Epidermal barrier abnormalities in exfoliative ichthyosis with a novel homozygous loss-of-function mutation in CSTA.

Autosomal recessive exfoliative ichthyosis (AREI) results from mutations in CSTA, encoding cysteine protease inhibitor A (cystatin A). We present a 25-year-old man from Iran with consanguineous parents, who presented with congenital erythroderma, hyperhidrosis and diffuse hyperkeratosis with coarse palmoplantar peeling of the skin, aggravated by exposure to water and by occlusion. Candidate gene analysis revealed a previously unknown homozygous loss-of-function mutation c.172C>T (p.Arg58Ter) in CSTA, and immunostaining showed absence of epidermal cystatin A, confirming the diagnosis of AREI. [Pemphigus vulgaris: an unusual clinical presentation].

BACKGROUND: Pemphigus vulgaris is a bullous auto-immune disease affecting the skin and mucosa. It is characterised by acantholysis that results in the formation of intraepithelial bullous lesions. Herein we report a case distinguished by its unusual clinical presentation. PATIENTS AND METHODS: A 45-year-old man, a chronic smoker, consulted for hyperkeratotic lesions of the toes on the right foot present for 5 months. Examination revealed a violet colour of the toes associated with localised yellowish keratoderma on the sole of the foot and impaired toe nails, as well as impairment on the nail

Palmoplantar nonpustular psoriasiform dermatitis in a rhesus macaque.

A case of psoriasiform dermatitis in an adult male rhesus macaque is reported. Appearing spontaneously, the condition presented the clinical and histopathological features of human palmoplantar nonpustular psoriasis. The animal developed multiple scaly plaques on his palms and soles, as well as nail hyperkeratosis and widening of the nail root. Microscopically, the skin lesions showed epidermal hyperkeratosis with multifocal parakeratosis, neutrophil microabscesses in the stratum corneum, a loss of granule cell layer under the microabscesses, acanthosis, and elongation of the rete ridges; the

[Fungal infections and nail psoriasis].

Onychomycosis and psoriasis are common diseases and it is possible that they coexist in the same patient. Psoriasis is a common cause of disturbance of the nail morphology. Nail disorders was reported in 10-80% of patients with psoriasis and may be associated with all clinical forms of disease. It has been suggested that the nail dystrophy in patients with psoriasis produce the lose of the natural protective barrier and increase vulnerability to fungal infections. But most authors report that in patients with psoriasis the prevalence of onychomycosis is no higher than in the general population

Onychomycosis incidence in type 2 diabetes mellitus patients.

The onychomycosis incidence was determined in 250 type 2 diabetes mellitus (T2DM) patients who were registered at the Internal Medicine Service from a Mexico city General Hospital throughout a year (January-December 2006). Out of the total of studied T2DM patients, 93 (37.2%) showed ungual dystrophy and from these, in 75.3% a fungal etiology was corroborated. Out of 70 patients, 34 were men and 36 women, with an average of 63.5 years. Correlation between T2DM evolution time and onychomycosis was significant (P < 0.01). Distal-lateral subungual and total dystrophic onychomycosis were the most f

Atypical hand-and-foot syndrome in an African American patient treated with capecitabine with normal DPD activity: is there an ethnic disparity?

Fluoropyrimidines constitute the backbone of chemotherapy regimens for gastrointestinal tumors, especially colorectal cancer (CRC). Comparison of oral and intravenous fluoropyrimidine treatments in patients with CRC has shown that hand-and-foot syndrome (HFS) (all grades) has the most frequently reported association with toxicity from capecitabine, an oral fluoropyrimidine. Present grading guidelines are generalized to patients belonging to all ethnicities. However, we noticed varying pattern and degree of manifestations of all grades of HFS in non-white patients as compared with white patient

Contact allergy to resorcinol monobenzoate.

Resorcinol monobenzoate is an ultraviolet light absorber used in cellulose plastics and is known to cause allergic contact dermatitis reactions. It is the most important allergen in spectacle frames but can be a potential allergen in shoes as well. We report 7 cases of allergy to resorcinol monobenzoate with different clinical presentations. Our series further illustrates several possible cross-sensitizers.

Palmoplantar lesions in psoriatic patients and their relation to inverse psoriasis, tinea infection and contact allergy.

One hundred and seven psoriatics with palmoplantar involvement were clinically examined. They were selected from 921 patients filed in 1976 with the diagnosis psoriasis. Fifty per cent of the patients with palmoplantar psoriasis had flexural changes. The frequency of palmar involvement in patients with inverse psoriasis compared to patients with psoriasis vulgaris was increased 5.3 times. Pustulosis palmoplantaris (PPP) in addition to psoriasis was diagnosed in 18 patients (17%). A dermatophyte infection was observed in 1 of 48 patients examined for tinea infection. Positive patch tests were o

[An aetiological study on pustulosis palmaris et plantaris due to tonsillar focal infection (author's transl)].

In clarification of the cause of Pustulosis palmaris et plantaris caused by tonsillar infection 19 patients with this condition and undergoing tonsillectomy were studied. IgE levels in the tonsils and blood serum were estimated both before and after tonsillectomy including various other laboratory tests. These results and the therapeutic effects of the tonsillectomy were analysed. 42 patients with tonsillitis undergoing tonsillectomy were used as controls. Lowered serum complement (C3) titres and decrease in neutrophil counts and serum IgG levels were observed in those patients whose cutaneous

Clofazimine-enhanced phagocytosis in pustulosis palmaris et plantaris.

The phagocytic ability of neutrophil leucocytes was found to be impaired in patients suffering from pustulosis palmaris et plantaris (PPP). This ability was studied with the aid of the yeast particle method. In 78% of 27 patients whose PPP was in a static phase, the phagocytic function was enhanced, while the PPP abated concomitantly and the pustules disappeared. Impaired phagocytosis and the beneficial effect of clofazimine signify the pathogenic importance of defective neutrophils in PPP.

Onychomycosis in Diabetics: Prevalence, Clinical Features and Spectrum of Causative Fungi in South-Eastern Nigeria.

INTRODUCTION: Onychomycosis is a fungal infection of the nail unit and one of the common nail diseases that occurs commonly in diabetic patients. It poses a threat of diabetic foot complications to diabetics and negatively affects the quality of life of the patients. OBJECTIVES: The overall aim of the study was to determine the prevalence and clinical features of onychomycosis in diabetics, as well as the spectrum of causative fungi in Nigeria as compared with age and sex-matched controls. METHODOLOGY: This was a hospital-based, comparative cross-sectional study. One hundred and fifty consec

Nail matrix arrest in the course of hand, foot and mouth disease.

UNLABELLED: Onychomadesis describes complete nail shedding from the proximal portion; it is consecutive to a nail matrix arrest and can affect both fingernails and toenails. It is a rare disorder in children. Except for serious generalised diseases or inherited forms, most cases are considered to be idiopathic. Few reports in literature concern common triggering phenomena. We present four patients in whom the same benign viral condition in childhood appeared as a stressful event preceding onychomadesis. In each case, spontaneous complete healing of the nails was achieved within a few weeks. C

Allergic contact dermatitis: Kids are not just little people.

Over the last ten years, there has been an increased awareness of allergic contact dermatitis (ACD) in children, in addition to adults. Historically, ACD was not considered a significant disease in the pediatric population. This may have been due to failure to employ patch testing, which is the gold standard for establishing a diagnosis of ACD. A number of epidemiologic studies now reflect upon the significance of positive patch tests in children and its utility in diagnosing ACD. While there is significant overlap among the most common allergens in children and adults, the patterns of exposur

[Treatment of therapy refractory verrucae vulgares with a ciclopirox-containing lacquer].

BACKGROUND AND OBJECTIVE: Anti-inflammatory actions of ciclopirox, an antifungal agent, have been described previously. We assessed the effectiveness of ciclopirox in the treatment of viral warts. PATIENTS/METHODS: Twenty-three immunocompetent patients (age 9-61 years) with common warts at the hands, feet and in the face resistant to conventional therapy were treated. Following keratolysis with a salicylic acid-containing patch, an 8% ciclopirox lacquer was applied 1-2 times daily. At the beginning and the end of the treatment lesions were photographed and the time span (minimum 4 weeks) unti
