Thoracoabdominal aorta in Marfan syndrome: MR imaging findings of progression of vasculopathy after surgical repair.

PURPOSE: To determine the natural history of vasculopathy of the thoracoabdominal aorta in patients with Marfan syndrome after composite graft repair of the aortic root. MATERIALS AND METHODS: A total 224 magnetic resonance (MR) images obtained in 48 patients with Marfan syndrome over a period of 2.3-9.4 years (mean, 5.0 years) were retrospectively reviewed. On each image, the diameter of the thoracoabdominal aorta was measured and the presence of dissection or major peripheral artery aneurysms was determined. RESULTS: In 31 (65%) of the 48 patients, no statistically significant change (3 mm

The ESTECH remote access perfusion cannula in minimally invasive cardiac surgery.

INTRODUCTION: Peripheral access cardiopulmonary bypass (CPB) and endoclamping of the aorta are prerequisites for performance of minimal access or totally endoscopic cardiac surgery on the arrested heart. We present our experience with the ESTECH remote access perfusion (RAP) cannula in arrested-heart totally endoscopic coronary bypass grafting (AHTECAB) and atrial-septal defect (ASD) repair via minithoracotomy and totally endoscopic ASD repair. PATIENTS AND METHODS: Remote access CPB was performed in 30 patients (17 male), with a median age of 56 years (range, 21-70 years) using the ESTECH RA

Functional Evaluation of Embedded Modular Single-Branched Stent Graft: Application to Type B Aortic Dissection With Aberrant Right Subclavian Artery.

BACKGROUND: Endovascular repair of type B aortic dissection (TBAD) with aberrant right subclavian artery (ARSA) is challenging due to anatomical complexity. The embedded modular single-branched stent graft (EMSBSG) could solve this problem. However, the hemodynamic efficacy of this innovative technique has not been fully assessed. This study aimed to propose morphometric and functional indicators to quantify the outcomes of EMSBSG in treating TBAD with ARSA. MATERIAL AND METHODS: A patient who had TBAD with ARSA underwent EMSBSG implantation was admitted. Computational fluid dynamics (CFD) an

Arch and descending aortic aneurysms: influence of perfusion technique on neurological outcome.

OBJECTIVE: Although cannulation of the femoral artery is used routinely for thoracic aortic operations with hypothermic circulatory arrest, retrograde perfusion through the descending aorta carries the risk of cerebral malperfusion or embolism. We have, therefore, routinely used a central cannulation technique for distal arch and descending aortic operations since 1995. In this study, we compared neurological outcome in consecutive patients undergoing femoral versus ascending aortic perfusion for these aneurysms. METHODS: Between 1987 and 1998, 61 patients underwent aortic resection with circ

Suspected Mesenteric Malperfusion in Acute Type A Aortic Dissection During Pregnancy: A Case Report and Review of Reperfusion-First Strategies.

Aortic dissection during pregnancy is rare but potentially catastrophic. Acute Type A aortic dissection generally requires urgent surgical repair. When visceral malperfusion is present, however, the optimal sequence of interventions may be complex, particularly when pregnancy and recent cesarean delivery increase operative risk. Endovascular reperfusion followed by delayed aortic repair has been proposed in selected patients, although evidence supporting this strategy in pregnancy remains limited. A 37-year-old woman at 31 weeks' gestation presented with chest and back pain. Initial CT pulmona

Biomechanics of aortic wall failure with a focus on dissection and aneurysm: A review.

Aortic dissections and aortic aneurysms are fatal events characterized by structural changes to the aortic wall. The maximum diameter criterion, typically used for aneurysm rupture risk estimations, has been challenged by more sophisticated biomechanically motivated models in the past. Although these models are very helpful for the clinicians in decision-making, they do not attempt to capture material failure. Following a short overview of the microstructure of the aorta, we analyze the failure mechanisms involved in the dissection and rupture by considering also traumatic rupture. We continue

[Acute aortic dissection and peripheral ischemia. Diagnostic and therapeutic problems. Remote control using nuclear magnetic resonance].

This study concerns 7 cases of acute aortic dissection associated with visceral and/or lower limb ischemia. Only those cases are included which raised diagnostic and therapeutic problems. Patients were excluded who had purely angiographic involvement of an aortic branch and minor rapidly resolving ischemic syndromes. Five of the 7 patients presented type B (type III or distal) and 2 type A (type I or proximal) dissection. All patients received anti-hypertensor medical treatment. All but one had undergone surgery at least once at the acute stage. Five had been followed up and monitored by magne
