OBJECTIVE: To assess functional outcomes and predictors of success in floating elbow injuries.
DESIGN: Retrospective clinical review.
SETTING: Level 1 trauma center.
PATIENTS: Eighteen patients with floating elbow injuries seen at the trauma center from 1995-2001.
INTERVENTION: All injuries were managed surgically. Each forearm fracture was managed with open reduction and internal fixation. Humerus fractures were managed with either open reduction and internal fixation or intramedullary nail. Definitive fixation was performed in all cases within 48 hours of arrival at the trauma center.

We reviewed the results of 10 patients (10 elbows) who underwent surgical reconstruction for clinically symptomatic posterolateral rotatory instability of the elbow. The symptoms resulted from previous dislocation or a hyperextension or varus stress injury of the elbow. Two elbows had underlying preexisting varus deformity. Surgical reconstruction was performed with a tendon graft in 6 elbows and reattachment of the lateral ulnar collateral ligament to the humerus in 4. Postoperatively, no patient had residual instability or a positive pivot shift test in the elbow. Results were graded as exce…

Most surgeons agree that total elbow arthroplasty is best avoided or deferred in patients younger than 60 years of age, although, at times, there may be no reasonable alternative. Other reconstructive options are available that may provide years of useful function, even if they are sometimes considered a surgical bridge to allow the patient to achieve the age at which implant arthroplasty would be appropriate. The patient and surgeon must have a clear understanding of the expectations and limitations of any proposed procedure. The key to success in reconstructive elbow surgery is the choice of…

Ultrasound examination of the elbow and hand is invaluable in diagnostics of acute and chronic joint diseases. Ultrasound is a noninvasive, immediately available procedure without radiation, which should be performed after exploration and clinical examination. It cannot, however, replace radiographic examination because the cortical substance represents a barrier for the ultrasound waves so that bony lesions (i.e., intraosseous tumors or osteomyelitis) cannot be detected. In the diagnosis of periarticular soft tissue lesions and intra-articular changes (loose bodies, intra-articular effusion, …

Background. This article describes a case of chondromatosis of the elbow in a 33-year-old male patient. An arthroscopic method was used to remove multiple loose bodies from the joint.
Case history. The patient complained of pain in the right elbow. He presented with edema in the elbow joint, pain during movement, occasional slippage, and a 300 functional reduction in elbow extension.
Procedure. After the arthroscope was introduced into the joint, numerous loose bodies were detected, covered with cartilage, pale pink synovial membrane with hyperthropied villi. The cartilage of the j…

We reviewed results of elbow arthroscopy for neglected osteochondritis dissecans (OCD) of the capitellum. Of more than 140 patients having elbow arthroscopy at our institution, 10 patients (11 elbows) had OCD of the capitellum and fulfilled inclusion criteria. Criteria included symptoms of at least 2 years' duration, loss of motion for at least 6 months, or radiographic evidence of secondary degenerative changes. One patient was lost to follow-up. The remaining 9 (10 elbows) were male patients with a median age of 20 years (range, 15 to 58 years). Follow-up averaged 4.6 years (range, 2 to 8 ye…

AIM: To assess whether ultrasound with intraarticular injection of saline increases the detection of intraarticular bodies of the elbow.
SUBJECTS AND METHODS: Twelve patients (age range 21-42 years, mean 29 years; 8 male, 4 female) referred over a 2-year period with an equivocal history of elbow locking and plain radiographs had a joint injection of at least 12 ml of sterile saline during ultrasound examination. No patient had a joint effusion before injection. Surgical correlation was available in six patients.
RESULTS: Ten patients had intraarticular bodies but these were demonstrated only…

BACKGROUND: Neuropathic arthropathy of the elbow is rare and characterized by a painless but unstable articulation. The functional capacity of patients with this condition has not been reviewed in detail.
METHODS: Five male patients, with an average age of fifty-one years, were treated for neuropathic arthropathy of the elbow. The underlying conditions associated with the arthropathy included syringomyelia, insulin-dependent diabetes mellitus, end-stage renal failure, and two cases of polyneuropathy of unknown cause. Four patients sought medical attention after a specific traumatic event. Per…

BACKGROUND: Few studies have examined the outcome of arterio-venous fistulas (AVF) at the elbow for haemodialysis.
METHODS: All AVFs at the elbow created between 1994 and 1998 were identified retrospectively from case notes and the computerised database. Patients were followed until fistula failure, death, or for a minimum of 6 months until June 1999. Life table analysis and Kaplan-Meier actuarial curves with the log rank test were used to assess the influence of age, sex, diabetes, hypertension, heart disease, cerebrovascular disease, use of erythropoietin, aspirin or warfarin, previous inse…

Compressive neuropathy of the ulnar nerve in the upper limb is a common problem that frequently results in severe disabilities. At the elbow, Lundborg concluded that the nerve was "asking for trouble" because of its anatomic course through confined spaces and posterior to the axis of elbow flexion. Normally, the ulnar nerve is subjected to stretch and compression forces that are moderated by its ability to glide in its anatomic path around the elbow. When normal excursion is restricted, irritation ensues. This results in a cycle of perineural scarring, further loss of excursion, and progressiv…

Osteochondritis dissecans (OCD) is an inflammation of the bone and cartilage that usually affects adolescents and young adults. A 16-year-old baseball player who had chronic elbow pain illustrates the typical course of OCD of the elbow. Radiographs may be diagnostic, but bone scan is a more sensitive diagnostic tool, and magnetic resonance imaging offers information for staging and characterization of lesions. If symptoms do not resolve with rest, surgery is recommended, including loose-body removal with curettage or drilling. The prognosis is good with early diagnosis and treatment. Left untr…

In the treatment of osteochondritis dissecans involving the elbow, we have used a bone-peg graft taken from the proximal part of the ulna and inserted into the defect. Thirty-two patients were followed from 2 to 10.5 years. The graft was utilised in 20 elbows, and 6 of these also had concomitant removal of a loose body. Another 6 elbows had removal of a loose body only. Ten elbows were treated conservatively in 5 of these the outcome was unsatisfactory, including 4 in which a bone-peg graft was later necessary. The bone-peg graft gave the best short-term results. Bony union of the dissecans si…

The authors report a case of recurrent dislocation of the elbow in a boy aged 11 years, surgically treated by reconstruction of the lateral capsuloligamentous structures. After a review of the literature, the need for a diagnostic protocol to establish the main cause of recurrent dislocation is emphasized.

The purpose of the study was to determine the changes that might be detected using magnetic resonance imaging (MRI) on patients with chronic lateral epicondylitis of the elbow and to judge its value concerning the clinical treatment. Thirty-four patients with chronic lateral epicondylitis were included in a prospective study. All individuals underwent MRI of the elbow on a 0.2-T dedicated system. The MRI findings were interpreted by an independent radiologist without knowledge of the clinical findings. In 6 surgical cases an additional histological study was done. The biopsy of the extensor te…

Radiological diagnostics (diagnostic imaging) of the traumatised shoulder and elbow joint has expanded considerably and has become increasingly meaningful in recent years. The introduction of computed tomography and magnetic resonance imaging has enabled a significantly better assessment of soft parts and of the inner spatial structure of the joints. Hence, focus of diagnosis has now shifted from the assessment of the bones (which was previously the main aspect of diagnosis) to a different analysis of the joint as a functional unit of bone and soft part structures, an analysis that can also de…

We report a case of a 17-year-old female pitcher with bilateral elbow osteochondritis dissecans. Osteochondritis of the elbow is a well-known disorder affecting pitchers and other individuals who sustain repetitive microtrauma to the elbow. Elbow osteochondritis has been described infrequently in female athletes. The incidence and reporting patterns of this disease are likely to increase as more female athletes participate in organized sports.

Hairy elbows syndrome was first described by Beighton in 1970. An association with short stature has been suggested. A 5-year-old girl with the hairy elbows syndrome who is otherwise healthy is described. Her height is on the 25th centile and appropriate for her family, in which no other members are affected. The mode of inheritance of this disorder remains to be determined.
