Extruded Filshie clip presenting as an ischiorectal abscess.

This report adds to the small, but significant, literature base describing late complications following laparoscopic sterilisation. In women with recalcitrant peri-anal sepsis (who have previously undergone a sterilisation procedure) the possibility of tubal clip migration should be borne in mind. This is also an important learning point from a medicolegal point of view as patients presenting with the sequelae of clip migration will need to be counselled, and possibly investigated, with respect to the efficacy of their sterilisation procedure.

Intractable abdominal pain following laparoscopic adjustable gastric banding.

A 42-year-old woman presented with intractable crampy abdominal pain continuing for 1 year. The pain worsened after eating and improved when walking. She had undergone laparoscopic adjustable gastric banding 4 years previously, with re-banding 18 months later because of anterior band slippage. The patient underwent numerous examinations, all with normal findings. A scintigraphy finally showed some tracer enrichment in the terminal ileum, which led to the assumption that a Meckel's diverticulum was causing her discomfort. Subsequent diagnostic laparoscopy showed no Meckel's diverticulum but ins

Thyroid uptake of 125iodine after prostate permanent brachytherapy.

PURPOSE: To decrease the seed migration rate after 125I prostate brachytherapy and improve the quality of implants to our knowledge we report for the first time the detection of thyroid uptake, possibly from a damaged seed, in a patient after 125I implantation. MATERIALS AND METHODS: Seed migration detection is routinely offered to our patients at the post-evaluation visit. A seed migration detector and comprehensive investigation procedure have been developed for the task. Chest radiographs are ordered to document the location of the detected seed. A fluoroscopic study may be performed if in

Imaging diagnosis: pneumothorax and focal peritonitis in a dog due to migration of an inhaled grass awn.

A 1-year-old working Kelpie developed pneumothorax and focal peritonitis after inhalation of a grass awn that migrated from the lung, through the diaphragm, into the peritoneal cavity. Radiographic evidence of sternal lymph node enlargement was fundamental in the diagnosis of intraperitoneal disease and prompted abdominal ultrasound leading to definitive diagnosis.

Laparoscopic repositioning of Heliogast gastric band after anterior slippage.

BACKGROUND: Gastric slippage is a well-described complication of gastric banding. The Heliogast band is equipped with a locking mechanism that enables its straightforward reopening and repositioning. Our experience with Heliogast band salvage after anterior slippage is reported. METHODS: The study sample comprised 418 consecutive patients who underwent 2-step laparoscopic gastric banding with the Lap-Band first (n=235) followed with the Heliogast band (n=183). Gastric slippage was diagnosed by symptoms of dysphagia and vomiting and was confirmed with Gastrografin swallow. Patients who did not

An unusual cause of small bowel obstruction: gossypiboma--case report.

BACKGROUND: The term "gossypiboma" denotes a mass of cotton that is retained in the body following surgery. Gossypiboma is a medico-legal problem especially for surgeons. To the best of our knowledge, the patient presented herein is the second reported patient in whom the exact site of migration of a retained surgical textile material into the intestinal lumen could be demonstrated by preoperative imaging studies. CASE PRESENTATION: A 74-year-old woman presented with symptoms of small bowel obstruction due to incomplete intraluminal migration of a laparotomy towel 3 years after open cholecyst

Extrauterine translocated contraceptive device: a presentation of five cases and revisit of the enigmatic issues of iatrogenic perforation and migration.

Translocation of an intrauterine contraceptive device to an extrauterine site in the peritoneal cavity is an uncommon complication. In cases reported in literature, the timing of extrauterine presentation and the distant sites of translocation often raise the issue of whether iatrogenic uterine perforation or migration of the device was responsible. We present and discuss five referred cases of the extrauterine device inserted in centres outside the University of Port Harcourt Teaching Hospital. The indication for insertion of the intrauterine contraceptive device in the patients (mean age 25.

[Intracardial migration of a bullet: diagnosis and management].

Venous pellet embolism to the cardia after shotgun wound is a very rare occurrence. Number and size of pellets, at the impact make this migration easier; embolism is asymptomatic and may occur 15 years after the injury. Many problems must be mentioned: mechanics of entry into the heart (own velocity, venous flow), topographic diagnosis (chest X-ray, transthoracic, transoesophageal ultrasound and CT-scan), local outcomes of this projectile (local erosion, clot, endocarditis), destination of a new migration (pulmonary embolism, left heart), indications of extraction, supervision. Extraction musn

Case report: further upward migration of a misplaced double-J ureteral stent.

A 45-year-old man was referred to our hospital for the removal of a misplaced double-J ureteral stent. The lower end of the stent shortly after insertion was at the level of sacroiliac joint. However one week later the stent was further migrated upwards to the level of 4th lumbar vertebra. Immediate ureterorenoscopy was performed to prevent further migration of the stent to the renal pelvis and the stent was removed by the grasping forceps under direct vision successfully.

Transurethral retrieval of a proximally dislodged ureteral stent.

A proximally migrated ureteral stent into the renal pelvis is an uncommon problem that poses a difficult technical challenge. A 32-year-old woman had her ureteral stent inadvertently dislodged into the renal pelvis at the time of stent exchange. The stent was successfully retrieved and repositioned transurethrally with a snare fed through the left ureter and into the renal pelvis. This is a relatively noninvasive method of retrograde retrieval of a proximally dislodged ureteral stent.

Intravesical migration of intrauterine device resulting in stone formation.

The intrauterine contraceptive device has been in use for many years, and migration of the device from the uterus to the pelvic cavity has been reported by many investigators. Although perforation of the uterus by an intrauterine contraceptive device is not uncommon, intravesical migration and secondary stone formation are rare complications. We report on a 27-year-old woman in whom an intrauterine contraceptive device (Copper-T) migrated from the uterus to the bladder and resulted in formation of a stone.

[Migration of the catheter of a totally implantable venous system. A rare mechanical complication].

Catheter migration, represents a rare, mechanical complication following the use of totally implantable venous devices. A case, in which the catheter was inserted into the right internal jugular vein by percutaneous route, without radiological control, is described. Following the chemotherapy infusion drugs, the patient developed a catheter displacement and subcutaneous extravasation. Etiological hypotheses and modalities of prevention are discussed.

[Double J ureteral catheter. Clinical complications].

OBJECTIVE: To analyze the clinical complications of double-J ureteral catheters. METHODS: The most relevant studies published in the literature since the self-retaining indwelling catheter was first described in 1967 are reviewed. RESULTS: The experience and results reported in the most relevant studies are presented. CONCLUSIONS: The double-J ureteral stent has become an integral part of the urological armamentarium. It allows good urinary drainage from the kidney to the bladder and is generally safe and well-tolerated. However, different complications may occur with short- or long-term us

Scrotal perforation by a ventriculoperitoneal shunt.

A case is reported of an adult male in whom the distal ventriculoperitoneal shunt tip migrated through the right inguinal canal into the scrotum. It perforated the scrotal skin, leading to an infection of the still draining shunt. Migration of ventriculoperitoneal shunts into the scrotum occurs occasionally in children but until now has not been reported in an adult with perforation of the scrotum.

[A case of cerebral arteriovenous malformation with accidental migration of a microcatheter during endovascular procedure].

This is a report of a case of cerebral arteriovenous malformation with accidental breakaway of a microcatheter during endovascular embolization. A 25-year-old female suddenly suffered severe headache. CT scan demonstrated intraventricular hemorrhage, and the angiogram revealed right parieto-occipital AVM. We performed endovascular embolization using a flow-directed microcatheter (1.5F BALT MAGIC catheter) 2 weeks after onset, and the microcatheter broke accidentally and a fragment of the soft tip migrated to the distal posterior cerebral artery during the procedure. Although we tried to retrie

Arterial bullet embolism resulting in delayed vascular insufficiency: a rationale for mandatory extraction.

This paper reports a case of migrating intravascular bullet embolus that initially produced no symptoms but resulted in an above-knee amputation 14 months after its entry into a peripheral artery. The missile entered through a penetrating gunshot wound to the abdominal aorta and later became lodged in the left popliteal artery. However, the bullet fragment migrated further into the posterior tibial artery with consequent vascular insufficiency requiring a supracondylar amputation of the left lower extremity 14 months later. Emphasis is placed on the need for a high index of suspicion for bulle

Mechanisms of unexpected death in infants and young children following foreign body ingestion.

Fatal foreign body ingestion in childhood usually results in sudden and unexpected death from acute upper airway occlusion. The most common age range for such episodes is one to three years. However, a variety of different mechanisms of death due to ingested foreign bodies may occur in children, including hemorrhage, acute cardiac tamponade, arrhythmia, centrally mediated respiratory arrest and sepsis. Sudden death may follow a protracted asymptomatic period and may also be due to foreign bodies impacted in the esophagus. A review of cases has been undertaken (N = 10; age = three and one-half
