Necrotizing fasciitis of the head and neck: an analysis of 47 cases.

Necrotizing fasciitis is an overwhelming infection common to the perineum, abdominal wall, and extremities. It is a surgical emergency related to a high mortality rate that is more often seen in elderly and immunocompromised patients. Necrotizing fasciitis occurs uncommonly in the head and neck region. Over a 12-year period, 47 cases of necrotizing fasciitis of the head and neck region were collected at this hospital. The demographics, predisposing factors, clinical presentation and courses, management, complications, and outcomes were analyzed. The cases were divided into two groups: survivor

Anaerobic bacteremia and necrotizing fasciitis in a patient with Crohn's disease.

Crohn's disease is a chronic, granulomatous disease that affects the gut that is frequently treated with immunosuppressive therapy. Infectious complications are common and are usually related to the transmural nature of the inflammation, frequently manifesting as abscesses or perianal sepsis. Necrotizing fasciitis has not been reported in Crohn's disease. A case of a fatal necrotizing fasciitis in a patient with Crohn's disease after gut biopsies and corticosteroid therapy is reported.

Necrotizing fasciitis in infancy: an uncommon setting and a prognostic disadvantage.

Necrotizing fasciitis is a potentially fatal, progressive soft tissue infection that typically occurs in adults, and only rarely occurs in infants. Although adults in whom necrotizing fasciitis develops are commonly diabetic, malnourished, or otherwise immunocompromised, infants in whom the disease develops are typically healthy and without clear predisposing factors. Herein, however, the authors report the case of an infant with compromised immunity secondary to the manifestations and treatment of panhypopituitarism, in whom postoperative necrotizing fasciitis developed after bilateral inguin

Cervical necrotizing fasciitis and radiotherapy: a report of two cases.

Two cases of cervical necrotizing fasciitis are presented. Both are unusual in that they had had radiotherapy for head and neck malignancy six and 14 years previously with no evidence of tumour recurrence. One case of necrotizing fasciitis has previously been cited in the literature associated with radiotherapy (Krespi et al., 1981). The two cases are discussed along with a review of the literature.

[A fulminating case of Edwardsiella tarda septicemia with necrotizing fasciitis].

A 67-year-old Japanese male, suffering from liver cirrhosis with hepatoma, was admitted to the Yokohama National Hospital because of ascites retention. On physical examination, his abdomen was massively distended with ascites and his lower extremities were edematous. Laboratory findings on admission revealed hypoalbuminemia, moderate icterus, pancytopenia and hepatitis C virus antibody positivity. After admission, abdominal distention and edema were improved with the use of diuretics. On the 15th day of hospitalization, the patient noted diarrhea and bowel movements that occurred 10 times a da

Necrotizing fasciitis appearing with petechiae as the first clinical sign.

A 64-year-old Japanese man with necrotizing fasciitis is reported. He developed an Enterobacter agglomerans infection in his left leg without any known causative surgery or trauma, although he had acute myelocytic leukemia as a predisposing condition. Uncommonly, the first clinical sign was petechiae. Surgical debridement could not be performed because of pancytopenia due to his original disease and chemotherapy. The patient died, and an autopsy was performed. Severe intravascular coagulation was observed in the cutaneous infected tissues, while little or no disseminated intravascular coagulat

[Necrotizing fasciitis: a medical and surgical emergency. Apropos of a case].

We report about a 66-years-old obese and diabetic female patient, treated with anti-inflammatories for osteoarthritis of the hip and operated for varices of the lower limbs by a bilateral stripping of the internal saphenous veins, who presented with a mortal necrotizing fasciitis during the postoperative period. Necrotizing fasciitis is a severe, infrequent disease jeopardizing the vital prognosis, in which an appropriate and early treatment (medical, using antibiotics, and surgical by extensive debridement) can prevent a fatal outcome. The most often involved germs are streptococci (45%). The

Necrotizing fasciitis: a complication of squamous cell carcinoma of the vulva.

Necrotizing fasciitis is an often fatal, often initially unrecognized condition. Although it was first described over 60 years ago, occurrence in the vulva was only first recognized in 1972. The condition is most often associated with diabetes, prior injury, surgery, or irradiation. Aggressive surgical excision is required, early in the course of the disease, to salvage the patient. An association with vulvar carcinoma in a nondiabetic patient has not been previously reported. We report such a case, with a poor outcome, because surgical intervention was not possible until late in the course of

Necrotizing fasciitis in the pediatric age group: report of a case.

Necrotizing fasciitis is a rapidly progressive invasive soft tissue infection that is rarely seen in the pediatric population. In the majority of cases described in the medical literature, there has been an identifiable initiating event such as instrumentation or other trauma to the skin. Because of the rapid progression of the infection, the key to a successful outcome is early recognition and rapid initiation of definitive surgical management. A case is presented in which necrotizing fasciitis was seen in a child with no precipitating skin trauma.

Necrotizing fasciitis of the middle third of the face. A case report.

A 48-year-old man with necrotizing fasciitis in the right middle third face is presented. The patient had uncontrolled diabetes mellitus and liver cirrhosis. The condition was controlled by early diagnosis, prompt surgical management and adjunctive broad spectrum antibiotic cover after entering hospital, the curettage of the necrotizing tissue of the right cheek was especially effective.

Amniotic membranes as a temporary wound dressing in necrotizing fasciitis.

A diabetic patient was seen at Magee-Womens Hospital with a vulvar abscess extending to the abdomen. Necrotizing fasciitis was diagnosed at operation, and the involved tissue was widely excised. The authors describe the successful use of amniotic membranes as a temporary wound dressing. The benefits of this method over conventional gauze dressings are easy availability, better adherence to the wound, less pain, and negligible cost to the patient.

Necrotizing fasciitis and progressive bacterial synergistic gangrene of the vulva.

Necrotizing fasciitis and progressive bacterial synergistic gangrene are two infrequently reported diseases in the gynecologic literature. The author reports 2 cases highlighting the many similarities and some important differences between these clinical entities. Diabetes mellitus and arteriosclerosis predispose women to both diseases. Aggressive antibacterial and surgical treatment is imperative.

Cellulitis and necrotizing fasciitis of the abdominal wall in pediatric patients.

Soft tissue infections of the abdominal wall in 14 children were classified as cellulitis (8), necrotizing fasciitis (5), or myositis/myonecrosis (1). These 3 categories were characterized by increasing anatomic depth of infection, clinical severity, and need for more radical surgical treatment. Ten of the 14 children were neonates. The most frequent associations were omphalitis (5), necrotizing enterocolitis (4), and urachal anomalies (3). The severest infections were usually polymicrobial and contained both aerobic and anaerobic bacteria. Important clinical findings in children with necrotiz

Necrotizing fasciitis in postpartum patients. A report of four cases.

Necrotizing fascitis is a rare but rapidly progressive soft tissue infection not previously reported in an obstetric patient. Since cure is dependent on rapid recognition and early surgical intervention, 4 cases are reported herein, the bacteriology discussed, and the literature reviewed.
