Morphological correlation between caloric tests and vestibular hydrops in Ménière's disease using intravenous Gd enhanced inner ear MRI.

The purpose of this study was to prove the hypothesis that caloric response in Ménière's disease (MD) is reduced by hydropic expansion of th … I. These findings concluded that the abnormal caloric tests with normal vHIT in MD indicated severe endolymphatic hydrops rather than vestibular hypofunction.

During the period July 1980 to February 1986, 142 patients with endolymphatic hydrops were followed postoperatively according to the AAO-HNS (1985) guidelines. Of these 142 cases, 122 patients were operated on by Kitahara & Futaki's procedure and 20 by the vein graft drainage procedure. The incidence of vertigo was cured in over 90% …

Outcomes of cVEMP and oVEMP in Individuals with Tinnitus and Normal Hearing.

PURPOSE: The purpose of the present study is to find the functions of vestibular reflexes in individuals' with normal hearing and tinnitus, … innitus may occur in ears with normal VEMP. Tinnitus may be the first signs of secondary or delayed endolymphatic hydrops. If this is demonstrated to be accurate, we should anticipate that such patients will gradually develop other endolymphatic hydrops symptoms like SNHL and clinical vestibular dysfunction.

The Meniere attack: an ischemia/reperfusion disorder of inner ear sensory tissues.

We believe Meniere attacks arise as a chance association of endolymphatic hydrops and vascular risk factors for intracerebral ischemia. Hydrops acts as a variable Starling resistor upon the inner ear vasculature that is capable of inducing ischemic attacks only in people with reduced perfusion pressure in the ear. The unique characteristics of the attacks (loss of vestibular response and hearing acutely followed by a return to apparent normalcy over hours) are explained by the differential sensitivity of the inner ear tissues to transient ischemia, with the sensory tissues (dendrites, hair cel…

Physiologic effects on the vestibular system in Meniere's disease.

Ménière syndrome is an inner ear disorder characterized by spontaneous attacks of vertigo, fluctuating low-frequency sensorineural hearing loss, aural fullness and tinnitus. When the syndrome is idiopathic and cannot be attributed to any other cause (eg, syphilis, immune-mediated inner ear disease, surgical trauma), it is referred to as Ménière disease. This article reviews the physiologic effects of Ménière disease on vestibular function, as measured by caloric, head impulse, and vestibular-evoked myogenic potential testing.

Case Report: Ménière's Disease-Like Symptoms in 22q11.2 Deletion Syndrome.

The 22q11.2 deletion syndrome (22q11.2DS), caused by a microdeletion on the long arm of chromosome 22, is characterized by congenital heart … bilaterally malformed lateral semicircular canals, and delayed 3D-FLAIR sequences revealed cochlear endolymphatic hydrops with dilation of the scala media in the left ear. This case shows that acute vertigo with SNHL can be one of the audiovestibular presentations in 22q11.2DS caused by disturbance of endolymphatic flow.

Control of Disabling Vertigo in Ménière's Disease Following Cochlear Implantation without Labyrinthectomy.

BACKGROUND: The placement of a cochlear implant (CI) can restore auditory function in the case of profound cochlear deafness, which may be due to Ménière's disease (MD) or be associated with symptoms related to endolymphatic hydrops. The usual treatment of disabling vertigo in MD is based on vestibular deafferentation by labyrinth ablation. The aim of the present study was to retrospectively evaluate the efficacy of the CI in the control of disabling vestibular manifestations in the case of MD unresponsive to medical treatments. METHODS: A case series of five MD patients with disabling vestib…

Diagnosis and management of drop attacks of vestibular origin: Tumarkin's otolithic crisis.

Erroneous signals of vestibular origin can cause sudden falls without warning. Drop attacks of vestibular origin in our experience most commonly occurred in patients with late or end-stage endolymphatic hydrops (usually idiopathic, ie, Meniere's disease). Eponymically termed Tumarkin's otolithic crisis, drop attacks of vestibular origin must be distinguished from drop attacks due to cardiovascular abnormalities, seizure disorders, vertebral basilar arterial insufficiency (transitory brain stem ischemia), multiple sclerosis, and drug-induced motor control disturbances. The treatment of choice f…

[Changes in otoacoustic emission during the glycerol test in the ears of patients with Meniere's disease].

Monitoring of cochlear function is one proposed clinical application of otoacoustic emission (OAE). To determine the clinical utility of OAE in monitoring, we studied changes in OAE during the glycerol test used to diagnose endolymphatic hydrops. Transiently evoked OAE (TEOAE) and distortion product OAE (DPOAE) were measured before and 3 hours after oral administration of glycerol in 22 ears of patients with Meniere's disease. The positive result in the glycerol test was observed in 11 of 22 ears. TEOAE was recorded using a nonlinear click stimulus, and total echo power (TEP) of the whole resp…

Experimental Animal Models for Meniere's Disease: A Mini-Review.

Several novel animal models that represent the pathophysiological process of endolymphatic hydrops (ELH) of Meniere's disease (MD) have been developed. Animal models are important to identify and characterize the pathophysiology of ELH and to corroborate molecular and genetic findings in humans. This review of the current animal models will be useful in understanding the pathophysiology of and developing proper treatments for MD. Surgical animal models will be replaced by medication-induced animal models. Study models previously developed in guinea pigs will be developed in several smaller ani…

Validity and Reliability of the Diagnostic Tests for Ménière's Disease.

Ménière's disease is defined as an idiopathic syndrome characterized by endolymphatic hydrops. Various tests and measurement methods have been employed for the diagnosis of Ménière's disease. These include audiological, vestibular, radiological, clinical, and biochemical tests. However, the lack of a definitive or gold standard diagnostic test sometimes complicates the process of diagnosis. Hence, the clinician should be well-experienced in deciding when to perform a test and how to interpret the results of the test. Furthermore, having the knowledge of the validity and reliability of these te…

[Endolymph homeostasis and Menière's disease: fundamentals, pathological changes, aminoglycosides].

Although low dose intratympanal gentamicin has empirically been very effective in treating Menière's disease, the mechanisms of elimination or amelioration of vertigo are still insufficiently understood. Most animal studies investigating the effect of aminoglycosides used high doses that damage or kill hair cells and many other cell types of the inner ear. Additional studies are needed to investigate the effects of low dose gentamicin to elucidate the mechanisms affecting vertigo. In this article it will be explained how disturbances of endolymph homeostasis lead to endolymphatic hydrops and f…

Autoregulation of cochlear blood flow.

We reviewed papers written about autoregulation of cochlear blood flow (CBF). In animals, autoregulation of CBF has been recognized not only by variations of systemic blood pressure (BP) but also by manipulations to the blood vessels, such as occlusion of the anterior inferior cerebellar artery, occlusion of the draining veins from the cochlea, and variations of the inner ear pressure. Recent reports have shown that autoregulation of CBF is not so different from that of cerebral blood flow in its strength, but it is disturbed in endolymphatic hydrops or aged animals. Since it became possible t…
