Fracture severity dependence of bone and muscle performance in patients following single or multiple vertebral fractures.

BACKGROUND: Few studies focus on the clinical, laboratory, radiological, and biological characteristics of bone and muscle of multiple vertebral fractures, which are associated with a more poor prognosis compared with single fracture. PURPOSE: To compare the BMD, bone turnover, muscularity, fatty infiltration of muscle, and prevalence of co-morbidities in patients with single and multiple vertebral fractures. METHODS: We recruited 100 patients with single fracture (age 66.96 ± 8.24 years) and 100 with multiple fractures (age 69.90 ± 7.80 years); performed dual-energy X-ray absorptiometry of

The Association Between Short-Term Bone Loss and Future Osteoporotic Vertebral Fractures.

OBJECTIVE: To investigate the relationship between longitudinal change in Hounsfield Units (HU) value and future osteoporotic vertebral fracture (OVF). METHODS: This longitudinal case-control study involved 52 cases with future OVF and 104 age- and sex-matched controls. Each participant underwent two chest computed tomography (CT) scans. The baseline regional (anterior, middle, posterior, superior, inferior) and integral HU value from T4, T7, T10, and L1 vertebrae were measured. The coefficient of variation (CV) indicated heterogeneity in HU. Bone loss was quantified by the annual rate of HU

Maltol induces diabetic fragility fractures by disrupting the balance of bone remodeling.

Type 2 diabetes is a major risk factor for fragility fractures, yet the contributors to skeletal fragility remain unclear. Through integrated clinical metabolomics, in vivo, and in vitro analyses, we identify maltol-a widely used food additive-as a previously unrecognized risk factor for hyperglycemia-associated bone fragility. Metabolomic profiling of femoral neck tissue from individuals with fragility fractures showed diabetes-associated maltol accumulation, and elevated circulating maltol levels correlated with increased fracture incidence. Mechanistically, maltol inhibits osteoblast differ

Bone void filler materials for augmentation in comminuted fractures: a comprehensive review.

Comminuted fractures, characterized by multiple bone fragments, present significant challenges in orthopedic surgery. Effective treatment often requires augmentation techniques to enhance fixation stability and promote bone regeneration. This review explores the application of bone void filler materials, including autografts, allografts, polymethyl methacrylate (PMMA), and synthetic bone substitutes such as calcium phosphate ceramics and bioactive glass, in managing comminuted fractures. Autografts are the gold standard due to their superior osteogenic potential but are limited by donor site m

Bone loss, osteoporosis, and fractures in patients with resected pancreatic head cancer.

BACKGROUND: Major pancreatic resections are considered to have a high risk of developing osteoporosis. This study aimed to examine factors associated with bone mineral density (BMD) loss, osteoporosis, and fractures among pancreatic head cancer patients who underwent resection. METHODS: We retrospectively investigated consecutive pancreatic head cancer patients who underwent resection and had evaluable computed tomography (CT) before surgery and at one-year follow-up. BMD was measured using CT images at the L1 vertebra. Osteoporosis was defined as BMD <135 Hounsfield units. The prevalence and

Low Rates of Bone Health Evaluation Before and After Primary Fragility Fractures.

BACKGROUND: Osteoporotic fractures pose a critical challenge in patient care, with earlier work pointing to a significant shortfall in preventing subsequent fractures. This study aimed to determine the rate and predictors of bone health evaluations (BHE) before and after an operatively treated first-time fragility fracture and assess the imminent risk of a subsequent fracture. METHODS: This retrospective cohort study at a level I trauma center included patients aged 60 years or older who underwent surgery for fragility fractures (hip, pelvis, wrist, humerus, or femur) due to low-energy mechan

Nonunion in Long Bone Fractures: A Comprehensive Review of Current Treatment Strategies.

Nonunion is a significant complication of long bone fractures, occurring when bone healing fails to progress despite adequate time and appropriate initial management. This condition affects a significant proportion of fractures, with rates varying considerably based on the anatomical location, fracture pattern, patient factors, and treatment methods. The management of nonunion remains challenging and requires a comprehensive understanding of the mechanical and biological factors influencing bone healing. This narrative review systematically examines the current treatment options available for

Multiple pathological bone fractures in a dog with adrenal-dependent Cushing's syndrome.

Pathological bone fractures may occur secondarily to Cushing's syndrome in people. Although low bone mineral density due to cortisol excess has been described in dogs, there is no evidence that dogs with Cushing's disease can develop osteoporosis or secondary bone fractures. Here, we report a case of a dog with Cushing's disease and multiple pathological bone fractures. A 14-year-old female spayed mixed-breed dog was attended due to progressive lethargy, weakness, difficulty moving, trembling and clinical signs of Cushing's syndrome (polyuria, polydipsia, alopecia and abdominal distension). Variation in treatment of long bone gunshot wound fractures across South Africa.

INTRODUCTION: The incidence of civilian gunshot injuries is on the rise worldwide. Unfortunately, there is a lack of high-level evidence guiding management. The treatment of orthopaedic injuries from gunshots is complex and requires consideration of multiple aspects, including energy transfer to the tissue, severity of the wound, possible contamination, presence of fractures and associated injuries. Our study aimed to describe the variations in the treatment of GSW fractures across orthopaedic units across South Africa, a country with a high burden of GSW injuries. METHODS: The gunshot-relate

Monotherapy Antibiotic Prophylaxis of Lower Extremity Long Bone Type III Open Fractures.

Gustilo type III open fractures involve extensive soft tissue damage and wound contamination that pose significant infection risks. The historical standard for antibiotic prophylaxis has been cefazolin and gentamicin. We conducted a retrospective cohort study of lower extremity type III open fractures treated with ceftriaxone alone for prophylaxis. Eighty-six patients were identified. Nearly all (98%) were managed with appropriate antibiotics, but only 55 (64%) received prophylaxis within 1 hour. Overall, there were 12 infections. This infection rate was not statistically different than the re

Radiotherapy-related insufficiency fractures and bone mineral density: what is the connection?

BACKGROUND: Radiotherapy-related insufficiency fractures (RRIFs) represent a common, burdensome consequence of pelvic radiotherapy. Their underlying mechanisms remain unclear, and data on the effect of osteoporosis are contradictory, with limited studies assessing bone mineral density (BMD) by dual-energy x-ray absorptiometry (DXA). METHODS: BMD by DXA (Hologic) scan and fracture risk following pelvic RRIF were retrospectively assessed in 39 patients (median age 68 years) at a tertiary cancer centre. Patient characteristics and treatment history are presented narratively; correlations were ex

Portal vein thrombosis is associated with an increased risk of bone fractures.

BACKGROUND: Portal vein thrombosis (PVT) is a rare but severe disease that often leads to portal hypertension-related complications. It is well-known that patients with portal hypertension associated with liver cirrhosis are at increased risk for bone fractures, however data on the impact of PVT on fracture risk are lacking. AIMS: This study aimed to explore the impact of PVT on the incidence of bone fractures in a large German primary care cohort. METHODS: Patients with PVT were extensively matched to non-PVT individuals in a 1:5 ratio. The primary outcome of the study was the incidence of

Expert consensus on the bone repair strategy for osteoporotic fractures in China.

Osteoporotic fractures, also known as fragility fractures, are prevalent in the elderly and bring tremendous social burdens. Poor bone quality, weak repair capacity, instability, and high failure rate of internal fixation are main characteristics of osteoporotic fractures. Osteoporotic bone defects are common and need to be repaired by appropriate materials. Proximal humerus, distal radius, tibia plateau, calcaneus, and spine are common osteoporotic fractures with bone defect. Here, the consensus from the Osteoporosis Group of Chinese Orthopaedic Association concentrates on the epidemiology, c

Vitamin D Supplementation and Its Impact on Different Types of Bone Fractures.

Vitamin D helps to balance the levels of calcium and phosphorus to maintain proper bone structure. It is also involved in essential biological roles and displays a wide spectrum of potential benefits in the human body. Since there are many types of fractures that occur at specific ages and due to different circumstances, the influence of vitamin D on the frequency of a particular fracture may differ. Thus, the authors investigated the possible preventive effect of vitamin D on the risks of vertebral fractures, hip fractures, stress fractures and pediatric fractures. Additional aspects of vitam
