Pre- and postoperative verbal memory and executive functioning in frontal versus temporal lobe epilepsy.

There is accumulating evidence for considerable overlap in preoperatively affected cognitive functions in patients with temporal lobe epilepsy (TLE) and frontal lobe epilepsy (FLE). The current study investigated whether it is possible to differentiate between patients with FLE and TLE prior to surgery, based on measures of verbal memory and executive functioning. Furthermore, the postoperative cognitive development was compared. Pre- and postoperative data from 109 patients with FLE and 194 patients with TLE were retrospectively analyzed. Preoperatively, there were no differences in verbal me

A Case of Cognitive Decline Resulting from Aging, Temporal Lobe Epilepsy, and Environmental Factors.

Cognitive functioning is imperative in our daily lives. It allows us to understand, process, and react appropriately to different situations. Aging has been linked to cognitive decline. The degree and rate of cognitive decline are crucial as they differentiate normal aging from dementia or memory loss secondary to medical conditions. A 63-year-old Caucasian woman with a 50-year history of temporal lobe epilepsy experienced memory difficulties in recent years. She was admitted voluntarily to the neuropsychiatry ward for a 3-day ambulatory electroencephalogram (EEG), which reported mild bitempor

Resting-state brain entropy in right temporal lobe epilepsy and its relationship with alertness.

BACKGROUND: To date, no functional MRI (fMRI) studies have focused on brain entropy in right temporal lobe epilepsy (rTLE) patients. Here, we characterized brain entropy (BEN) alterations in patients with rTLE using resting-state functional MRI(rs-fMRI) and explored the relationship between BEN and alertness. METHOD: Thirty-one rTLE patients and 33 controls underwent MRI scanning to investigate differences in BEN and resting-state functional connectivity (rs-FC) in regions of interest (ROIs) between patients and controls. Correlation analyses were performed to examine relationships between th

Decision-making impairments following insular and medial temporal lobe resection for drug-resistant epilepsy.

Besides the prefrontal cortex, the insula and medial structures of the temporal lobe are thought to be involved in risky decision-making. However, their respective contributions to decision processes remain unclear due to the lack of studies involving patients with isolated insular damage. We assessed adult patients who underwent resection of the insula (n = 13) or of the anterior temporal lobe (including medial structures) (n = 13) as part of their epilepsy surgery, and a group of healthy volunteers (n = 20), on the Iowa Gambling Task (IGT) and on the Cups Task. Groups were matched on sociode

Intracranial papillary endothelial hyperplasia (Masson's tumour) following gamma knife radiosurgery for temporal lobe epilepsy.

We present a rare case of intracranial papillary endothelial hyperplasia, or 'Masson's tumour,' following gamma knife radiosurgery for epilepsy. A 59-year-old woman presented with a 4-month history of escalating headaches and progressive neurological deficit. MR scan of brain showed enlargement of an enhancing right temporal lobe lesion, midline shift and obstructive hydrocephalus. She had previously undergone non-curative gamma knife radiosurgery at the age of 44 years for medically refractory complex partial seizures. Postprocedure imaging had shown signal change and enhancement within the r

Evaluating executive function in patients with temporal lobe epilepsy using the frontal assessment battery.

Previous studies have demonstrated executive dysfunction in patients with temporal lobe epilepsy (TLE). Frontal assessment battery (FAB) is a short neuropsychological tool that was developed for assessment of frontal lobe function in a clinical setting. The aim of the present study is to evaluate the clinical utility of FAB for detection of executive dysfunction in TLE patients. Forty-eight TLE patients and 48 sex and age-matched healthy controls participated in this study. Compared to healthy participants, the total FAB score was significantly lower among the TLE patients. TLE patients perfor

Antithyroid antibodies as a potential marker of autoimmune-mediated late onset temporal lobe epilepsy.

UNLABELLED: In one third of patients with epilepsy starting in adulthood the aetiology remains undetermined. Some patients with late onset temporal lobe epilepsy (TLE) together with memory decline have elevated antithyroid antibodies. PURPOSE: To compare the prevalence of antithyroid antibodies (aTR-ab) in adult onset temporal lobe epilepsy (TLE) patients with known and unknown TLE aetiology (N=42). Moreover, the sera of these patients was also assayed for antinuclear antibodies (ANA), anticardiolipin antibodies (ACL), anti glutamic acid decarboxylase antibodies (GAD) and antiglidadin antibod

Abnormal interictal gamma activity may manifest a seizure onset zone in temporal lobe epilepsy.

Even though recent studies have suggested that seizures do not occur suddenly and that before a seizure there is a period with an increased probability of seizure occurrence, neurophysiological mechanisms of interictal and pre-seizure states are unknown. The ability of mathematical methods to provide much more sensitive tools for the detection of subtle changes in the electrical activity of the brain gives promise that electrophysiological markers of enhanced seizure susceptibility can be found even during interictal periods when EEG of epilepsy patients often looks 'normal'. Previously, we de

Prediction of visual field deficits by diffusion tensor imaging in temporal lobe epilepsy surgery.

Visual field deficits due to optic radiation injury are a common complication of temporal lobectomy in epilepsy surgery. In this prospective study, diffusion tensor imaging (DTI) based fiber tracking was performed on 48 patients who had temporal lobectomy for pharmaco-resistant epilepsy. Pre- and intra-operative DTI based fiber tracking was used to visualize the optic radiation and to predict the post-operative visual field defects. The course of the optic radiation could be successfully reconstructed by DTI based fiber tracking. There was a significant correlation between the fiber tracking e

Intractable temporal lobe epilepsy with rare spikes is less severe than with frequent spikes.

OBJECTIVE: To analyze clinical, electrophysiologic, and neuroradiologic characteristics of a group of patients with nonlesional intractable temporal lobe epilepsy (TLE) and rare or absent interictal epileptiform abnormalities (IEA). METHODS: Between 1990 and 2000, 31 patients (11 men; mean +/- SD age 34.3 +/- 11.7 years) with nonlesional intractable TLE were consecutively selected on the basis of the absence or paucity of IEA (<1/h) on serial scalp EEG recording; these were defined as "oligospikers." The clinical and laboratory characteristics of oligospikers were compared with those of a gro

[MRI in patients with temporal lobe epilepsy: correlation between MRI findings and clinical features].

There have been many X-ray computed tomography (CT) studies on epilepsy in the literature. However, mesial temporal sclerosis, known to form an important pathology of temporal lobe epilepsy, has not been visualized by CT, while magnetic resonance imaging (MRI) has enabled it to be depicted to some extent. Various views exist as to the MRI visualization of mesial temporal sclerosis. Furthermore, only few papers have adequately dealt with clinical features in relation to MRI findings. This study attempted to investigate MRI findings on temporal lobe epilepsy, and then correlate them with clinica

The potential therapeutic role of berberine in treating epilepsy focusing on temporal lobe epilepsy: State of art and ongoing perspective.

Epilepsy is a neurological disease characterized by unprovoked recurrent epileptic seizures. Temporal lobe epilepsy (TLE) is the commonest type of focal epilepsy in adults that resist to the conventional anti-seizure medications (ASMs). Interestingly, ASMs do not affect the epileptogenesis and progression of disease. Therefore, repurposing of natural products with anti-inflammatory, anti-oxidant and anti-seizure effects such as berberine (BRB) may be logical in treating refractory epilepsy and TLE. However, the molecular mechanism of BRB against the development of epilepsy and progression of e

Evaluating the Association of Calcified Neurocysticercosis and Mesial Temporal Lobe Epilepsy With Hippocampal Sclerosis in a Large Cohort of Patients With Epilepsy.

BACKGROUND: Neurocysticercosis (NCC) is a parasitic infection of the central nervous system that has been associated with mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS). However, this association has not been completely established. OBJECTIVE: To evaluate the prevalence of calcified NCC (cNCC), its characteristics and a possible association between cNCC and MTLE-HS in a cohort of 731 patients with epilepsy. METHODS: We review clinical, EEG and neuroimaging findings of 731 patients with epilepsy. From these, 659 had CT-scans and 441 patients had complete neuroimaging with

High seizure frequency prior to antiepileptic treatment is a predictor of pharmacoresistant epilepsy in a rat model of temporal lobe epilepsy.

PURPOSE: Progress in the management of patients with medically intractable epilepsy is impeded because we do not fully understand why pharmacoresistance happens and how it can be predicted. The presence of multiple seizures prior to medical treatment has been suggested as a potential predictor of poor outcome. In the present study, we used an animal model of temporal lobe epilepsy to investigate whether pharmacoresistant rats differ in seizure frequency from pharmacoresponsive animals. METHODS: Epilepsy with spontaneous recurrent seizures (SRS) was induced by status epilepticus. Frequency of

Epilepsy surgery in a liver-transplanted girl with temporal lobe epilepsy and hippocampal sclerosis following PRES with status epilepticus.

INTRODUCTION: Posterior reversible encephalopathy syndrome (PRES) with status epilepticus may occur after liver transplant. This may rarely lead to refractory epilepsy and hippocampal sclerosis (HS). CASE DESCRIPTION: We report the first case of epilepsy surgery in a liver-transplanted patient with refractory temporal lobe epilepsy. A 3-year-old girl underwent liver transplant for congenital biliary atresia. Four days after transplant she manifested PRES with status epilepticus, but she recovered within a couple of weeks. At the age of 5 years she started presenting complex partial seizures,

Comparison of selective amygdalohippocampectomy and anterior temporal lobectomy for mesial temporal lobe epilepsy: a systematic review and meta-analysis.

BACKGROUND: Anterior temporal lobectomy (ATL) and selective amygdalohippocampectomy (SAH) are the two most widely adopted surgical approaches for drug-resistant mesial temporal lobe epilepsy (MTLE). Although both procedures target seizure control, they differ in extent of resection and potential morbidity. The relative effectiveness of ATL and SAH remains debated, particularly regarding seizure freedom, visual field deficits, and postoperative complications. OBJECTIVE: To systematically compare seizure freedom (Engel I) rates, Engel III-IV outcomes, visual field deficits, and postoperative co

Resection of the temporal piriform cortex in mesial temporal lobe epilepsy: Association with seizure freedom in a retrospective cohort study.

OBJECTIVE: This study evaluated the association between temporal piriform cortex (tPC) resection and long-term postoperative outcomes in temporal lobe epilepsy (TLE). METHODS: We conducted a retrospective analysis of patients with TLE undergoing surgery between 2012 and 2022, with a minimum follow-up of 2 years. The extent of resection, including the tPC, hippocampal head/body, and amygdala, was assessed using magnetic resonance imaging scans. These measurements, along with demographic data and other presurgical evaluations, were analyzed for their associations with Engel class postoperative

The decreased connectivity in middle temporal gyrus can be used as a potential neuroimaging biomarker for left temporal lobe epilepsy.

OBJECTIVE: We aimed to explore voxel-mirrored homotopic connectivity (VMHC) abnormalities between the two brain hemispheres in left temporal lobe epilepsy (lTLE) patients and to determine whether these alterations could be leveraged to guide lTLE diagnosis. MATERIALS AND METHODS: Fifty-eight lTLE patients and sixty healthy controls (HCs) matched in age, sex, and education level were recruited to receive resting state functional magnetic resonance imaging (rs-fMRI) scan. Then VHMC analyses of bilateral brain regions were conducted based on the results of these rs-fMRI scans. The resultant imag

Epilepsy Surgery in Extratemporal vs Temporal Lobe Epilepsy: Changes in Surgical Volumes and Seizure Outcome Between 1990 and 2017.

BACKGROUND AND OBJECTIVES: Seizure outcome after extratemporal lobe epilepsy (exTLE) surgery has often been poorer than after temporal lobe epilepsy (TLE) surgery, but recent improvements in diagnostics and surgery may have changed this. Our aim was to analyze the changes in presurgical and surgical volumes and seizure outcome 2 years after surgery for patients with exTLE compared with those with TLE. METHODS: We performed a retrospective, single-center cohort study including patients from the Bethel presurgical-surgical-postsurgical database from 1990 to 2017. We used logistic regression to
