Circulating fibrinogen is a prognostic and predictive biomarker in malignant pleural mesothelioma.

BACKGROUND: To investigate the clinical utility of pretreatment plasma fibrinogen levels in malignant pleural mesothelioma (MPM) patients. METHODS: A retrospective multicenter study was performed in histologically proven MPM patients. All fibrinogen levels were measured at the time of diagnosis and clinical data were retrospectively collected after approval of the corresponding ethics committees. RESULTS: In total, 176 MPM patients (mean age: 63.5 years ± 10.4 years, 38 females and 138 males) were analysed. Most patients (n=154, 87.5%) had elevated (≥ 390 mg dl(-1)) plasma fibrinogen levels.…

PakSurg 1: determining the epidemiology and risk factors of surgical site infections in Pakistan-a multicentre, prospective cohort study.

INTRODUCTION: Surgical site infections (SSIs) are among the the most common postoperative complications, despite being highly preventable. Multiple studies have explored the incidence and risk factors of SSIs globally. However, nationally representative data capable of informing evidence-based guidelines remain limited in Pakistan. Hence, the aim of this study is to identify the incidence and risk factors of developing SSIs following surgery and to explore existing SSI prevention practices in Pakistan. METHODS AND ANALYSIS: This study is a multicentre, prospective cohort study across various

Motor Neuron Disease Systematic Multi-Arm Adaptive Randomised Trial (MND-SMART): a multi-arm, multi-stage, adaptive, platform, phase III randomised, double-blind, placebo-controlled trial of repurposed drugs in motor neuron disease.

INTRODUCTION: Motor neuron disease (MND) is a rapidly fatal neurodegenerative disease. Despite decades of research and clinical trials there remains no cure and only one globally approved drug, riluzole, which prolongs survival by 2-3 months. Recent improved mechanistic understanding of MND heralds a new translational era with many potential targets being identified that are ripe for clinical trials. Motor Neuron Disease Systematic Multi-Arm Adaptive Randomised Trial (MND-SMART) aims to evaluate the efficacy of drugs efficiently and definitively in a multi-arm, multi-stage, adaptive trial. The

Is untargeted iron supplementation harmful when iron deficiency is not the major cause of anaemia? Study protocol for a double-blind, randomised controlled trial among non-pregnant Cambodian women.

INTRODUCTION: The WHO recommends daily oral iron supplementation for 12 weeks in women and adolescents where anaemia prevalence is greater than 40%. However, if iron deficiency is not a major cause of anaemia, then, at best, untargeted iron supplementation is a waste of resources; at worst, it could cause harm. Further, different forms of iron with varying bioavailability may present greater risks of harm. METHODS AND ANALYSIS: A 12-week three-arm, double-blind, randomised controlled supplementation trial was conducted in Cambodia to determine if there is potential harm associated with untarg

Accessible ethics and legal advice for wastewater surveillance: The WWS ethics adviser app.

Wastewater surveillance (WWS) has a long history in infectious disease management, from the detection of Salmonella Paratyphi B in sewers by Moore in 1948, to its role in global poliovirus eradication and more recently the deployment of WWS to track SARS-CoV-2. Whilst current interest has focused on pathogen sequencing to monitor outbreaks, WWS encompasses a much broader molecular landscape that includes a wide array of chemical compounds, proteins and DNA from human, animal, plant and microbial organisms. This diversity creates novel opportunities for public health applications, environmental

The public funding of expensive cancer therapies: synthesizing the "3Es"--evidence, economics, and ethics.

The cost of new cancer therapies and drugs has risen sharply and somewhat alarmingly in the last five years. In those developed countries where healthcare systems are (primarily) publicly funded, this has caused legitimate concern among government administrators and politicians who must make decisions regarding funding. In the Canadian context, provincial Departments of Health are mandated to provide comprehensive healthcare services to all citizens out of a "fixed pot" of financial resources, which is determined annually as part of the provincial government's overall budget process. In recent

Sleep and mental illness: an umbrella review protocol.

INTRODUCTION: Sleep is an important natural process carried out by the human body. It allows the body to rest and the brain to process information attained over the course of the day. Sleep is intrinsically linked to mental health, with those with mental illness more likely to require support in attaining adequate sleep. Recently, the College of Psychiatrists in Ireland's REFOCUS committee has sought to co-design an information leaflet for those with mental illness detailing the latest recommendations from the literature regarding sleep for this population, regardless of whether or not they ta

New World cutaneous leishmaniasis: current challenges in diagnosis and parenteral treatment.

Many physicians in the United States and other nonendemic countries lack familiarity with New World cutaneous leishmaniasis (CL) and fail to include it in their differential diagnosis when seeing patients with suggestive lesions and recent high-risk travel. Moreover, even when the diagnosis of New World CL is considered and confirmed, physicians in the United States still face obstacles in obtaining appropriate treatment. In this report, we present 3 cases of New World CL that were either initially misdiagnosed or faced significant delays in therapy. We also discuss the optimal approach by whi

What does the best interests principle of the convention on the rights of the child mean for paediatric healthcare?

The present review analyses the implications of the best interests of the child principle, which is one of the most widely discussed principles of medical ethics and human rights, for paediatric healthcare. As a starting point, it presents the interpretation of the best interests principle by the United Nations Committee on the Rights of the Child. On this basis, it points out possible fields of application of the best interests principle with regard to paediatric healthcare and discusses the potential difficulties in the application of the best interests principle. Based on this, it illustrat

Protocol for exploring health promoter-led mental wellness initiatives for early prevention, screening and quality of life in patients with cervical cancer of rural Eastern Cape, South Africa: a mixed-methods study.

INTRODUCTION: Cervical cancer is in the top five list of highly prevalent cancers. It contributes to millions of premature deaths globally. Low- and middle-income countries like those in sub-Saharan Africa are among the worst affected despite the preventable nature of the disease. The disease is caused by the human papillomavirus, a sexually transmitted infection for which an effective vaccine exists. This study aims to determine the effectiveness of a health promoter-led intervention on early screening, prevention and quality-of-life improvement among patients with cervical cancer. METHODS A
