[Sigmoidoperianal fistula associated with diverticulitis and Cul de Sac situation--diagnostics and therapy].

We report on the case of a 64-year-old female patient who presented herself in our outpatient clinic because of a perianal fistula with recurrent abscesses. We describe the step diagnostics and the surgical treatment of the causal sigmoido-perianal fistula with diverticulitis and Cul de sac situation. Clinical examination, fistulography, colonoscopy and MRT were part of the precise representation and preparation for the high anterior rectosigmoidal resection with simultaneous rectopexy according to Sudeck which were performed without complications. The sigmoidoperianal fistula must be taken in…

Emergency surgery for colonic diverticulitis: differences between right-sided and left-sided lesions.

BACKGROUND AND AIMS: Right-sided diverticulitis is rare in Western societies but is not uncommon in Asian countries. Many of the patients are operated with the presumptive diagnosis of appendicitis. This study compared the results of emergency surgery for patients with right-sided and left-sided diverticulitis. PATIENTS AND METHODS: Demographic data, type of operation, and surgical outcome were recorded in 60 patients who had undergone emergency surgery for colonic diverticulitis (37 right-sided, 23 left-sided). RESULTS: Patients with right-sided disease were significantly younger (mean age …

Acute diverticulitis of the cecum and ascending colon: thin-section helical CT findings.

OBJECTIVE: Our objective was to describe thin-section helical CT findings of acute diverticulitis of the cecum and ascending colon in 12 patients. CONCLUSION: Thin-section helical CT is able to reveal inflamed diverticula in acute diverticulitis of the cecum and ascending colon, which are diagnostic of this disease. Preservation of the enhancing pattern of the colonic wall was the most common helpful ancillary finding.

Virtual Care of Acute Diverticulitis: A Phase I/II Randomised Clinical Feasibility Trial.

BACKGROUND: Acute uncomplicated diverticulitis is commonly managed in hospital in Australia. Virtual care may offer reassurance to clinicians and enable out-of-hospital management by providing remote monitoring. This study evaluated the feasibility, safety and acceptability of a virtual care pathway for acute diverticulitis (iCAD). METHODS: This Phase I/II trial comprised a single-centre pilot of patients managed via the iCAD pathway, followed by a multicentre randomised trial comparing iCAD with conventional inpatient care. The iCAD model involved initial in-person assessment and intravenous…

Two Cases of Acute Diverticulitis Following Ocrelizumab Infusion in Patients With Multiple Sclerosis.

Ocrelizumab is an anti-CD20 monoclonal antibody used to treat primary progressive and relapsing-remitting multiple sclerosis. Several prior case reports have demonstrated colitis in association with ocrelizumab infusion, and one case report has shown ocrelizumab-associated diverticulitis. We report on two cases in which ocrelizumab treatment of multiple sclerosis was complicated by acute diverticulitis. A 50-year-old woman and a 41-year-old man, both with relapsing-remitting multiple sclerosis, presented with acute abdominal pain. One patient had no known gastrointestinal history while the oth…

Current diagnosis and management of acute colonic diverticulitis: What you need to know.

Acute colonic diverticulitis is a common disease treated by acute care surgeons. Acute uncomplicated colonic diverticulitis involves thickening of the colon wall with inflammatory changes and less commonly requires the expertise of a surgeon; many cases may be treated as an outpatient with or without antibiotics. Complicated diverticulitis involves phlegmon, abscess, peritonitis, obstruction, stricture, and/or fistula and usually requires inpatient hospital admission, treatment with antibiotics, and consideration for intervention including operative management. This review will discuss what th…

Endoscopic findings after CT proven acute diverticulitis: a systematic review and meta-analysis.

BACKGROUND: Traditionally, international guidelines recommend patients with acute diverticulitis should be followed up with a colonoscopy 6-8 weeks after discharge. However, the need for an interval colonoscopy has been increasingly challenged in the setting of computed tomography (CT). Previous meta-analyses have included studies which combined suspected rather than imaging confirmed diverticulitis and often without correlation with endoscopic findings. This meta-analysis aims to investigate endoscopic findings of patients with CT confirmed diverticulitis. METHODS: An electronic search of Me…

Multifocal pyoderma gangrenosum secondary to subclinical diverticulitis: case report and brief literature review.

Pyoderma gangrenosum is characteristically associated with inflammatory bowel disease. However, the association between this neutrophilic dermatosis and diverticular disease is scarcely mentioned in the literature. Diverticulitis should be included in the differential diagnosis in patients with pyoderma gangrenosum and gastrointestinal complaints, or even in asymptomatic patients, particularly in the elderly. Misdiagnosis can lead to inadequate treatments and serious complications.

Necrotizing fasciitis from perforated sigmoid diverticulitis with subsequent pyoderma gangrenosum: a case report.

Postsurgical pyoderma gangrenosum is a very rare form of cutaneous ulceration that is poorly recognized outside of dermatology and in some circumstances has been mistaken for necrotizing fasciitis. Here, we present a rare case of sigmoid diverticulitis with left ureter obstruction that perforated and quickly progressed into necrotizing fasciitis of the left buttock and leg via retroperitoneal spread in an immunocompetent patient. Nearly a year after intense surgical therapy, the patient rapidly developed ulcerating lesions over the left hip which presented a diagnostic dilemma. These were init…

Multicentre international trial of laparoscopic lavage for Hinchey III acute diverticulitis (LLO Study).

BACKGROUND: Laparoscopic lavage was proposed in the 1990s to treat purulent peritonitis in patients with perforated acute diverticulitis. Prospective randomized trials had mixed results. The aim of this study was to determine the success rate of laparoscopic lavage in sepsis control and to identify a group of patients that could potentially benefit from this treatment. METHODS: This retrospective multicentre international study included consecutive patients from 24 centres who underwent laparoscopic lavage from 2005 to 2015. RESULTS: A total of 404 patients were included, 231 of whom had Hin…

Laparoscopic Lavage Versus Primary Resection for Acute Perforated Diverticulitis: Review and Meta-analysis.

OBJECTIVE: To compare clinical outcomes after laparoscopic lavage (LL) or colonic resection (CR) for purulent diverticulitis. BACKGROUND: Laparoscopic lavage has been suggested as an alternative treatment for traditional CR. Comparative studies to date have shown conflicting results. METHODS: Electronic searches of Embase, Medline, Web of Science, and Cochrane databases were performed. Weighted mean differences (WMD) were calculated for effect size of continuous variables and pooled odds ratios (POR) calculated for discrete variables. RESULTS: A total of 589 patients recruited from 3 random…

Severity of Diverticulitis Does Not Influence Abdominal Complaints during Long-Term Follow-Up.

BACKGROUND: Diverticulitis can lead to localized or generalized peritonitis and consequently induce abdominal adhesion formation. If adhesions would lead to abdominal complaints, it might be expected that these would be more prominent after operation for perforated diverticulitis with peritonitis than after elective sigmoid resection. AIMS: The primary outcome of the study was the incidence of abdominal complaints in the long-term after acute and elective surgery for diverticulitis. METHODS: During the period 2003 through 2009, 269 patients were operated for diverticular disease. Two hundred…

Development and validation of a diagnostic prediction model distinguishing complicated from uncomplicated diverticulitis.

OBJECTIVES: Most diverticulitis patients (80%) who are referred to secondary care have uncomplicated diverticulitis (UD) which is a self-limiting disease and can be treated at home. The aim of this study is to develop a diagnostic model that can safely rule out complicated diverticulitis (CD) based on clinical and laboratory parameters to reduce unnecessary referrals. METHODS: A retrospective cross-sectional study was performed including all patients who presented at the emergency department with CT-proven diverticulitis. Patient characteristics, clinical signs and laboratory parameters were …
