The hybrid method of treating thoracic aortic pathology is devoid of the disadvantages of traditional open surgery and, at the same time, has a broader range of applications than the endovascular method. From 2014 to 2019, we researched 122 patients with thoracic aortic pathology treated with the hybrid method (open surgery with thoracic endovascular aortic repair (TEVAR) at the National M. Amosov Institute of Cardiovascular Surgery National Academy of Medical Science of Ukraine. In the general group, 34 patients had a descending aortic aneurysm without dissection or rupture; 71 patients had a

While thoracic endovascular aortic repair is an effective treatment option for descending thoracic aorta pathology, it does have limitations. The main limitation is related to the anatomical difficulties when disease involves the aortic arch. A fenestrated, branched aortic stent graft and hybrid operation has been introduced to overcome this limitation, but it is a custom-made device and is time consuming to manufacture. Furthermore, these devices cannot be used in an emergency setting. We report two patients with massive descending thoracic aortic aneurysm and ruptured aortic dissection very

BACKGROUND: Aneurysms affecting the aorta are a common condition associated with high mortality as a result of aortic dissection or rupture. Investigations of the pathogenic mechanisms involved in syndromic types of thoracic aortic aneurysms, such as Marfan and Loeys-Dietz syndromes, have revealed an important contribution of disturbed transforming growth factor (TGF)-β signaling.
OBJECTIVES: This study sought to discover a novel gene causing syndromic aortic aneurysms in order to unravel the underlying pathogenesis.
METHODS: We combined genome-wide linkage analysis, exome sequencing, and ca

OBJECTIVES: Arch replacement combined with antegrade stent grafting of the descending aorta represents a hybrid surgical approach for extensive thoracic aortic disease. This multicentre study evaluates the early results of this method in complex aortic dissection (AD).
METHODS: Retrospective data acquisition was achieved by institution of an international registry. A hybrid stent graft with integrated vascular prosthesis for arch replacement (E-vita open®) was used. From January 2005 to March 2009, 106 patients (mean age 57; 77% male) with complex AD (55 acute, 51 chronic) were studied.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) in children and adolescents after blunt traumatic aortic injury (BTAI) is being performed increasingly despite no endovascular graft being approved for TEVAR in this population. The smaller diameter of the aorta and access vessels and steeper angle of the aortic arch pose specific challenges for TEVAR in this population. Moreover, data are lacking regarding medium to long term complications. This case presents an adolescent patient who underwent TEVAR for BTAI and suffered a focal aortic dissection several months later.

A 63 year-old woman was admitted to the emergency department with vertigo, nausea, and vomiting. On arrival, she was fully oriented and cooperative. She denied any pain in her chest, neck, back, or abdomen. A bruit was heard on both sides of her neck. Cranial computed tomography (CT) revealed no abnormality, while thoracic CT disclosed dissection in the ascending aorta, aortic arch, and bilateral common carotid arteries. After several hours, the patient underwent vascular surgery. She had an uneventful course and was discharged without any sequelae after 10 days. Vertigo is a rare presentation

The success of diffusion models in medical imaging highlights their potential to generate high-quality synthetic datasets that closely resemble real clinical data, addressing limited dataset availability and patient privacy concerns. We present DiffusionTBAD, a novel text-to-image diffusion-based pipeline for synthesizing diagnostically accurate computed tomography angiography (CTA) images of type B aortic dissection (TBAD). Using few-shot learning, DiffusionTBAD fine-tunes a diffusion model guided by textual prompts to capture the distinct features and variability of TBAD cases. The synthetic

CASE: A 54-year-old man presented with a giant T10-L2 chordoma. The tumor encased the thoracoabdominal aorta, presenting a significant challenge for en bloc resection. We used a novel anterior transperitoneal and transdiaphragmatic approach. By longitudinally incising the diaphragm along the aortic hiatus, we obtained direct visualization to safely dissect the aorta from the tumor in both thoracic and abdominal cavities. This facilitated a successful 2-stage en bloc spondylectomy and reconstruction with a three-dimensional-printed vertebral body. At 24 months, the patient was neurologically in

BACKGROUND: Recent studies show that hyperactivation of mTOR (mammalian target of rapamycin) signaling plays a causal role in the development of thoracic aortic aneurysm and dissection. Modulation of PP2A (protein phosphatase 2A) activity has been shown to be of significant therapeutic value. In light of the effects that PP2A can exert on the mTOR pathway, we hypothesized that PP2A activation by small-molecule activators of PP2A could mitigate AA progression in Marfan syndrome (MFS).
METHODS: Two distinct mouse models of MFS underwent daily oral administration of small-molecule activators of

Traumatic thoracic aortic injury represents one of the most life-threatening complications of blunt chest trauma and is associated with substantial morbidity and mortality if not promptly recognized and treated. Rapid deceleration mechanisms may produce intimal disruption of the aortic wall, resulting in dissection, false lumen formation, and potential aortic rupture. We report the case of an 18-year-old previously healthy male patient who presented with chest pain and hemothorax three days after a motorcycle accident. Computed tomography angiography (CTA) demonstrated a Stanford type B trauma

OBJECTIVE: Aortic remodeling of the thoracic aorta has been studied in patients treated with medical or endovascular therapy for the treatment of acute aortic dissections; however, particular attention has not yet focused on identifying specific growth patterns and rates across all aortic zones. Additionally, previous studies have not delineated between dissections with and without visceral aortic involvement, and we hypothesize that these two cohorts may exhibit distinct differences. The aim of this study is to investigate aortic behavior over time in medically managed acute Society for Vascu

BACKGROUND: The long-term efficacy of stent extension in thoracic endovascular aortic repair (TEVAR) for Stanford type B aortic dissection (TBAD) has not been thoroughly evaluated.
METHODS: This retrospective multicentre study included patients with TBAD undergoing TEVAR between 2014 and 2021. On the basis of distal descending aorta coverage, patients were stratified into stent extension (SE) and nonstent extension (non-SE) groups. Stent extension was defined as the intentional distal placement of an additional covered stent to lengthen the repair zone within the descending aorta. After 1:1 p

BACKGROUND: Endovascular repair has emerged as a less invasive alternative treatment for residual arch disease after limited repair of type A aortic dissection. However, cervical or supraclavicular incisions are generally required to establish access for branch bridging stent-graft deployment. This study aimed to assess the feasibility of total percutaneous endovascular arch repair using an innovative off-the-shelf unibody endograft system.
METHODS: Patients who underwent endovascular aortic arch repair with a unibody endograft system for residual arch dissection from September 2023 to Januar
