Clinical, psychological, and physiological correlates of patients who defecate after meal.

BACKGROUND AND AIMS: Food is the most important synchronizer of gastrointestinal motility and secretion. Many patients with functional bowel or anorectal disorders complain of fecal urge and stool output after eating. PATIENTS AND METHODS: In this prospective observational study, 408 consecutive outpatients with functional bowel and/or anorectal disorders (74% female, 50.2±15.6 years, 24.8±5.0 kg/m²) filled Rome III questionnaires. Depression and anxiety scores, a physiological evaluation (total and segmental colonic transit time, colonic transit response to eating using a standard 1000 kcal

Effects of electroacupuncture at ST25 and BL25 in a Sennae-induced rat model of diarrhoea-predominant irritable bowel syndrome.

BACKGROUND: Electroacupuncture (EA) may have a role in the treatment of diarrhoea symptoms. Serotonin (5-hydroxytryptamine, 5-HT) is an important neurotransmitter and paracrine signalling molecule in the gastrointestinal (GI) tract, which initiates peristaltic, secretory, vasodilatory, vagal and nociceptive reflexes. In addition, according to the results of our previous report, EA stimulation mediates GI peristalsis by increasing expression of 5-HT and tryptophan hydroxylase (TPH). AIM: To investigate the effect of EA at acupuncture points ST25 and BL25 in a rat model of diarrhoea. METHODS:

Review article: biomarkers and personalised therapy in functional lower gastrointestinal disorders.

BACKGROUND: Treatment of IBS and functional lower gastrointestinal disorders is still based predominantly on symptoms; biomarkers that reflect the mechanism or pathophysiology have been identified. Given the diverse mechanisms that result in the same clinical phenotype of IBS, it is hypothesised that identification of biomarkers may lead to individualisation of medical therapy. AIM: To review the biomarkers that have been appraised in IBS. METHODS: A single author reviewed the published literature on biomarkers appraised in IBS. RESULTS: The current literature suggests that these biomarkers

Preclinical studies of opioids and opioid antagonists on gastrointestinal function.

Opioid receptors in the gastrointestinal (GI) tract mediate the effects of endogenous opioid peptides and exogenously administered opioid analgesics, on a variety of physiological functions associated with motility, secretion and visceral pain. The studies reviewed or reported here describe a range of in vivo activities of opioid receptor antagonists upon GI function in rodents, focusing on mu receptors. Naloxone, and the peripherally acting mu-opioid receptor antagonists alvimopan and methylnaltrexone, reverse morphine-induced inhibition of GI transit in mice and rats, and morphine- or lopera

The Role of Food Structure on Fatty Acid Bioaccessibility: A Decade of TIM-1 Simulated Digestion Studies in Review.

Food structure has emerged as a critical concept with wide-ranging implications for nutrition and health. Studies with in vitro digestion models are enabling a better fundamental understanding of the structure-function relationships that define how food matrix properties change and influence nutrient release and transit during gastrointestinal digestion. Dietary lipids are of particular relevance due to their high energy density and associations with cardiometabolic risk, including postprandial rises in blood lipids. Studies using the dynamic TIM-1 digestion model enable investigations of fatt

Leukocyte Esterase-Activated Nanoconjugates Enables Precise Local Therapy of Ulcerative Colitis via Inflamed Tissue-Selective Drug Delivery.

Leukocyte esterase (LE), markedly upregulated in inflamed colonic tissues, offers a unique enzymatic trigger for selective drug activation in ulcerative colitis (UC). To exploit this pathological hallmark, we developed LE-activated nanoconjugates that enable inflamed tissue-selective drug delivery as a strategy to achieve precise local therapy for UC. Dexamethasone (DEX) was covalently conjugated to poly(lactide-co-glycolide) (PLGA) via ester bonds to form nanoconjugates (DPNCs) with suppressed drug release during gastrointestinal transit. These nanoconjugates accumulated in inflamed colonic t…

Enteric glutamatergic interneurons regulate intestinal motility.

The enteric nervous system (ENS) controls digestion autonomously via a complex neural network within the gut wall. Enteric neurons expressin … line and enkephalin. Knocking out the glutamate transporter VGLUT2 from enkephalin neurons disrupts gastrointestinal transit, while ex vivo optogenetic stimulation of glutamatergic neurons initiates colonic propulsive motility. Our results posit glutamatergic neurons as key interneurons that regulate intestinal motility.

Imaging Systemic Dysfunction in Parkinson's Disease.

Parkinson's disease is now widely recognized to be a multisystem disorder affecting the brain and peripheral autonomic nerves. Extensive pathology is present in both the sympathetic and parasympathetic nervous system and the intrinsic gastrointestinal plexuses in patients. Autonomic pathology and symptoms such as constipation can predate the clinical diagnosis by years or decades. Imaging studies have contributed greatly to our understanding of Parkinson's disease but focused primarily on imaging cerebral pathology. However, given the importance of understanding the nature, chronology, and fun

A mesalazine enema for ulcerative colitis.

Given by mouth, drugs based on 5-aminosalicylic acid (5-ASA) are often effective in inducing or maintaining remission in patients with ulcerative colitis. All of them depend on delaying the release of 5-ASA, so minimising its absorption in the small intestine and maximising delivery to the colon. With sulphasalazine (a chemical combination of 5-ASA and sulphapyridine) and olsalazine (a combination of two molecules of 5-ASA) cleavage of the azo bond by bacteria releases free 5-ASA in the colon. Oral formulations of mesalazine (the approved name for 5-ASA when given alone as a drug) use various

Octreotide in dumping and short bowel syndromes.

Octreotide inhibits intestinal motility and reduces gastrointestinal secretions. These actions have led to its evaluation in two postsurgical conditions: dumping syndrome and short bowel syndrome. Octreotide substantially reduces symptoms of early and late dumping and prevents the associated phenomena including the increase in packed cell volume considered to be indicative of reduced plasma volume. Its therapeutic benefit probably relates both to slowing of gastric emptying and small bowel transit and inhibition of the release of putative mediators (peptide hormones) of the vasomotor symptoms.

[Diagnosis of Chronic Constipation].

Patients with chronic constipation (CC) usually complain of mild to severe symptoms, including hard or lumpy stools, straining, a sense of incomplete evacuation after a bowel movement, a feeling of anorectal blockage, the need for digital maneuver to assist defecation, or reduced stool frequency. In clinical practice, healthcare providers need to check for 'alarm features' indicative of a colonic malignancy, such as bloody stools, anemia, unexplained weight loss, or new-onset symptoms after 50 years of age. In the Seoul Consensus on the diagnosis and treatment of chronic constipation, the Bris

Density map estimation with convolutional neural networks to count radiopaque markers on colonic transit studies.

BACKGROUND: A radiopaque marker study measures colonic transit time for work-up of primary constipation. It requires the patient to ingest multiple tiny radiopaque markers, which the radiologist must count manually on follow-up abdominal radiographs. Counting these markers is tedious but cognitively simple. OBJECTIVE: To develop a convolutional neural network (CNN) capable of counting the number of radiopaque markers on abdominal radiographs. MATERIALS AND METHODS: The image archive at a large tertiary children's hospital was searched to identify abdominal radiographs performed in children f

Surgical strategies in short bowel syndrome.

Extensive loss of small bowel in all age groups has significant morbidity and mortality consequences. Despite the astonishing ability of the small bowel to compensate for an extensive loss, long-term parenteral nutrition and enteral nutrition, tailored to the need of the patients in relation to the missing intestinal regions is needed. Organ-preserving surgical intervention becomes necessary in patients with a very short intestinal transit time and in an other group of patients with impaired propulsive peristalsis. Intestinal transplantation is indicated in recurrent septical infections or if

Does lack of glottic closure affect quality of life as a cause of constipation in laryngectomized patients?

The permanent disappearance of glottic closure eventually causes inadequate defecatory propulsion resulting in functional constipation. The aim of this study is to reveal functional constipation probably induced by lack of glottic closure in laryngectomized patients. The study was conducted in a prospective, tertiary care center. Forty patients who had undergone total laryngectomy (study group) and 30 who had undergone microlaryngoscopic surgery (control group) were evaluated. Based on the European Organisation for Research and Treatment of Cancer, Quality of Life Questionnaires C30, the prese
