Impact of ClockΔ19 mutation on neural recording performance and neural inflammatory responses.

Chronically implanted neural electrodes for neural recording, stimulation, and chemical sensing are essential tools in studying and treating neurological disorders. However, their performances are limited by implant-induced inflammation, causing a loss in signal quality over time. Because disease states can alter inflammatory processes, it is critical to characterize the tissue response to neural implants within relevant disease models. Circadian rhythm disruption is a hallmark of many psychiatric and neurodegenerative disorders, yet its influence on neural implant performance remains poorly u

Study of electromechanical activity of the stomach in humans and in dogs with particular attention to tachygastria.

The relationship between electric and mechanical activities of the gastric antrum was investigated in both humans and dogs. In 2 patients, in whom platinum monopolar electrodes were implanted on the serosal surface of the antrum and a perfused manometric tube was placed in the stomach, the number of gastric contractions detected by manometry was only less than 50% of the pacesetter potentials accompanied by action potentials or second potentials. No contraction of the stomach was recorded by manometry when pacesetter potentials occurred without action potentials or second potentials. The relat

Self-assessment of cochlear health by cochlear implant recipients.

Recent technological advances in cochlear implant (CI) telemetry have enabled, for the first time, CI users to perform cochlear health (CH) measurements through self-assessment for prolonged periods of time. This is important to better understand the influence of CH on CI outcomes, and to assess the safety and efficacy of future novel treatments for deafness that will be administered as adjunctive therapies to cochlear implantation. We evaluated the feasibility of using a CI to assess CH and examined patterns of electrode impedances, electrically-evoked compound action potentials (eCAPs) and e

Cochlear implantation in the mouse via the round window: effects of array insertion.

Animal models are the only means of assessing the effects of cochlear implantation (CI) at a cellular and molecular level. The range of naturally occurring and genetically-modified mouse strains which mimic human deafness provide excellent opportunities for auditory research. To date, there are very few studies of CI in mice. The main aims of this study were to develop a reproducible and viable technique to enable long term CI in the mouse and to assess the response of the mouse cochlea to implantation as a means of evaluating the success of the procedure. Electrode array implantation via the

Establishing reliable neural health measures in adult cochlear implant users.

Following cochlear implantation, measures of auditory nerve function based on implant-driven electrical stimulation can be used to characterize neural health and support longitudinal monitoring and treatment outcome assessment, with particular relevance for advanced therapies targeting neural hearing loss. This study evaluated the reliability and measurement precision of neural-health measures derived from three complementary electrode-specific tests: panoramic electrically-evoked compound action potentials (PECAPs), interphase-gap (IPG) effects, and polarity effects. Six experienced adult coc

Thalamic stimulation for neuropathic pain.

Eighteen patients with neuropathic pain underwent thalamic electrode implantation. Satisfactory initial pain relief ensued in 14, and their electrode systems were internalized for long-term use. Twelve of the 14 continue to obtain either complete or partial pain relief by regular stimulation. One of the other two patients has had a complete remission of pain, apparently spontaneously, and the other had to have the electrodes removed after it retracted from his thalamus on two occasions. The electrodes have been placed in the sensory nucleus of the thalamus where stimulation evokes paresthesias

Low-Frequency Hearing Preservation With Long Electrode Arrays: Inclusion of Unaided Hearing Threshold Assessment in the Postoperative Test Battery.

Purpose The goal of this work was to evaluate the low-frequency hearing preservation of long electrode array cochlear implant (CI) recipients. Method Twenty-five participants presented with an unaided hearing threshold of ≤ 80 dB HL at 125 Hz pre-operatively in the ear to be implanted. Participants were implanted with a long (31.5-mm) electrode array. The unaided hearing threshold at 125 Hz was compared between the preoperative and postoperative intervals (i.e., initial CI activation, and 1, 3, 6, 9, and 12 months after activation). Results Eight participants maintained an unaided hearing thre

Piezoelectric shaping of radical mastoid cavity and dissection of ossified cochlea prior to cochlear implantation.

Cochlear implantation (CI) in radical cavities remains challenging due to the risk of postoperative electrode extrusion. We report a single-stage approach employing piezoelectric instrument (PEI) to optimize cavity osteoplasty, atraumatic cochlear access, and electrode stabilization in the setting of partial scala tympani ossification. A 72-year-old female with left profound sensorineural hearing loss and prior canal wall (CW)-down mastoidectomy underwent CI with posterior CW reconstruction. Angled and straight PEI inserts facilitated controlled bone removal over the facial nerve and creation

Single-fiber versus macroscale electrodes: enzyme loading and impacts on bioelectronic applications in flexible biodevices.

The integration of enzymes into miniaturised carbon electrodes is a central challenge in advancing flexible and implantable bioelectronic devices. Here, we investigate how progressive reduction of electrode dimensions, from macro-scale flexible electrodes to single microfiber configurations, affects catalytic performance in the ethanol bioelectrooxidation. Using alcohol dehydrogenase (ADH) as a model enzyme, we show that multi-fiber electrodes maintain high catalytic activity even after substantial size reduction, whereas single-fiber electrodes exhibit a marked decrease in current density and

Centromedian nucleus targeting in the pediatric population treated with thalamic responsive neurostimulation for drug-resistant epilepsy.

The management of drug-resistant epilepsy (DRE) in the pediatric population using neurostimulation of the centromedian (CM) nucleus of the thalamus (CMN) has been reported to be effective and safe. We present a case series of pediatric patients treated with responsive neurostimulation (RNS) and report on contact localization in relation to preliminary outcomes, specifically seizure reduction rates. Thirteen pediatric patients treated with RNS underwent direct targeting of the CMN based on Magnetization-Prepared 2 Rapid Gradient-Echo (MP2RAGE) scans, using ClearPoint neuronavigation. The implan

Long-term decoding of movement force and direction with a wireless myoelectric implant.

OBJECTIVE: The ease of use and number of degrees of freedom of current myoelectric hand prostheses is limited by the information content and reliability of the surface electromyography (sEMG) signals used to control them. For example, cross-talk limits the capacity to pick up signals from small or deep muscles, such as the forearm muscles for distal arm amputations, or sites of targeted muscle reinnervation (TMR) for proximal amputations. Here we test if signals recorded from the fully implanted, induction-powered wireless Myoplant system allow long-term decoding of continuous as well as discr

Daily spinal cord stimulation suppresses autotomy behavior in rats following peripheral deafferentation.

Autotomy in experimental animals following peripheral nerve section has been interpreted as a sign of pain corresponding to the chronic pain observed in patients with extensive nerve lesions. Such pain may be alleviated by spinal cord stimulation. In the present study, the effect of such stimulation, via chronically implanted electrodes, on autotomy behavior following sciatic nerve section was assessed in the rat. The stimulation was applied for 30 min daily during a 10-day period. There were four groups of animals, 16 in each, half of them females. Stimulating electrodes were implanted in all

Patient-interactive, computer-controlled neurological stimulation system: clinical efficacy in spinal cord stimulator adjustment.

Over the past 20 years, continuing technical advances have rendered spinal cord stimulation an easily implemented low-morbidity technique for the management of chronic intractable pain in properly selected patients. Percutaneous methods for the insertion of arrays of multiple epidural electrodes, which are driven by noninvasively programmable "multichannel" implanted devices, have been among the most important of these technical improvements. The same implanted electronics may be used with peripheral nerve or intracerebral electrodes. If the capabilities of this new hardware are to be used to

NeuroRoots, a bio-inspired, seamless brain machine interface for long-term recording in delicate brain regions.

Scalable electronic brain implants with long-term stability and low biological perturbation are crucial technologies for high-quality brain-machine interfaces that can seamlessly access delicate and hard-to-reach regions of the brain. Here, we created "NeuroRoots," a biomimetic multi-channel implant with similar dimensions (7 μm wide and 1.5 μm thick), mechanical compliance, and spatial distribution as axons in the brain. Unlike planar shank implants, these devices consist of a number of individual electrode "roots," each tendril independent from the other. A simple microscale delivery approac

The multichannel auditory brain stem implant: performance in twenty patients.

The auditory brain stem implant has been used effectively to provide hearing sensations to individuals deafened by bilateral auditory nerve tumors (neurofibromatosis type 2). During tumor removal, the auditory brain stem implant is implanted into the lateral recess of the fourth ventricle by a translabyrinthine approach and is intended to stimulate auditory neurons of the cochlear nucleus complex. A new eight-electrode multichannel auditory brain stem implant was developed and evaluated in 20 patients who had at least 3 months' experience with the device. Mild nonauditory sensations (primarily

Safety of postimplantation MRI with Dixi microdeep electrodes in situ: An in vitro evaluation of MRI-related heating at 1.5T.

OBJECTIVE: Postimplantation assessment of the position of depth EEG electrodes for intracerebral recordings in patients with refractory focal epilepsy can be performed with MRI or with CT after coregistration to a preimplantation MRI. While both methods offer risks and advantages, postimplantation MRI risks depend on the electrode heating profile under different MRI conditions. We aimed to assess the MRI-related heating of Dixi microdeep electrodes at 1.5T in multiple electrode configurations and with varying levels of radiofrequency (RF) power. METHODS: In vitro tests of heating due to RF po

Model-Based Image Updating for Brain Shift in Deep Brain Stimulation Electrode Placement Surgery.

OBJECTIVE: The efficacy of deep brain stimulation (DBS) depends on accurate placement of electrodes. Although stereotactic frames enable co-registration of image-based surgical planning and the operative field, the accuracy of electrode placement can be degraded by intra-operative brain shift. In this study, we adapted a biomechanical model to estimate whole brain displacements from which we deformed preoperative CT (preCT) to generate an updated CT (uCT) that compensates for brain shift. METHODS: We drove the deformation model using displacement data derived from deformation in the frontal c

An ASIC for Recording and Stimulation in Stacked Microchannel Neural Interfaces.

This paper presents an active microchannel neural interface (MNI) using seven stacked application specific integrated circuits (ASICs). The approach provides a solution to the present problem of interconnect density in three-dimensional (3-D) MNIs. The 4 mm2 ASIC is implemented in 0.35 μm high-voltage CMOS technology. Each ASIC is the base for seven microchannels each with three electrodes in a pseudo-tripolar arrangement. Multiplexing allows stimulating or recording from any one of 49 channels, across seven ASICs. Connections to the ASICs are made with a five-line parallel bus. Current contro
