Comparing approaches to expose type C fractures of the distal humerus for ORIF in elderly patients: six years clinical experience with both the triceps-sparing approach and olecranon osteotomy.

BACKGROUND: Although open reduction and internal fixation (ORIF) is a standard fracture treatment method, the optimal way to expose a fracture prior to ORIF is debated. We compared the effects of two exposure methods, the triceps-sparing approach and olecranon osteotomy, on the functional outcomes of ORIF-treated type C distal humerus fractures in elderly people. METHODS: From January 2006 to January 2011, 75 elderly patients with type C distal humerus fractures were treated with ORIF, and we retrospectively reviewed their medical records, radiographs, and follow-up charts to identify any com

Treatment of Unstable Elbow Injuries with a Hinged Elbow Fixator: Subjective and Objective Results.

INTRODUCTION: Injuries around the elbow pose a challenging problem for orthopaedic surgeons. The complex bony architecture of the joint should be restored and the thin soft tissue envelope needs to be handled with meticulous care. Elbow instability is a complication seen after dislocations and fractures of the elbow and remains a treatment challenge. The purpose of this study was to provide subjective and objective results following the surgical treatment of unstable elbow dislocations with an external hinged fixation technique. METHODS: Forty-six consecutive patients with complex trauma of t

Percutaneous treatment of radial neck fractures in adult patients.

INTRODUCTION: Radial neck fractures in adults are rare, but outcomes are often poor. Closed reduction and internal fixation (CRIF) technique has been advocated for the treatment of minimally displaced fractures in children, with a few reports on adult subjects. The aim of the present paper is to investigate mid-term results of a CRIF technique in adults with retrograde intramedullary K-wires in Mason's type II and III fractures. The proposed technique yields to good anatomical reduction of displaced neck fractures, faster rehabilitation, and easier hardware removal after fracture consolidation

Triceps-sparing ulnar approach for total elbow arthroplasty.

To prevent insufficiency of the triceps after total elbow arthroplasty, we have, since 2008, used a triceps-sparing ulnar approach. This study evaluates the clinical results and post-operative alignment of the prosthesis using this approach. We reviewed 25 elbows in 23 patients. There were five men and 18 women with a mean age of 69 years (54 to 83). There were 18 elbows with rheumatoid arthritis, six with a fracture or pseudoarthrosis and one elbow with osteoarthritis. Post-operative complications included one intra-operative fracture, one elbow with heterotopic ossification, one transient ul

Olecranon Osteotomy Fixation Following Distal Humerus Open Reduction and Internal Fixation: Clinical Results of Plate and Screws Versus Tension Band Wiring.

Olecranon osteotomy allows for improved visualization of the distal humeral articular surface. This study compared the clinical outcomes of 2 methods of olecranon repair following olecranon osteotomy as part of distal humerus fracture repair. This was a retrospective review of distal humerus fractures treated via a transolecranon approach during a 9-year period. In each case, the olecranon osteotomy was fixed with either tension band wiring (TBW) or plate fixation (PF). Patient demographics, injury information, and surgical management were recorded. Measured outcomes included elbow motion, tim

Unicondylar fractures of the distal humerus: an operative approach.

This paper retrospectively reviews a series of 22 unicondylar fractures of the distal end of the humerus that were treated by open reduction and internal fixation over a 10 year period. The fracture patterns were classified according to the system of Müller et al. A strict rating scale was developed that incorporated subjective data, objective elbow motion, and the functional status of the involved elbow. At an average follow-up of 5.9 years (range 2.3 to 12.3 years), 12 elbows were rated as excellent, 6 as good, and 4 as fair. Complications included extensive posttraumatic arthritis in four p

Management of distal humeral coronal shear fractures.

Coronal shear fractures of the distal humerus are rare, complex fractures that can be technically challenging to manage. They usually result from a low-energy fall and direct compression of the distal humerus by the radial head in a hyper-extended or semi-flexed elbow or from spontaneous reduction of a posterolateral subluxation or dislocation. Due to the small number of soft tissue attachments at this site, almost all of these fractures are displaced. The incidence of distal humeral coronal shear fractures is higher among women because of the higher rate of osteoporosis in women and the diffe

Distal Humerus Fractures in the Elderly Population.

Distal humerus fractures present complex challenges in the elderly patient. These fractures often occur in patients who are living independently but have poor bone quality and low physiologic reserve, thus complicating management decisions and treatment. The goal is a painless, functional, stable elbow that allows completion of the activities of daily living. Nonsurgical management is reserved for those who cannot tolerate surgery. Open reduction and internal fixation is the preferred choice in fractures amenable to rigid fixation and early motion. Although total elbow arthroplasty provides im

[APPLICATION OF COMPRESSION MINI-SCREWS IN TREATMENT OF PATIENTS WITH INJURY OF ELBOW JOINT BONES].

A case report presents the experience of application of compression pileateless mini-screws (Gerbert's screws) in treatment of intra-articular fractures, which formed the elbow joint (44 cases). There were performed 32 operations concerning fracture of head of radius, 10 operations on the occasion of fractures of distal section of the humerus and 2 operations on the coronoid process. Long-term treatment results were followed-up in 31 patients during more than 6 months. On basis of analysis of treatment results the authors made a conclusion that the application of mini-screws in case of bone fr

Ipsilateral Shoulder and Elbow Dislocation.

BACKGROUND: Shoulder dislocations and elbow dislocations are common presentations to the emergency department (ED). Simultaneous ipsilateral elbow and shoulder dislocations are rarely reported and typically occur secondary to trauma. CASE REPORT: A 45-year-old female presented to the ED after a fall from standing and complained of upper right extremity pain. Radiographs revealed posterior dislocation of the right elbow and anterior dislocation of the right shoulder without fractures. Successful reduction of the elbow and shoulder were both achieved, and the patient was placed in a long-arm sp

Acute Anterior Compartment Syndrome of the Thigh without a Fracture Following a Blunt Trauma - A Case Report.

INTRODUCTION: Malunion of intra-articular fracture of the elbow in children is uncommon and may be difficult to treat. Intra-articular corrective osteotomies are still not commonly performed in children, and the main reason is the concern regarding the risk of osteonecrosis. We present a case of extra-articular corrective osteotomy for malunion after open reduction and internal fixation for fracture dislocation of the elbow. CASE REPORT: An 8-year-old boy was injured by a fall and he underwent an operation the day after the injury with diagnosis of lateral condyle fracture of the right humeru

Sporadic Class II Congenital humeroradial synostosis and Left Micromelia in a three-and-a-half-months female Ghanaian infant.

Congenital humeroradial synostosis (CHRS) is a rare musculoskeletal condition that significantly affects the mobility of the elbow joint. They occur in various types and forms depending on the types and numbers of bones involved at the elbow. CHRS may present with elbow deformity and limitation of function. Appropriate timely diagnosis and counseling are required since CHRS is mostly managed conservatively according to literature and may prevent avoidable fractures of the radius from attempts by parents to straighten the flexed fixed elbow and finally offer adequate time for delayed surgical i

Elbow injuries in gymnastics.

The medical literature contains little about elbow injuries in gymnastics. However, 30 females with 32 elbow injuries from gymnastics were found and studied. Two had Panner's disease (osteochondritis of the humeral capitellum), and all the others had fractures or dislocations. The majority required open or closed procedures. Recommendations to help reduce elbow injuries would include the use of experienced spotters, thicker pads whenever possible, and education of gymnasts in techniques of falling. After injury has occurred immobilization of the elbow for as short a period as necessary and fol

Treatment of complex intercondylar humeral fractures in adolescents by open reduction and internal fixation through the transolecranon approach.

This is a review of six cases of complex intercondylar fractures of the humerus in adolescents, treated operatively through a transolecranon approach. Outcome was assessed by means of the Mayo Elbow score, the Disability of the Arm, Shoulder and Hand score, and the Oxford Elbow score. All patients reported 'good' to 'excellent' results. One patient required revision surgery because of fracture nonunion, and all patients underwent removal of hardware from the olecranon after osteotomy union. The transolecranon approach is effective in visualization of complex intercondylar fractures, and good o

[Lateral humeral condyle fractures in children call for follow-up X-ray].

Lateral humeral condyle fractures are among the most common elbow fractures in children (20%), often in children aged six to ten years. X-rays of children this age are hard to interpret, and a seemingly undislocated fracture can still be unstable. Failure to recognise this problem can lead to the wrong treatment, non-union and malfunction of the elbow. An 11-year-old boy fell on his left arm. A plaster cast was administered and worn for four weeks without control X-ray. Four months later the boy had an extension defect of ten degrees, and a control computed tomography confirmed non-union.
