Treatment of osteomyelitis of the spine using percutaneous suction/irrigation and percutaneous external spinal fixation.

External skeletal fixation is a well-known tool in the management of infection of long bones. However, the application of external skeletal fixation in the treatment of spinal infection has not been previously reported. We have used percutaneous external spinal fixation (PESF) for the treatment of osteomyelitis of the spine in 23 patients since 1981. The treatment consists of percutaneous vertebral biopsy for bacteriologic diagnosis, installation of a suction/irrigation system into the intervertebral disk space, and posterior stabilization (and reduction if indicated) with an external fixator

Bicondylar tibial plateau fractures treated with a hybrid ring external fixator: a preliminary study.

Twenty-three knees in 22 patients with Schatzker type VI tibial plateau fractures were treated with a hybrid ring external fixator using tensioned wires proximally and half-pins distally. All but two injuries were secondary to high-energy trauma. Six were open injuries, and eight patients had other major musculoskeletal trauma. Eight patients were treated with limited open reduction and internal fixation before application of the frame. The remainder received percutaneous cannulated screw fixation to stabilize the articular surface without opening the fracture site. Twenty-three fractures were

Somatosensory evoked potential evaluation of acute nerve injury associated with external fixation procedures.

The effects of different mechanisms of acute nerve injury on peripheral nerve function during K wire application and 1 stage limb lengthening were evaluated prospectively in 24 goats using somatosensory evoked potentials. Stable somatosensory evoked potential recordings throughout 3-day experiments were obtained in animals with wires placed at a distance from or directly adjacent to a nerve but without producing any tension or pressure. Complete loss of the peroneal nerve somatosensory evoked potentials occurred if this nerve was perforated by wire, underwent excessive pressure by wire, or had

[Joint bridging external fixation of fractures of the distal lower leg with severe skin damage].

During the 36 months from April 1989 until April 1992, 34 fractures of the lower end of the leg were treated by joint bridging application of external fixation using the so-called ".. Kugelspannfixateur UNIFIX". They were: 24 intraarticular compression fractures, seven bimalleolar dislocation fractures, two shot fractures and one postoperative empyema. All demanded treatment urgently. Because of severe skin lesions or because of medical reasons, neither osteosynthesis nur conventional conservative methods could be used. With the aid of UNIFIX, good alignment was achieved in all cases and pract

Fractures of the shaft of the radius after treatment of Colles fractures with pins and plaster or external fixation.

Refracture of the radius through a pin site after the removal of pins and plaster or an external fixator for Colles fracture is rarely mentioned in the literature. We present the details of three such cases and review two others reported in the literature. Our aim is to alert orthopedists to the potential for this complication. Patients with superficial or deep pin tract infections appear to be at greater risk.

External transpedicular fixation test of the lumbar spine correlates with the outcome of subsequent lumbar fusion.

External transpedicular fixation was applied to the lower lumbar spine in a prospective study on 42 patients with chronic low back pain combined with suspected instability of the lumbar segments; the diagnosis was failed disk surgery, spondylolisthesis, and degenerative disk disease. The aim was to realign the involved segments, to restore disk height, and to record changes in pain and performance during the external fixation test. Pain was recorded on a visual analog scale, and performance was assessed using the Oswestry disability score. As independent observer assessed the test and treatmen

Use of an external skeletal fixation device and bone graft for arthrodesis of the metacarpophalangeal joint in horses.

Arthrodesis of the metacarpophalangeal joint was attempted in 9 adult horses, using a new external skeletal fixation device and autogenous cancellous bone graft. Four horses had excellent results, 2 horses had fair results, arthrodesis failed in 2 horses, and 1 horse fractured the third metacarpal bone. The advantages of the technique included minimal surgical trauma to the area of injury, immediate rigid fixation of the metacarpophalangeal joint, and access for treatment of soft-tissue lesions at the site of injury. Although pin loosening and pin-track infection were complications when the de

Unilateral external fixation for severe open tibial fractures. Preliminary report of a prospective study.

In a prospective survey of severe open tibial fractures, the incidence of serious complications with relatively rigid one- or two-plane unilateral fixator frames was considerably lower than with internal fixation by plates and screws. Unilateral configurations that contain rigid components provide better wound access than bilateral frames and interfere less with knee and ankle motion; they allow unencumbered partial and even full weight-bearing. This increases comfort and mobility and may have a positive effect on fracture healing. No standard formula exists, however, for the application of an

Comparison of two-stage open reduction and internal fixation and single-stage external fixation for complex pilon fractures: a randomized controlled trial.

PURPOSE: To compare functional and radiological outcomes between two stage ORIF and single stage external fixation for complex pilon fractures. METHODS: Prospective, single-center randomized controlled trial at a Level I trauma facility (April 2021-April 2023). Sixty skeletally mature patients with AO/OTA 43-C pilon fractures unsuitable for primary ORIF were randomized to two-stage ORIF (control group) or single-stage limited internal fixation with external fixation (LIFEF) (treatment group). Minimum follow-up was 24 months. The primary outcome was the AOFAS score at final follow-up. Secondar

A case report of circular external fixator with low-profile mini-fragment plate fixation: A combination of two methods for a tibial pilon fracture.

Tibial pilon fractures are difficult to treat. These fractures are associated with a high frequency of soft tissue complications. Therefore, two-stage surgery and less invasive surgical strategies using external fixation have been reported. The patient was a 79-year-old man. The right tibial pilon fracture was diagnosed as AO/OTA 43C3.1, Rüedi and Allgöwer type 2. He was treated with a low-profile mini-fragment plate and circular (Ilizarov type) external fixation. Herein, we report on a combination of these two methods: circular external fixator with low-profile mini-fragment plate fixation. A

Management of simple (types A and B) closed tibial shaft fractures using percutaneous lag-screw fixation and Ilizarov external fixation in adults.

PURPOSE: Although intramedullary fixation of closed simple (type A or B) diaphyseal tibial fractures in adults is well tolerated by patients, providing lower morbidity rates and better mobility, it is associated with some complications. This study evaluated the results of managing these fractures using percutaneous minimal internal fixation using one or more lag screws, and Ilizarov external fixation. METHODS: This method was tested to evaluate its efficacy in immediate weight bearing, fracture healing and prevention of any post-immobilisation stiffness of the ankle and knee joints. This rand

Delta frame triplanar external fixation for displaced intra-articular calcaneal fractures: mid- to long-term outcomes and comparative literature review : *A single-stage external fixation-based approach for restoring calcaneal anatomy.

PURPOSE: Displaced intra-articular calcaneal fracture treatment may involve surgical intervention to restore the anatomy of the calcaneus and promote proper healing. Numerous surgical techniques, such as open reduction internal fixation (ORIF) or percutaneous fixation, have been utilized with varying degrees of success in achieving anatomical reduction and functional outcomes. However, complication rates are still high, and there is ongoing debate regarding the optimal surgical approach. This study presents a delta-frame triplanar external fixation technique combining intra- and extra-calcanea

Anatomic study of the axillary nerve in a Chinese cadaveric population: correlation of the course of the nerve with proximal humeral fixation with intramedullary nail or external skeletal fixation.

INTRODUCTION: The axillary nerve can be injured during external skeletal fixation with Schanz screws or proximal locking screws of intramedullary nails. Being aware of the axillary nerve's anatomic relationship to the proximal humerus is vital for avoiding complications. METHODS: We investigated the relationship of the axillary nerve to surrounding bony landmarks by studying 88 axillary nerves in 44 embalmed cadaveric adult Chinese males. These measurements were then compared with the results from a similar study among Caucasians using the same reference points. RESULTS: We identified three
