Histocompatibility antigens in polymyalgia rheumatica and giant cell arteritis.

Fifty-five Caucasoid patients with polymyalgia rheumatica or giant cell arteritis were typed for HLA, A, B, C and DR locus antigens. In the total patient group, DR4 had a significantly increased frequency. The prevalence of CW3 and CW6 was also significantly increased, the rise of CW3 possibly being attributable to its well-documented linkage disequilibrium with DR4. The rise in CW6 cannot be attributed to its known linkage disequilibrium with DR7.

[Giant cell arteritis: temporal arteritis, rheumatic polymyalgia].

Fourteen patients have been diagnosed for Giant Cell Arteritis by temporal biopsy or clinical criteria in the last five years. Two of them started with atypical symptoms. Polymyalgia Rheumatica was the commonest symptom (71%). Two patients (14%) had an irreversible blindness in eye one. Histologic changes of arteritis were shown by temporal biopsy in 8 cases, one of which had no cranial manifestations. A raised ESR was a constant finding (100%) followed by a alpha-2 globulin increase (93%) and anaemia (57%). One patient had rare complication of this disease: an aortic arch syndrome. The whole

Polymyalgia rheumatica and giant cell arteritis--a difficult diagnosis.

This five-year study of 108 patients with giant cell arteritis and/or polymyalgia rheumatica drawn from all departments of a district general hospital emphasizes the difficulties of diagnosis. A correct diagnosis was made by the referring doctor in 33 per cent of patients and on initial attendance at hospital in 67 per cent of patients. Symptoms were present for more than three months before referral to hospital in 39 per cent of patients, and the delay before diagnosis at hospital was greater than one month in 20 per cent. Systemic illness (present in 83 per cent of cases), anaemia (33 per ce

[Visceral giant cell arteritis similar to polyarteritis nodosa].

Biopsy from the small and large intestines of a 60-year old female patient with intestinal tract ulcerations helped to reveal angiitis similar to polyarteritis nodosa but with progressive proliferation of giant multinuclear cells in the inflammatory infiltrate rich in eosinophils. As much as 20-31% eosinophils were found in peripheral blood. Postmortem examination also confirmed the presence of angiitis in the kidneys, liver and thalamus. The position of arteritis between polyarteritis nodosa and visceral giant cell arteritis is discussed.

Coronary ischaemia and occlusion in giant cell (temporal) arteritis.

Reports of coronary artery involvement in giant cell (temporal) arteritis--polymyalgia rheumatica (GC(T)A--PMR) together with other large arteries arising from the aorta have been numerous over the past 40 years, but on this the specialist cardiac literature has been virtually silent. This article summarises that evidence, and records nine additional patients from a large group of cases with both GC(T)A--PMR and ischaemic heart disease (IHD) observed since a previous report in 1960. The case histories illustrate the benefit from corticosteroids and the hazards of non-diagnosis and premature ce

Measles giant cell pneumonia in childhood leukemia in remission.

A four-year old girl had acute lymphoblastic leukemia in remission with immunosupressive chemotherapy and complicated giant cell pneumonia. Investigations by light and electron microscopy and immunoperoxidase study of autopsy material disclosed measles virus nature of the penumonia, although there was no typical rash of the disease. Similar giant cells were seen in the gastric and colon mucosa, and the other of Warthin-Finkeldey type in the spleen, and lymph nodes of pulmonary hiluses. To the best of our knowledge, this is the first discription of gastric giant cells with inclusion of measles …

Giant cell arteritis (cranial arteritis, polymyalgia rheumatica).

Giant cell arteritis, which is probably due to disturbed immune mechanisms, has a spectrum of clinical symptoms in elderly people. In nearly all cases such general signs as loss of appetite, loss of weight and fever are present. The sedimentation rate is almost without exception about 100 mm in the first hour. The two most frequent and typical clinical syndromes are polymyalgia rheumatica and cranial arteritis. The polymyalgia rheumatica is characterized by periarticular pain which is mostly symmetrical and accentuated in the shoulder girdle. Increasingly severe temporal headache and ocular di

Supervoltage irradiation in the management of giant cell tumor of bone.

Thirteen patients with giant cell tumor of bone were treated with megavoltage irradiation from 1958 to 1972. Nine cases were believed to be true giant cell tumors of bone. The only patient with recurrence following a geographic miss was salvaged surgically. Curative therapy for giant cell tumor of bone may be 3500 rads in 15 fractions in 3 weeks; it is well tolerated with minimal morbidity. Special care must be used in interpretation of the post-therapy radiologic appearance of these lesions because an initial period of osteolysis lasts many weeks before any radiographic indication of healing

[Giant cell granulomatous angiitis of the central nervous system].

The case is described of a coloured woman of 58, previously affected by filaria, who presented with meningo-encephalitis which proved fatal within a month. Pathological anatomy showed granylomatous angiitis with giant cells scattered throughout the brain and the meninges but confined exclusively to the nervous system. Biological and anatomopathological tests were unable to connect this condition with any precise aetiology. The aetiology of granulomatous angiitis is discussed in this connection and the clinical and anatomopathological charateristics peculiar to giant cell granulomatous angiitis…

Giant cell lesion of the mandible in coincidental hyperparathyroidism and hyperthyroidism.

A patient with hyperparathyroidism, thyrotoxicosis and a giant cell lesion of the mandible is reported. The hyperparathyroidism was secondary to renal hypoplasia and/or thyrotoxicosis. A review of the literature revealed other cases with giant cell lesions and secondary hyperparathyroidism. This study indicates that giant cell lesions are not specific for primary hyperparathyroidism.
