Descriptive epidemiology of pediatric tennis injuries: insights from a decade of national data.

OBJECTIVES: Tennis is a sport which can result in a range of injuries due to the extensive use of both the upper and lower extremities durin … ere most commonly affected across sex and age subgroups. Children most commonly had lacerations and facial injuries, while adolescents most commonly had strains/sprains and ankle injuries. The distribution of injury types and locations significantly differed between males and females, and children and adolescents (p < 0.01). Fewer than 1% of injuries required hos …

A Nationwide Analysis of Gunshot Wounds of the Head and Neck: Morbidity, Mortality, and Cost.

BACKGROUND: Gun violence in the United States rose continuously from 2010 to 2022, spiking during the pandemic, and peaking in 2021 at 48,830 deaths (14.8 per 100,000). Previous reports investigated health and financial burden associated with gunshot wounds (GSWs) during 2004 to 2013; however estimates related specifically to head and neck (H&N) injuries have been lacking. This population-based study aims to examine incidence, morbidity, mortality, and health resource utilization of H&N injuries utilizing the Nationwide Inpatient Sample database. METHODS: A population-based study was undertak

Fascicular Turnover Flap: An Approach for Facial Nerve Reconstruction.

Facial nerve injuries are a common complication associated with parotidectomy. These functionally debilitating injuries are conventionally treated with nonvascularized nerve grafting; however, this reconstructive modality produces moderate donor site morbidity and has limited efficacy for repairing large defects. In addition, nonvascularized nerve grafts are highly susceptible to radiotherapy and require a well-vascularized wound bed to produce adequate therapeutic results. The fascicular turnover flap, described by Koshima et al, utilizes a single fascicle to bridge two nerve endings that are

Face Autotransplantation After an Accident.

Microsurgical replantation is considered the best method to treat avulsion or amputation injuries of unique head parts, including the scalp, eyelid, nose, ear, lip, and tongue. However, there is currently no report on microsurgical replantation of substi-tutable facial tissues in the literature. This report presents a patient with avulsed facial composite tissue composed of the skin and subcutaneous layers treated by microsurgical replantation. The present case shows that microsurgical replantation of small facial composite tissues is feasible. The authors suggest that the indication spectrum

Ocular protection in facial paralysis.

PURPOSE OF REVIEW: Facial nerve paralysis has a profound impact on patients' quality of life, of which one of the most important sequelae is a risk of corneal surface disease. Herein, we discuss methods to protect the eye following insult to the facial nerve. RECENT FINDINGS: Protection of the ocular surface in patients with facial nerve injury is of paramount importance to prevent corneal injury and potential blindness. Many interventions on the eye are temporary and therefore easily reversible. A systematic approach to managing the eye is required in these patients. SUMMARY: This article p

The Impact of a Horse Kick: A Systematic Review of Horse-Related Maxillofacial Trauma.

Objectives: To systematically review the evidence on horse-related maxillofacial trauma, with emphasis on horse-kick injuries, anatomical patterns, associated injuries, management, outcomes, and prevention. Methods: This systematic review followed the PRISMA 2020 statement. PubMed/MEDLINE, Scopus, Web of Science, and the Cochrane Library were searched for original clinical studies reporting facial, craniofacial, orbital, dental, or maxillofacial injuries related to horse-associated activities. Data were extracted on study characteristics, demographics, mechanism, mounted/unmounted status, anat…

[Cochlear implantation through retro-facial approach with congenital microtia malformation with facial nerve deformity: a case report].

The difficulty of cochlear implantation in patients with congenital microtia is usually increased due to the vague anatomical marks and facial nerve malformation. The common types of facial nerve malformation include facial nerve bony cover loss, aberrant position, and bifurcation malformation. Bifurcation malformation may obscure the oval window, press against stapes, and bifurcate in the vestibular window while obscuring the round window. It is important to correctly identify the facial nerve and choose a reasonable surgical approach to avoid postoperative complications. This article describ

Full-face motorcycle helmet protection from facial impacts: an investigation using THOR dummy impacts and SIMon finite element head model.

OBJECTIVE: Facial impacts are both common and injurious for helmeted motorcyclists who crash; however, there is no facial impact requirement in major motorcycle helmet standards. This study examined the effect of full-face motorcycle helmet protection on brain injury risk in facial impacts using a test device with biofidelic head and neck motion. A preliminary investigation of energy absorbing foam in the helmet chin bar was carried out. METHOD: Flat-faced rigid pendulum impacts were performed on a THOR dummy in an unprotected (no helmet) and protected mode (two full-face helmet conditions).

Patterns and trends in facial fractures in New Zealand between 1999 and 2009.

OBJECTIVES: To describe patterns and trends in facial fractures in New Zealand over a decade. METHOD: A secondary analysis of Ministry of Health data on facial fractures occurring between 30 June 1999 and 1 July 2009. Patient demographic characteristics and the details of injury were analysed and described. RESULTS: Over the study period, the incidence of injury was constant. A total of 26,637 facial fractures were recorded. Most (79%) occurred in males, giving a male-to-female ratio of 4:1. Peak injury rates for both males and females, coincide with the legal alcohol purchasing age. Interpe

Management of cranial nerves I through VII following skull base surgery.

Cranial nerve injuries are common with skull base surgery. While injuries to the seventh and tenth cranial nerves can be corrected to satisfactory degrees, rehabilitation of the third, fourth, and sixth nerves is possible to only a limited degree. This study stresses the management of facial paralysis following skull base surgery and is based upon the author's experiences in dealing with 38 patients who suffered such a facial paralysis. The best results of rehabilitative surgical treatment were achieved with techniques that connect the central stump to the peripheral system. The time between n

Post-traumatic bilateral facial palsy: a case report and literature review.

Bilateral facial paralysis due to basilar skull fracture involving the temporal bone is rare and, unlike unilateral facial palsy, it can be difficult to recognize because of a lack of facial asymmetry. Thorough clinical history and physical exam, high-resolution CT scan and electrodiagnostic tests can help to make the diagnosis of bilateral facial nerve palsy and early detection, evaluation and intervention may be important for optimal functional recovery. A 16-year-old male sustained closed head injury after motor vehicle collision. The initial head CT scan showed bilateral temporal bone frac

Facial paralysis: a critical review of accepted explanation.

Historically, paralysis of facial muscles has been divided into "upper motor neuron injury" and "lower motor neuron injury". Patients who experience a stroke in the cortex or internal capsule have UMN injury and cannot purse their lips or smile on command. They are, however, able to wrinkle their forehead, raise their eyebrows, and completely close their eyes. Patients with LMN injury, in addition to the aforementioned impairments cannot raise their eyebrows. The classical explanations for these clinical findings are that the upper facial muscles receive bilateral innervation from the cerebral

Craniofacial and skull base trauma.

BACKGROUND: Traumatic craniofacial and skull base injuries require a multidisciplinary team approach. Trauma physicians must evaluate carefully, triage properly, and maintain a high index of suspicion to improve survival and enhance functional recovery. Frequently, craniofacial and skull base injuries are overlooked while treating more life-threatening injuries. Unnoticed complex craniofacial and skull base fractures, cerebrospinal fluid fistulae, and cranial nerve injuries can result in blindness, diplopia, deafness, facial paralysis, or meningitis. Early recognition of specific craniofacial

Solitary Frontal Sinus Fractures Compared to Multiple Facial Fractures, Energy Impact Dependency.

Frontal sinus fractures account for 2% to 15% of maxillofacial injuries. Up to 66% to 87% of the patients with frontal sinus fractures experience associated facial fractures. The majority of classifications used today categorize frontal sinus fractures depending on the integrity of the anterior table, posterior table, and the nasofrontal outflow. A retrospective study was performed, which included 24 patients diagnosed with frontal sinus fractures. Treatment in all patients consisted of open reduction and bone fixation. We analyzed population variables, injury etiology, fracture site, associat…
