Modified cerclage wiring in comminuted transolecranon fracture-dislocations of the elbow.

Transolecranon fracture-dislocations are a result of high-energy trauma, caused due to axial loading of the flexed forearm, with associated anterior dislocation of the ulna with respect to the distal humerus. The usual management of these comminuted and unstable fractures is by using locking compression plates via the dorsal approach. However, plating in cases of poor soft tissue coverage and open wounds can be precarious. In this study, we aimed to evaluate outcomes of cerclage wiring in the management of comminuted trans-olecranon fracture-dislocations in such scenario. A total of seven pati

Fractured coracoid process with acromioclavicular joint dislocation: A case report.

RATIONALE: Coracoid processes (CPs) fracture with acromioclavicular (AC) joint dislocation are extremely rare. This combined injury has brought many challenges to surgeons, and the mechanism underlying the injury is still not fully understood. There is no clear consensus on its treatment. PATIENT CONCERNS: Here, we describe a CP fracture with AC joint dislocation in a middle-aged manual worker. DIAGNOSIS: Radiographs showed a fracture of the base of the CP and a third-degree AC joint separation. INTERVENTIONS: The patient was treated surgically with open reduction and internal fixation of t

Clinical and radiological evaluation of surgical management in olecranon fracture-dislocations.

PURPOSE: The treatment of olecranon fracture-dislocations (OFDs) remains challenging. OFDs are often misdiagnosed as Monteggia lesions, and the real frequency is actually higher. However, studies on OFDs are limited. This study aimed to report on the surgical management of OFDs and to highlight the importance of three-dimensional computed tomography (3D CT) evaluation in the treatment of OFDs. MATERIALS AND METHODS: The study participants included 18 patients (11 men, 7 women, mean age 44 years (range 24-78) with OFDs. Each patient's medical records, radiographs, and 3D CT scans were reviewed

Multiple carpometacarpal dislocations and an associated fracture of the hamate: an uncommon injury.

Dislocation of the four ulnar carpometacarpal (CMC) joints with a concurrent fracture of the hamate is a rare injury, with a paucity of published cases. A great force is required to dislocate a CMC joint. Diagnosis can easily be missed, due to other serious associated injuries. Appropriate treatment of CMC joint dislocations usually leads to excellent outcomes. We present a case of dorsal dislocation of the four ulnar CMC joints after punching a wall. The injury was treated with a closed reduction and percutaneous transfixation with Kirschner-wires. Despite the extensive injury, the functional

Fractures and Dislocations About the Elbow and Their Adverse Sequelae: Contemporary Perspectives.

Fractures and dislocations of the elbow can result in adverse outcomes. The elbow is a unique joint that allows for great mobility but is predisposed to instability, either simple or complex, in traumatic settings. Even simple elbow instability, in which no fracture is present, may be associated with tremendous soft-tissue injury. Surgical treatment is often required for complex instability in which various fractures are present. The treatment goals for fixation of elbow fractures and dislocations include stable fracture fixation, a stable concentrically reduced joint, and early range of motio

Suspension Arthroplasty for Old Fracture-Dislocations of the Fifth Carpometacarpal Joint.

It is well known that treatment of the old fracture-dislocations of the fifth carpometacarpal (CMC) joint is challenging. For patients with persistent pain due to this condition, numerous surgical techniques have been proposed. Here, we present a novel surgical technique, CMC joint suspension arthroplasty, using a partial slip of the extensor carpi ulnaris tendon. This procedure follows principles similar to the procedure of trapeziectomy with suspension arthroplasty for treatment of thumb CMC joint arthritis. We believe this suspension arthroplasty has several advantages compared with other s

Surgical treatment of dorsal perilunate fracture-dislocations and prognostic factors.

INTRODUCTION: Perilunate injuries are rare entities which can be difficult to diagnose. Most common type is dorsal perilunate fracture dislocation (97%). The purpose of treatment is anatomic reduction and stable fixation. We aimed to present the radiologic and functional results of surgically treated dorsal perilunate fracture-dislocations and discuss the factors influencing the prognosis. METHODS: Between 2007 and 2013, 17 patients were operated for perilunate fracture-dislocations. The mechanism of injuries, soft tissue traumas, etiologic factors and stages according to Herzberg classificat…

Talar Fractures and Dislocations: A Radiologist's Guide to Timely Diagnosis and Classification.

The talus, the second largest tarsal bone, has distinctive imaging characteristics and injury patterns. The predominantly extraosseous vascular supply of the talus predisposes it to significant injury in the setting of trauma. In addition, the lack of muscular attachments and absence of a secondary blood supply can lead to subsequent osteonecrosis. Although talar fractures account for less than 1% of all fractures, they commonly result from high-energy trauma and may lead to complications and long-term morbidity if not recognized and managed appropriately. While initial evaluation is with foot

Case of bilateral Galeazzi fractures associated with dislocation of the right elbow.

We report an unusual case of bilateral Galeazzi fractures associated with dislocation of the right elbow and fracture of the right scaphoid caused by a motorbike accident in a 32-year-old man. Bilateral radiuses were fixed with plates, and the right scaphoid was fixed with a screw after closed reduction of the right elbow. A satisfactory result was obtained with a return to work and no radiographic problems 7 years after surgery. The mechanism of the injury was thought to be a high-velocity fall on the outstretched hands combined with extreme pronation of the forearms and extension of the wris

High dislocation cumulative risk in THA versus hemiarthroplasty for fractures.

BACKGROUND: Although not all elderly patients with femoral neck fractures are candidates for THA, active, mentally competent, independent patients achieve the most durable functional scores with THA compared with hemiarthroplasty. However, a relatively high frequency of early or late dislocation could reduce the potential benefits with THA. QUESTIONS/PURPOSES: We asked whether the incidence of first-time, recurrent dislocation, and revision differed in patients with hip fractures having THA or hemiarthroplasty. PATIENTS AND METHODS: We retrospectively reviewed 380 patients with hip fractures

Outcome of distal clavicular fracture separations and dislocations in immature skeleton.

BACKGROUND: There are only a few studies with long-term follow-up of distal clavicular injuries in children and their treatment is not clearly defined. The purpose of our study is to suggest a new classification system according to the fracture pattern and the degree of the displacement, to evaluate the long-term follow-up and also to propose an algorithm for treatment. METHODS: We conducted a retrospective study from 1983 to 2008. Seventy-five children and adolescents, age ranging from 3 to 16 years (46 >8 years), were treated in our department. We classified all these injuries into five gro

Acute dislocations of the distal radioulnar joint and distal ulna fractures.

This article reviews acute dislocations of the distal radioulnar joint (DRUJ) and distal ulna fractures. Acute dislocations can occur in isolation or in association with a fracture to the distal radius, radial metadiaphysis (Galeazzi fracture), or radial head (Essex-Lopresti injury). Distal ulna fractures may occur in isolation or in combination with a distal radius fracture. Both injury patterns are associated with high energy. Outcomes are predicated on anatomic reduction and restoration of the stability of the DRUJ.

Intramedullary nailing for the treatment of dislocated pediatric radial neck fractures.

INTRODUCTION: Pediatric radial neck fractures represent 5-10% of all elbow fractures. Open reduction allows anatomic fracture fixation but compromises the vulnerable blood supply of the radial head. Intramedullary nailing combines the advantages of closed reduction and stable internal fracture fixation. MATERIAL AND METHODS: During a 6-year-period the medical records of all children sustaining a radial neck fracture treated operatively at our institution were analyzed retrospectively. RESULTS: Forty-two patients with dislocated fractures of the radial neck were treated with intramedullary na

Surgical treatment on displaced and dislocated sagittal fractures of the mandibular condyle.

PURPOSE: The purpose of this study was to evaluate the effect of surgical treatment on displaced and dislocated sagittal fractures of the mandibular condyle (SFMC). PATIENTS AND METHODS: Twenty-four patients with 28 displaced and dislocated SFMCs were distinguished into type M, type C, and type L fractures according the location of the fracture line. The fractured fragment was reduced and fixated with two 0.6-mm 4-hole micro-plates via a preauricular temporal incision. The fragment was extirpated when it was too small to be fixated. The postoperative position and profile of the fragment was e

Patient experience of distraction splinting for complex finger fracture dislocations.

The study design is qualitative phenomenological and grounded theory. Intraarticular fractures of the finger joints can severely limit function due to stiffness and pain. Distraction with early movement is thought to deliver the best results and this has been used to treat these types of injuries at The Alfred Hospital for eight years. Qualitative data from patient interviews were used to describe patients' own experiences of treatment with distraction splinting and identify key issues in patient adherence. The key finding was a disconnect between perceived complexity of injury and treatment.

"Stabilized arthroplasty" for old fracture dislocations of the fifth carpometacarpal joint.

The procedure of stabilized arthroplasty we present in this paper aims at a global functional restoration of old fracture dislocations of the fifth carpometacarpal joint. The conflict is eliminated by resecting the base of the metacarpal, whereas length of the fifth digit ray is restored by fusion to the adjacent fourth metacarpal. Fifth metacarpal mobility is maintained via transfer to the fourth carpometacarpal joint. The base of the fifth metacarpal is resected through a dorso-lateral approach to the fourth-fifth intermetacarpal space. The preferred plane of resection is not perpendicular t…
