"Rewarding and challenging at the same time": emergency medicine residents' experiences caring for patients who are homeless.

OBJECTIVES: The objectives were to examine how emergency medicine (EM) residents learn to care for patients in the emergency department (ED) … affects residents emotionally in complex, multifaceted ways. Emotions were dominated by feelings of frustration. This frustration was often related to feelings of futility in truly helping homeless patients, particularly for patients who were frequent visitors to the ED and who had concomitant alcohol dependence. CONCLUSIONS: Caring for ED patients who are h …

Near-Misses and Stop Buttons in Slot Machine Play: An Investigation of How They Affect Players, and May Foster Erroneous Cognitions.

In modern casinos, multiline slot machines are becoming increasingly popular compared to traditional, three-reel slot machines. A paucity of … ed that near-misses encountered in the stop button game would trigger greater levels of arousal and frustration in players, as indexed by larger SCRs, and greater force applied to the spin button to initiate the next spin. Erroneous cognitions pertaining to the stop button and near-misses respectively were assessed following play. Results showed that a small …

Assessing expressed emotion: comparing Camberwell Family Interview and Five-minute Speech Sample ratings for mothers of children with behaviour problems.

Little is known of the concordance between ratings of expressed emotion (EE) derived from the Camberwell Family Interview (CFI) and Five-minute Speech Sample (FMSS) for parents of children with behaviour problems. Concordance between CFI and FMSS ratings of EE was assessed prior to intervention and compared to parent-rated behaviour after intervention, at follow-up, 12 months later. Female primary caretakers of 75 children (3-10 years) showing behavioural difficulties were interviewed using FMSS and CFI. Interviews were coded independently by criterion-standard raters. Using CFI, 57 families w

Examining the association between rumination, negative affectivity, and negative affect induced by a paced auditory serial addition task.

The present study examined the relations among a depressive ruminative response style, a general propensity to experience negative affectivity, and negative affect induced by a paced serial auditory addition task (PASAT). Ninety nonclinical individuals completed a computerized version of the PASAT, which elicits a generalized negative affect response [Lejuez, C. W., Kahler, C. W., & Brown, R. A. (2003). A modified computer version of the paced auditory serial addition task (PASAT) as a laboratory-based stressor: Implications for behavioral assessment. Behavior Therapist, 26, 290-292]. As hypot

The effect of 6-OHDA lesions of the lateral septum on schedule-induced polydipsia.

In this study the relationship between 6-hydroxydopamine lesions of dopaminergic innervation of the lateral septum and schedule-induced polydipsia was examined. Animals with lesions were found to have a significant increase in water intake during a test involving an intermittent schedule of food delivery. Additional experiments with these animals showed that lesions had no effect on: (i) spontaneous drinking, the amount of drinking after 24 h food or water deprivation, water intake following a hypertonic saline challenge; (ii) eating behavior with or without food deprivation; and (iii) spontan

Identification of the roaming mechanism and the formation of H3+ in core ionised cyclopropane.

Spectroscopic validation of the roaming process in molecular dissociation was first reported for formaldehyde, but today roaming is acknowledged to be a nearly ubiquitous dissociation pathway. The process has been described as a frustrated dissociation process, and as such, direct detection of the roaming process is challenging. In the present study the cyclopropane dication is the starting point for a roaming process that results in the production of H3+. In our experiment, we measure the complete momenta of all charged fragments and unravel the kinematics for relevant fragmentation processes

Understanding Myalgic Encephalomyelitis/Chronic Fatigue Syndrome and the Emerging Osteopathic Approach: A Narrative Review.

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a debilitating syndrome of unknown origin, characterized by profound postexertional malaise and fatigue, unrefreshing sleep, cognitive impairments, immune dysfunction, pain, autonomic dysfunction, and neuroendocrine symptoms. Although ME/CFS is well documented within the medical literature, it remains difficult to diagnosis and manage. Some of the current challenges include an absence of diagnostic markers, differing diagnostic criteria, and an overall lack of awareness within the medical community. As a result, patients are often

[Inapparent charges for the assistance to nephropathic patient on dialysis].

The increasing technological effectiveness has undoubtedly produced an improvement in clinical parameters of dialysis patients, but this satisfactory therapeutic result did not follow an adequate improvement in mortality or in the perception of quality of life as per patients. Furthermore, dialysis treatment is often associated with "inapparent charges" that reduce the perception of well-being, independently of clinical changes. Thirty years ago, we carried out a national survey on inapparent charges, which represent frustrating aspects that negatively affect patients' perception of their qual

Managing highly insulin-resistant diabetes mellitus: weight loss approaches and medical management.

The prevalence of obesity and diabetes is epidemic. Severe insulin resistance (defined as the need for > or = 200 units of insulin per day to achieve glycemic control) is commonly seen with obesity and can complicate diabetes management. The management of patients with diabetes who have severe insulin resistance is difficult, and at times frustrating, and requires a multifaceted approach. Weight loss is the best treatment option, which can be a challenging task for patients to achieve and maintain. Medications that decrease insulin needs like metformin, thiazolidinediones, or pramlintide may h

[Primary hyperparathyroidism--choice of surgical procedures and follow up after surgery].

Surgery for primary hyperparathyroidism (PHPT) is usually one of the most gratifying of all operations; however, at times, it can be one of the most frustrating. Parathyroidectomy has been practiced for about 70 years. During that time, a great deal of knowledge about the anatomy, embryology, pathology, and physiology of this disease has been acquired. Difficulties in parathyroidectomy for PHPT are due to the variability in the number of parathyroid glands, the different locations of normal and pathologic glands from others that are subtly diseased. To be successful, a parathyroid surgeon must

Diagnostic dilemma of an unsuspected hyperfunctioning accessory parathyroid gland after total parathyroidectomy with autotransplantation in a peritoneal dialysis patient.

Persistent or recurrent hyperparathyroidism in patients with chronic renal failure may be a frustrating problem. We report a case history of a peritoneal dialysis patient who underwent total parathyroidectomy with autotransplantation for secondary hyperparathyroidism, developed tertiary hyperparathyroidism, and in an attempt to control hypercalcemia underwent seven partial resections of the autotransplant. Subsequently, a total excision of the parathyroid autograft was performed, but the patient continued to have hyperparathyroidism and unexpectedly was found to have a hyperplastic fifth parat
