In a series of 123 children with disorders of defecation, constipation was the main problem in 89 and fecal incontinence in 34. All but three of the latter also had constipation. The principal physical findings were abdominal protuberance with palpable stool in the bowel and the presence of a fecal mass in the rectal ampulla. Laboratory and radiologic findings were nonspecific except in two of the three patients found to have congenital megacolon (Hirschsprung's disease). Rectal suction biopsy, performed in 69 patients, was diagnostic in all three with Hirschsprung's disease. Anorectal manomet…

BACKGROUND/PURPOSE: Megarectum in association with anorectal malformation contributes to chronic constipation and fecal incontinence. Resection of megarectum in anorectal malformation improves bowel function, but neuropathy and poor sphincter quality may affect the outcome of fecal continence adversely. The aim of this study was to evaluate the benefits of resection of megarectum in anorectal malformation and to ascertain the impact of anal sphincter quality and neuropathy on the outcome.
METHODS: We studied 62 children with intractable fecal incontinence after repair of anorectal malformatio…

PURPOSE: Multiple instruments are used to assess fecal continence and bowel function after anorectal malformation (ARM) repair, but their comparability is uncertain. This systematic review examined inter-system agreement and differences in the findings produced when two or more scoring systems were applied to the same ARM cohort.
METHODS: MEDLINE, Embase, and Web of Science were searched from inception through 26 April 2026. Original studies applying at least two named fecal continence or bowel function scoring systems to the same ARM cohort were eligible. Risk of bias was assessed using an a

We recently read with interest the article "Diagnostic approach to faecal incontinence: What test and when to perform?". This is a comprehensive and practical review, which has particular significance for guiding clinicians to improve the examination strategy. Although we appreciate their work very much, based on the in-depth analysis of this research, we found some detailed problems in the article and will give our comments in this letter. If the author can further improve the relevant research, it will be valuable.

Paul is an 8-year-old boy with a long-standing history of encopresis and enuresis. Potty training was initiated when he was 2 years old. At this time, his mother was absent from the home for 6 weeks when she cared for her ill father in a different city. The process of teaching Paul to use the bathroom was described as "inconsistent" due to multiple caretakers.Paul never successfully mastered bowel and bladder control. He continues to wet and soil his clothes on a daily basis at home and school. According to his parents, he does not accept responsibility and comments about his soiling such as,

Faecal incontinence is a high prevalence disease in the general population. This pathology is commonly under-estimated and causes a great impact on clinical status and on the quality of life of affected patients. The prevalence of faecal incontinence in several studies has been estimated between 2% and 15% of the general population. The prevalence increases if we study selected populations, such as elderly people. The main cause of faecal incontinence is obstetric anal sphincter damage. In the past years, the presence of incontinence due to sphincter lesions, especially the obstetric ones, was

The aim of surgical treatment is to anatomically restore rectal prolapse and functionally remedy fecal incontinence and disorder of rectal emptying. There is not yet sufficient evidence-based knowledge of the advantages and disadvantages of various surgical methods, but in practice transabdominal surgery is recommended for patients in good condition and perineal surgery for elderly and frail patients suffering from associated diseases. The progress of laparoscopic surgery has, however, made the transabdominal operation possible for those in increasingly poor condition. With this procedure a si…

Faecal incontinence is a distressing and socially debilitating problem. Nurses are ideally placed to support patients and assist them in improving their quality of life. In order to provide the necessary holistic care the nurse needs to understand the nature of the patient's symptoms and be aware of a variety of management interventions. In this article the author outlines the various causes of faecal incontinence and highlights the importance of a thorough nursing assessment which takes into account the physical, psychological and social aspects of the symptoms. Planned care should be based o

The prevalence of bowel dysfunction in multiple sclerosis (MS) patients is higher than in the general population. Up to 70% of patients complain of constipation or faecal incontinence, which may also coexist. This overlap can relate to neurological disease affecting both the bowel and the pelvic floor muscles, or to treatments given. Bowel dysfunction is a source of considerable ongoing psychosocial disability in many patients with MS. Symptoms related to the bladder and the bowel are rated by patients as the third most important, limiting their ability to work, after spasticity and incoordina

BACKGROUND: Faecal incontinence is a debilitating problem with significant medical, social and economic implications. Treatment options include conservative, non-operative interventions (e.g. pelvic floor muscle training, biofeedback, drugs) and surgical procedures. Surgery is used in selected groupsof people when the structural and functional defects in the pelvic floor muscles or the anal sphincter complex can be corrected mechanically.
OBJECTIVES: To assess the effects of established surgical techniques for the treatment of faecal incontinence in adults who do not have rectal prolapse. Our

Faecal incontinence is a disabling disorder which profoundly undermines self-esteem. ESNS is a new operation which may be an alternative to colostomy. Good communication, knowledge and expertise are vital in caring for patients undergoing ESNS. All treatment options should be considered for patients with faecal incontinence.

Faecal incontinence is distressing both to the patient and the carers. However with accurate diagnosis of its cause, the condition can often be treated. Causes can be classified by pathophysiology. The most common cause in the elderly is faecal impaction with overflow incontinence. Other causes include inflammatory conditions of the bowel, neurological disorder, functional incontinence and iatrogenic incontinence. Management depends on an accurate diagnosis. A proper bowel and drug history is important. A rectal examination is mandatory, in order not to miss a diagnosis of faecal impaction wit

The surgical treatment of idiopathic faecal incontinence remains a difficult problem particularly in those cases where advanced neuropathy is present. The physiological basis for the post-anal repair have now been questioned and the long term functional results seem to be rather unsatisfactory. Pelvic floor repair is a more comprehensive approach with encouraging initial results which need to be assessed in a long term experiment and in a prospective randomised comparison with conventional post-anal repair. Gracilis transposition in idiopathic faecal incontinence has shown very disappointing r

PURPOSE OF REVIEW: Postpartum anorectal and pelvic floor disorders (PFD) are common, though under-recognized. There is limited knowledge regarding their diagnosis, treatment, and prevention. Here we provide a critical review of this topic and highlight knowledge gaps and treatment options for these problems.
RECENT FINDINGS: Recent advances include dynamic 3D and 4D ultrasound of the pelvic floor to reveal pathology, anal sphincter defects, and pelvic organ prolapse. Treatments for fecal incontinence include anal inserts, vaginal inserts, translumbosacral neuromodulation therapy (not yet stud…
