal-infant attachment.

PURPOSE: Mother or newborn admission to intensive care units (maternal ICU [MICU] or neonatal ICU [NICU]) is one of the most significant sources of psychological stress during the postpartum period. This cross-sectional observational study aimed to evaluate the effect of intensive care exposure on maternal anxiety, depression, and mother-infant attachment. METHODS: Mothers were allocated to four groups according to ICU admission: MICU only (n = 28), NICU only (n = 69), combined MICU + NICU (n = 46), and control (n = 64). All assessments were conducted within the first postpartum week, prior t

Translating Combined Mother-Baby Interaction Therapy and Interpersonal Psychotherapy for Postpartum Depression from In-Person to Digital Delivery via MommaConnect.

BACKGROUND AND PURPOSE: Postpartum depression (PPD) is a common childbirth complication that impairs the mother-infant relationship and contributes to long-term developmental challenges among children. Although PPD treatment reduces depressive symptoms, it is not necessarily sufficient to improve maternal-infant interaction. In addition, several barriers preclude timely access to care including mental health stigma, limited specialized providers, and challenges with navigating the healthcare system and securing necessary childcare and transportation. Our team developed a 12-session patient- an

Effectiveness of Psychosocial Interventions in Preventing Postpartum Depression Among Teenage Mothers-Systematic Review and Meta-analysis of Randomized Controlled Trials.

Postpartum depression is a significant public health issue that occurs within the first 12 weeks after childbirth. It is more prevalent among teenage mothers compared to adults. However, the findings of the existing interventional studies cannot be readily applied to teenage mothers due to their unique psychosocial concerns. Furthermore, these findings have shown inconsistencies regarding the benefit of psychological and psychosocial interventions in reducing the incidence of postpartum depression. The current review is aimed at investigating the effectiveness of psychosocial interventions in

Optimizing a Novel Smartphone App to Prevent Postpartum Depression Adapted From an Evidence-Based Cognitive Behavioral Therapy Program: Qualitative Study.

BACKGROUND: Low-income pregnant patients are at high risk of postpartum depression (PPD). Mothers and Babies (MB) is a cognitive behavioral therapy-based program that prevents up to 50% of de novo PPD when provided in person to low-income Spanish- and English-speaking people who are pregnant without depression. MB is limited by the need for trained personnel to support it. Transforming MB into a smartphone app may mitigate this key barrier. OBJECTIVE: We aimed to use qualitative data from target end users to create and optimize MBapp, a novel app centered on the MB program. METHODS: Draft wi

Postpartum depression and associated factors among childbearing women from the recent Demographic and Health Survey data of Mozambique: Multilevel analysis.

INTRODUCTION: Postpartum depression is a non-psychotic depressive illness that affects women who have recently given birth. Symptoms of the illness include thoughts of suicide, low self-worth, guilt about not being able to care for their newborn, blaming themselves thoughts, and nervousness. Despite there being many studies conducted at the hospital level there are limited studies conducted at the national level to determine the individual and community level factors. Depression during childbirth can have several detrimental effects, including increased risk of complication and decreased satis

Postpartum Depression and Inflammatory Biomarkers of Neutrophil-Lymphocyte Ratio, Platelet-Lymphocyte Ratio, and Monocyte-Lymphocyte Ratio: A Prospective Observational Study.

OBJECTIVES: Postpartum depression (PPD) is a severe mental health disorder affecting a significant proportion of mothers, often undiagnosed and untreated, with potential long-term effects. While numerous studies have identified risk factors for PPD, the relationship between inflammatory markers and PPD remains unknown. This study aimed to investigate the potential correlation between indirect inflammatory markers, specifically neutrophil-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and monocyte-lymphocyte ratio (MLR), and the risk of developing PPD, assessed by the Edinburgh Pos

Trajectories of cognitive reactivity and its predictive value on postpartum depression in Chinese women: a latent class growth modeling analysis.

Many women are experiencing postpartum depression (PPD) after giving birth. How to recognize and intervene in high-risk PPD women early and effectively remains unknown. Our objective is to describe the latent trajectory groups of cognitive reactivity (CR) in perinatal women, and their relationship to demographic and disease-related factors, as well as investigate the associations with PPD. Data from 321 perinatal women who were evaluated in urban tertiary hospitals in China at three-time points: 32-35 weeks of pregnancy, 1 week postpartum, and 6 weeks postpartum. Latent class growth modeling w

Associations between postpartum depression and assistance with household tasks and childcare during the COVID-19 pandemic: evidence from American mothers.

BACKGROUND: The early postpartum period is recognized cross-culturally as being important for recovery, with new parents receiving increased levels of community support. However, COVID-19-related lockdown measures may have disrupted these support systems, with possible implications for mental health. Here, we use a cross-sectional analysis among individuals who gave birth at different stages of the pandemic to test (i) if instrumental support access in the form of help with household tasks, newborn care, and care for older children has varied temporally across the pandemic, and (ii) whether ac

The impact of disrespectful care during childbirth on maternal mental health: postpartum depression and suicidal ideation 6-9 months after birth.

INTRODUCTION: Postpartum depression is a common mental health disorder that has negative consequences for the mother, the family environment, and the newborn's development. In severe cases, it can be accompanied by suicidal ideation, posing a significant risk to maternal health and the mother-child bond. Recently, studies have begun to investigate how obstetric violence during childbirth, which many women experience, can negatively impact their mental health. The objective of this study was to assess the relationship between inappropriate or disrespectful treatment during birth and the presenc

What comes first: Postpartum depression or low mother-to-infant bonding? A cross-lagged analysis in a low-risk Brazilian sample.

Longitudinal evidence on the direction of the association between postpartum depression (PPD) and mother-to-infant bonding (MIB) remains mixed and is largely based on high-income countries. We investigated the bidirectional associations between PPD symptoms and MIB in a low-risk Brazilian sample during the first year postpartum. One hundred mothers from the São Paulo metropolitan area (32 ± 5.9 years) were assessed at three time points during the first year postpartum (T1: 6 months, T2: 10 months, and T3: 12 months). Mothers completed a Structured Questionnaire (T1), the Edinburgh Postnatal De
