Modification in aortic arch replacement surgery.

OBJECTIVE: We modified the conventional aortic arch replacement procedure to avoid circulation arrest and a prolonged extracorporeal circulation time, especially in cases of acute aortic dissection. We herein present our experience with a modified branch-first approach to acute aortic dissection, with anastomosis of the supra aortic vessels prior to commencing cardiopulmonary bypass. METHODS: Since 2012, 41 patients (aortic dissection, 36; arch aneurysm, 5) have undergone the modified procedure. Procedurally, the implanted graft was used as a landing zone for second-stage endovascular stent-g

Endovascular Coil Embolization with Low-Profile Visualized Intraluminal Support Junior Stent for Ruptured Dissecting Aneurysm of Proximal Superior Cerebellar Artery-Case Report and Literature Review.

BACKGROUND: Superior cerebellar artery (SCA) aneurysm arising from the SCA itself is rare, and treatment of this aneurysm is challenging because of the important anatomic structures, such as the perforating arteries to the brainstem and cranial nerves. We describe a successful coil embolization with a Low-profile Visualized Intraluminal Support Junior (LVIS Jr.) stent for the proximal SCA dissecting aneurysm. CASE DESCRIPTION: A 50-year-old woman presented with right oculomotor nerve palsy and subarachnoid hemorrhage. Cerebral angiography showed a dissecting wide neck aneurysm at the anterior

Influence and critique of the INSTEAD Trial (TEVAR versus medical treatment for uncomplicated type B aortic dissection).

Endovascular stent-grafting represents a novel nonsurgical therapeutic concept for type B aortic dissection in both the acute and subacute/chronic settings with unknown impact on outcomes. In a prospective trial, 140 patients with clinically stable type B dissection were randomly subjected to elective stent-graft placement in addition to optimal medical therapy (n = 72) or to optimal medical therapy (n = 68) with surveillance (arterial pressure according to World Health Organization guidelines, ≤120/80 mm Hg) and recurrent computed tomography imaging. Primary endpoint was 2-year all-cause mort

The association of connective tissue disorders with cervical artery dissections.

A predisposing weakness of the vessel wall has been assumed in patients with spontaneous cervical artery dissections (sCAD). Skin biopsies from many patients with sCAD show mild connective tissue alterations. However, their assessment depends on an invasive and highly specialized technique. Clinical signs of connective tissue disease are absent in the majority of CAD patients. In this review we document that only very few CAD patients are affected by known inherited connective tissue disorders like Ehlers-Danlos syndrome, Marfan syndrome or Osteogenesis Imperfecta. In a second part of this rev

[Acute type A aortic dissection combined with surgical treatment with acute myocardial infarction].

Acute myocardial infarction, as a result of coronary malperfusion caused by acute type A aortic dissection, has been identified as one of significant factors relating to operative mortality. This complication could be diagnosed with a combination of electrocardiography and echocardiography in acute phase. However, the indication of coronary angiography and/or intervention has been controversial as it is time-consuming and renders additional stress to a critical patient requiring an emergency operation. We report a case of myocardial infarction successfully treated with percutaneous translumina

Thoracic aorta dissection associated with aberrant right subclavian artery: treatment with endovascular stent-graft placement.

Dissecting aneurysm is the condition produced by separation of the layers of the arterial wall by circulating blood. Although rare, the coexistence of aortic dissection and aberrant right subclavian artery may be catastrophic. In this study we report the endovascular treatment of a patient with thoracic aorta dissection associated with aberrant right subclavian artery. Aortic clamping proximal to the left subclavian artery in a patient with an aberrant right subclavian artery slows or eliminates flow to both vertebral arteries. Endovascular repair eliminates the complications associated with a

Endovascular management of traumatic axillary artery dissection--a case report and review of the literature.

A 43-year-old man was admitted to this hospital with a cool, slightly numb, left upper extremity after trauma. Physical examination revealed a cool left forearm and hand, with no distal pulses. An intraoperative angiogram demonstrated short-segment dissection and occlusion of the left axillary artery. A 10 mm x 40 mm self-expanding stent was placed across the intimal flap to reinstitute peripheral flow. Early postoperative duplex scanning showed normal flow through the axillary artery.

Type A aortic dissection developing during endovascular repair of an acute type B dissection.

PURPOSE: To describe an extension of type B dissection into type A during endoluminal treatment of acute type B dissection. CASE REPORT: A 57-year-old man with acute type B dissection underwent endovascular repair using 2 Excluder stent-grafts. Before releasing the second stent-graft, the patient complained of mild central chest pain, headache, and visual flashes, but no hemodynamic changes were noticed. An angiogram, performed soon afterwards, showed a retrograde extension of the dissection through the aortic arch involving both the left carotid artery and the ascending aorta. The procedure

Extended repair of thoracic aorta through left thoracotomy after aortic root replacement.

A patent distal false lumen after repair of type A aortic dissection often poses serious late complications. We present a successful repair of dissecting aortic aneurysm through left thoracotomy, extending from the ascending to descending thoracic aorta after composite graft replacement of the aortic root. Although staged operations, including the elephant trunk procedure, are usually chosen for remaining extensive aortic disease after replacement of the aortic root or the ascending aorta, a single-stage repair from the ascending to the descending thoracic aortas through left thoracotomy can b

[Surgical treatment for chronic dissecting aneurysm (DeBakey type I)--a case using "elephant trunk" and "aortic tailoring"].

We present a surgical case of a 54-year-old man with chronic dissecting aortic aneurysm (Type I). At the first operation, ascending aorta and aortic arch were replaced with a tube graft under cardiopulmonary bypass and selective cerebral perfusion. Elephant trunk method was employed for the second operation. At the second operation, graft replacement of the proximal two-thirds of the descending aorta was performed, and the lower third of the descending aorta was tailored. Aortic tailoring consisted of the longitudinal aortomy and the removal of the initimal flap from the distal part of the des

Application of the reversed elephant trunk technique during total thoracoabdominal replacement in patients with Marfan's syndrome.

The reversed elephant trunk operation has been applied in patients with extensive aortic involvement as a scheduled staged operation. We report application of the same technique in two patients with Marfan's syndrome. The two patients underwent total replacement of the thoracoabdominal aorta for a DeBakey IIIb aortic dissection. The proximal end of the prosthetic graft was invaginated to facilitate future proximal operation. No complication related to the trunk had been observed during the follow-up period.

[Combined acute ascending aneurysm and chronic descending thoracic aneurysms].

A 66-year-old woman with a known chronic descending thoracic dissecting aneurysm was admitted following a chest X-ray which revealed a widening in the mediastinum and an increased cardiothoracic ratio. Echocardiography and chest CT showed a combined ascending thoracic dissecting aneurysm with a tripple lumen, pericardial effision, moderate aortic regurgitation, and the chronic descending thoracic dissection with incompletely thrombosed aneurysm. The chest CT also showed a normal aortic arch. The diagnosis was then made as combined acute ascending aortic dissection (DeBakey II) and chronic desc

[Surgical treatment for Stanford type A acute aortic dissection].

From March 1983 to September 1994, 53 consecutive patients underwent an emergency operation for type A acute aortic dissection. Eighteen patients before 1988 (Group I) underwent simple ascending aortic replacement (n = 13) and composite graft replacement (n = 5) by conventional method. Thirty-five patients after 1989 (Group II) underwent extended ascending aortic replacement (n = 17), hemiarch replacement (n = 6), total arch replacement (n = 8) and composite graft replacement (n = 4) using a brief period of circulatory arrest. The overall hospital mortality rate was 36.8% in Group I and 25.7%

Acute aortic dissection in a patient with osteogenesis imperfecta.

A case of osteogenesis imperfecta complicated with acute type A aortic dissection is presented. Emergency graft replacement of the ascending aorta was performed successfully despite the anticipated difficulties with tissue friability. Therefore, such an operation is suggested to be worthy of consideration and feasible in patients with osteogenesis imperfecta.

[Rupture of dissecting aortic aneurysm associated with the right-sided aortic arch and anomalous course of the left brachiocephalic vein--a case report].

A case of ruptured dissecting aortic aneurysm (DeBakey IIIb) associated with the right sided aortic arch and anomalous course of the left branchiocephalic vein was reported. A sixty-nine-year-old female suddenly had the severe back pain and soon fell into shock. The diagnosis of a ruptured dissecting aortic aneurysm associated with the right sided aortic was obtained on CT scanning. CT films also showed the left brachiocephalic vein behind the ascending aorta. Emergency operation was performed through median sternotomy and left thoracotomy. The descending aorta, forming an aneurysm with the ab
