PSOAS MUSCLE ABSCESS FOLLOWING CESAREAN SECTION: A CASE REPORT.

BACKGROUND: Psoas muscle abscess is a rare but potentially life-threatening condition and is exceptionally uncommon after cesarean section. Its nonspecific clinical presentation may delay diagnosis. CASE PRESENTATION: A 37-year-old multiparous woman (gravida 4, para 2, with two previous spontaneous abortions) at 34 weeks of gestation underwent emergency repeat cesarean section for pathological cardiotocography, uterine hypertonia, threatened fetal asphyxia, and previous cesarean section. The pregnancy was complicated by pregnancy-induced hypertension. The cesarean section was uncomplicated, a

Domain-specific functional outcome prediction in stroke rehabilitation: A multicenter artificial intelligence study.

BACKGROUND: Stroke is a leading cause of long-term disability worldwide, yet existing clinical decision support tools rely on global disability metrics, such as the modified Rankin Scale, which do not adequately reflect patient-centered rehabilitation or recovery goals. OBJECTIVE: We aimed to develop, validate, and implement artificial intelligence (AI)-based clinical support models for predicting domain-specific functional outcomes after stroke across multiple rehabilitation institutions and timepoints. METHODS: We utilized prospective and retrospective multicenter data collected from patie

Effect of an ERAS Nursing Protocol on Recovery After Minimally Invasive Lumbar Fusion: A Randomized Trial.

BACKGROUND: Perioperative care for minimally invasive lumbar interbody fusion (MIS-LIF) is often suboptimal. Evidence for nurse-led Enhanced Recovery After Surgery (ERAS) protocols in this population is lacking. Our study was to evaluate the impact of a structured nursing management protocol based on ERAS principles on postoperative recovery outcomes in patients undergoing MIS-LIF for lumbar spinal stenosis. METHODS: In a prospective randomized controlled trial, 70 patients scheduled for single-level MIS-LIF were randomly assigned to either an ERAS group (n = 35) or a conventional care group

Lifelong Sequelae of a Childhood Tibial Fracture: Severe Procurvatum Tibial Malunion Leading to Secondary Knee Osteoarthritis.

The sequelae of tibial malunion following surgical repair in the pediatric population are well documented. Persistent tibial malunion introduces varus or valgus forces that predispose patients to knee compartment overload, limiting ambulation and promoting osteoarthritis and osteoporosis. To prevent these sequelae, tibial malunions are categorized by deformity severity to guide surgical interventions, including intra-articular, extra-articular, and combined approaches. A male in his early 70s presented to the orthopedic clinic for reevaluation of severe right knee osteoarthritis secondary to a

Ultrasound-guided erector spinae plane block versus anterior quadratus lumborum block for postoperative analgesia after percutaneous nephrolithotomy: A randomized controlled study.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is associated with significant postoperative pain. Ultrasound-guided erector spinae plane block (ESPB) and anterior quadratus lumborum block (QLB) are increasingly used for analgesia; however, direct comparative evidence in PCNL remains limited. This randomized controlled study compared their analgesic efficacy following unilateral PCNL. METHODS: Eighty-four adult patients (American Society of Anaesthesiologists I-III) were randomized to receive ESPB or anterior QLB. Three patients in each group had block failure. The primary outcome was time to

Case Report: Early rehabilitation initiated in the intensive care unit for myasthenic crisis.

Myasthenia gravis is an autoimmune disorder of the neuromuscular junction characterized by fatigable muscle weakness. Myasthenic crisis is a life-threatening condition characterized by acute respiratory failure resulting from severe weakness of the respiratory muscles. Currently, no established guidelines exist for physiological assessment at the time of mobilization, or for the content and intensity of exercise therapy, in patients with severe myasthenia gravis presenting with myasthenic crisis. We report a case of newly diagnosed myasthenia gravis with myasthenic crisis wherein exercise ther

Combined Transcranial-endonasal Reconstructive Surgery for Cerebrospinal Fluid Leakage Resulting from Traumatic Anterior Skull Base Fractures Involving the Parasellar Region.

Extensive traumatic anterior skull base fractures from the frontal sinus to the parasellar region are frequently accompanied by multiple dural defects that cause persistent cerebrospinal fluid (CSF) leakage. Conventional transcranial reconstruction using a frontal periosteal flap is frequently insufficient, and parasellar dural defects are often deep, complex, and difficult to identify. In this report, we describe a combined transcranial-endonasal reconstructive technique and report our experience. Simultaneous combined transcranial and endoscopic surgery was performed in three patients with C

Functional results of free tissue transfer for complex heel-calcaneal defects.

BACKGROUND: Loss of soft tissue in heel-calcaneal region is frequently caused by trauma, infection, or tumors. Limited availability of similar tissue becomes challenging, therefore, the use of free tissue transfer offers a solution. Our aim is to describe long term functional outcome of different free tissue transfers for these defects. PATIENTS AND METHODS: We retrospectively reviewed 24 consecutive cases of the heel-calcaneal defect between January 2009 and December 2014. The free fasciocutaneous perforator (FCP) flaps were performed in 14 cases and free muscle flaps with skin graft in 10 c

Bone loss during the acute stage following burn injury.

The aim of this study was to investigate the effect of burn injury on bone metabolism and bone densitometry in the early period. Twenty-one patients with >25% total body surface area (TBSA) burns and 20 healthy controls participated. TBSA burned, ambulation, and functional status were recorded. After 30 days, we measured bone mineral densities of the L1-L4 vertebrae, the left distal forearm, and the left proximal femur in the patients. At 1 and 4 weeks after the burn, changes in bone turnover were assessed in patients by changes in deoxypyridinoline levels in the urine and osteocalcin in the s

Atraumatic Bilateral Calcaneal Insufficiency Fractures in a Postmenopausal Woman With Severe Osteoporosis: A Case Report.

Calcaneal insufficiency fractures are uncommon and often overlooked because their early symptoms resemble more prevalent causes of heel pain. A 68-year-old postmenopausal woman presented with two months of progressive bilateral heel pain and swelling without trauma. Radiographs revealed nondisplaced bilateral calcaneal fractures characterized by subtle vertical lucencies and mild cortical discontinuity. Dual-energy X-ray absorptiometry confirmed severe osteoporosis, and laboratory evaluation excluded secondary metabolic bone disease. The initial symptoms mimicked plantar fasciitis and Achilles

Gene editing and modulation for Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive muscle disease caused by loss of dystrophin protein, encoded by the DMD gene. DMD manifests early in childhood as difficulty walking, progresses to loss of ambulation by the teens, and leads to death in early adulthood. Adeno-associated virus-vectorized gene therapies to restore dystrophin protein expression using gene replacement or antisense oligonucleotide-mediated pre-mRNA splicing modulation have emerged, making great strides in uncovering barriers to gene therapies for DMD and other genetic diseases. While this first-generation of DMD th

Osteotendinous Suturing for Anchorage of Hip Abductor Mechanism in Total Hip Arthroplasty through a Lateral Approach: A Technical Note.

INTRODUCTION: We describe a novel technique for approximation and reattachment of the capsule, hip abductors, and vastuslateralis muscles during closure of the arthrotomy and surgical wound, after total hip arthroplasty through a modified direct lateral approach. This anatomical restoration of soft tissues could thereby lead to early post-operative rehabilitation and recovery. CASE REPORT: This is a method of osteotendinous suturing that could lead to reestablishing the functional continuity of the gluteus minimus, medius muscles, and the anterior capsule through drill holes in the greater tr

Pompe disease: a neuromuscular disease with respiratory muscle involvement.

Pompe disease is a single disease continuum that includes variable neuromuscular symptoms and rates of progression. However, specific clinical features, such as an early onset of respiratory problems preceding limb muscular weakness, distinguish Pompe disease from other neuromuscular diseases in which respiratory insufficiency occurs after loss of ambulation. The management of Pompe disease also differs from other neuromuscular diseases in that specific treatment is now available, making early recognition of the disease a priority. The results from clinical trials with recombinant human acid a

Prevention of thromboembolic phenomena.

The leading cause of death from total hip replacement is pulmonary embolism. Prophylactic anticoagulation has been effective in decreasing thromboembolic phenomena but has been associated with a high rate of complications. A low dose warfarin prophylaxis combined with anti-embolic hose, elevation of the legs and early ambulation was employed in 415 total hip replacements. Clinical thrombosis occurred in 2.4 per cent and there was 1.45 per cent pulmonary emboli but none resulted in death. Two deaths from non-embolic causes occurred for a mortality rate of 0.49 per cent. Systemic complications o…

Injury severity: role of alcohol, substance use and risk-taking.

Secondary brain injury may occur early after severe traumatic brain injury due to hypoxia and/or hypotension. Prehospital care by ambulance paramedics has the goal of preventing and treating these complications and, thus, improving outcomes. In Australia, most ambulance services recommend paramedics attempt endotracheal intubation in patients with severe head injury. Even though most patients with severe head injury retain airway reflexes, most states do not allow the use of appropriate drugs to facilitate intubation. In contrast, recent evidence from trauma registries suggests that this appro

Ipsilateral femoral neck and shaft fractures. Report of two cases using an alternate technique.

A technique for ipsilateral femoral neck and shaft fracture using the sliding compression hip screw with plate combined with trochanteric antegrade Ender nailing of the femur was applied in two cases. Ender nails can be passed without difficulty past a compression hip screw and the bicortical plating screws. The hip and femur can be fixed internally through a single approach in a single position. Sliding compression hip screw devices can provide excellent preliminary stable femoral neck fixation. Blood supply to the femoral head is not disturbed while the femoral intramedullary fixation is per

Effect of a multimodal bundled intervention on quality of recovery after laparoscopic colorectal cancer surgery: a single-center, single-blind, pragmatic randomized controlled trial.

BACKGROUND: Despite the minimally invasive nature of laparoscopic colorectal surgery, postoperative recovery is frequently compromised by pain, fatigue, and delayed gastrointestinal function. Current rehabilitation strategies largely rely on unimodal approaches, and evidence for structured multimodal bundled interventions is limited. This study aimed to evaluate whether a multimodal bundled intervention comprising electroacupuncture, abdominal massage, structured breathing training, and symptom-titrated supervised ambulation improves quality of recovery in patients undergoing laparoscopic surg

Effect of liposomal bupivacaine thoracic paravertebral block on postoperative analgesia in patients undergoing video-assisted thoracoscopic surgery: a randomized controlled trial.

BACKGROUND: Thoracic paravertebral block (TPVB) can provide effective analgesia for patients undergoing video-assisted thoracoscopic surgery (VATS). However, the duration of analgesia achieved with conventional local anesthetics combined with dexamethasone remains limited. This study aimed to determine if liposomal bupivacaine (LB) is superior to bupivacaine combined with dexamethasone (BD) with respect to duration of postoperative analgesia in VATS patients. METHODS: Adults scheduled for VATS were randomized to treatment with LB or BD. The primary outcome was duration of analgesia. Secondary

A Comparative Study between Local Infiltration of Ropivacaine Cocktail and Post-operative Epidural Bupivacaine in Post-operative Analgesia in Primary Total Knee Arthroplasty.

INTRODUCTION: Post-operative pain following total knee arthroplasty (TKA) poses a significant challenge, often limiting early mobilization and delaying rehabilitation. While epidural analgesia with bupivacaine has been widely used, it is associated with complications such as motor block and hypotension, which may impair functional recovery. Local infiltration analgesia (LIA) using ropivacaine-based cocktails has emerged as a potential alternative with a better safety profile and effective pain control. MATERIALS AND METHODS: This prospective, randomized comparative study was conducted on 50 p
