Proximal interphalangeal joint fractures of the hand: treatment with an external dynamic traction device.

The authors present a series of 11 consecutive patients who sustained an intra-articular fracture of the proximal interphalangeal (PIP) joint and 1 patient with a fracture of the interphalangeal joint of the thumb treated with a Kirschner wire external dynamic traction device. Range of motion and grip strength were measured. We used the Michigan Hand Outcome Questionnaire (MHQ) to obtain a subjective response to the treatment. Average range of motion of the PIP joint was 64 degrees and distal interphalangeal joint was 52 degrees . The average grip strength was 86% with a mean MHQ score of 90.

Bone health of children with intestinal failure measured by dual energy X-ray absorptiometry and digital X-ray radiogrammetry.

BACKGROUND & AIMS: Children with intestinal failure (IF) receiving long-term parenteral nutrition (PN) are at risk of developing low bone mineral density (BMD). Next to the dual energy X-ray absorptiometry (DXA) method, digital X-ray radiogrammetry (DXR) using the BoneXpert software has become available to obtain the Bone Health Index (BHI) in hand radiographs. In this study we 1) evaluated the prevalence of low BMD in children with IF using DXA and DXR, 2) compared DXA and DXR results, and 3) aimed to identify factors associated with low BMD. METHODS: A retrospective study was performed incl

A Strong Supporter: Evidence for the Role of the Fifth Finger in Habitual Gripping Activity.

OBJECTIVES: The fifth finger plays a key role in manual dexterity, yet its habitual use and functional integration within the hand remain poorly understood. This study investigates the contribution of the fifth ray to habitual gripping activities and its synergistic relationship with the thumb. MATERIALS AND METHODS: The "Validated Entheses-based Reconstruction of Activity" (VERA) method and multivariate statistical analyses were applied to the hand bones of 43 adult male/probable male individuals from three post-Medieval skeletal collections in the Netherlands. Principal component and pairwi…

Clinical Characterization of Skin Findings on the Hands in Proteus Syndrome.

BACKGROUND: Proteus syndrome is a progressive asymmetric overgrowth disorder caused by mosaic activating variants in AKT1. It most frequently affects the skin, soft tissues, bones, and central nervous system and increases the risk for certain tumors and venous thromboembolism. A hallmark feature is the progressive plantar cerebriform connective tissue nevus (CCTN). The natural history of skin manifestations of the hand in Proteus syndrome is less well characterized. METHODS: A retrospective review of photography collected from 2000 through 2024 was conducted as part of an ongoing natural hist

Current Concepts in Radial Club Hand.

Radial club hand is a complex congenital abnormality of the radial or pre-axial border of the upper extremity. It has a wide range of phenotypes from hypoplasia of the thumb to complete absence of the radius and the first ray. Centralization with tendon transfer is a popular method for maintaining the correct position of radial club hand. On the other hand, various corrections were devised, e.g. radialization after distraction to emphasize the fact that the head of the ulna is positioned under the radial carpal bones and is no longer placed in a slot in the center of the carpus, microvascular

Frequency of radiographic damage and progression in individual joints in children with juvenile idiopathic arthritis.

OBJECTIVE: To evaluate the presence and progression of radiographic joint damage, as assessed with the adapted Sharp/van der Heijde score (SHS), in individual joints in the hand and wrist in patients with juvenile idiopathic arthritis (JIA) and to compare progression of damage among different JIA categories. METHODS: A total of 372 radiographs of both wrists and hands obtained at first observation and at last followup visit (after 1-10 years) in 186 children with polyarticular-course JIA were evaluated. All radiographs were scored using the adapted SHS by 2 independent readers. Radiographic a

Functional reconstruction in large and complex soft tissue defects of forearm and hand with multifunctional anterolateral thigh flap.

OBJECTIVES: The aim of this study was to evaluate the results of free anterolateral thigh (ALT) flap reconstruction for large and complex soft tissue defects of forearm and hand. PATIENTS AND METHODS: Thirteen patients (11 males, 2 females; mean age 32.5 years; range 18 to 55 years) underwent free ALT flap reconstruction for forearm and hand defects. The etiology of defects was crush injury in nine patients, tumor excision in two patients, electric burn in one patient, and unstable scar excision in one patient. In five patients a flow through ALT flap was used to reconstruct vascular defects.

Intermediate bone grafting of alveolar clefts.

During the five year period 1980 to 1985 bone grafting was performed in 37 cleft patients before the eruption of the canine tooth (group IBG) and in 30 patients after the eruption of the canine tooth (group SGB). The initial healing was more favourable in the IBG group. In 72.5% of the clefts oro-nasal fistulae were present preoperatively. In all cases the fistulae were successfully closed. In the IBG group with orthodontic closure of the gap in the dental arch the interdental bone height in the grafted area was more than 75% of normal bone height in all clefts. In group SBG this situation was

Clinical features and treatment of congenital fusion of the small and ring finger metacarpals.

Nine cases of congenital fusion of the small and ring finger metacarpals are reported. The underlying etiology of congenital fusion of the small and ring finger metacarpals was ulnar ray deficiency in four hands, cleft hand in two hands, and symbrachydactyly in one hand; two hands were unclassifiable. X-ray films revealed that the small finger was hypoplastic and accompanied by brachymesophalangy in all cases. It was supposed that the main cause of congenital metacarpal fusion was the deficit of mesenchymal cells. Treatment for metacarpal fusion included splitting the metacarpal fusion and aut

Metastatic gastric cancer presenting with shoulder-hand syndrome: a case report.

INTRODUCTION: Shoulder-hand syndrome is a relatively rare clinical entity classified as a complex regional pain syndrome type 1 and consisting essentially of a painful 'frozen shoulder' with disability, swelling, vasomotor or dystrophic changes in the homolateral hand. The pathophysiology is not completely clear but a predominant 'sympathetic' factor affecting the neural and vascular supply to the affected parts seems to be involved. Shoulder-hand syndrome has been related to many surgical, orthopedic, neurological and medical conditions; it is more often seen after myocardial infarction, hemi

Diffuse venous malformation with intraosseous involvement.

A young boy with a large venous malformation of his left arm and hand was evaluated for Maffucci's syndrome. Initial hand films were remarkable for cystic bony lesions suggestive of enchondromas. Additional roentgenograms and magnetic resonance imaging studies showed the bony abnormalities were the result of a venous malformation infiltrating bone. Magnetic resonance imaging is a useful diagnostic tool in differentiating enchondromas from vascular anomalies. Venous malformations infiltrating bone should be included in the differential diagnosis of Maffucci's syndrome.

[Skeletal distribution and biological markers of Paget's disease].

The authors have re-examined the skeletal distribution of Paget's disease of bone by means of radionuclide bone scanning performed in 170 untreated pagetic subjects (93 men, 77 women). 863 high uptake foci were detected. The finding that 8.3 p. 100 of the sites revealed by scanning were not visible on X-ray films shows that the disease is underdetected by standard radiography. Peripheral foci in the feet and hands were discovered in more than a quarter of the patients. There was a significant positive correlation between skeletal extension index on the one hand and plasma alkaline phosphatase

Nonunion of the clavicle treated with plate fixation: a review of forty-seven consecutive cases.

Forty-seven consecutive patients underwent superior plating for clavicular nonunion between 1995 and 2005. This retrospective analysis was performed to assess the efficacy of plate fixation and the need for iliac crest bone grafting. Forty-three patients were treated with pelvic reconstruction plates and four with straight dynamic compression plates. Of the patients, 30 were treated with local bone graft only, 14 with demineralized bone matrix, and 3 with distant autogenous bone graft. Of the fractures, 93% united after a single operation. Of the patients, 69% returned the Disabilities of the

Clinical Outcomes of Severely Angulated Fifth Metacarpal Neck Fractures Treated Nonsurgically.

BACKGROUND: Reasonable functional outcomes for nonoperative management of isolated, closed fifth metacarpal neck fractures with up to 70° angulation have been reported; however, reported outcomes for fractures with greater than 70° angulation are limited. This study describes clinical outcomes of nonsurgically treated fifth metacarpal neck fractures with angulation of greater than 70°. METHODS: A retrospective review of patients treated between May 1, 2016, and May 1, 2020, included: (1) patients aged 18 years and above with an isolated, closed, fifth metacarpal neck fracture; (2) nonsurgical

Nonvascularized Iliac Crest Bone Graft for Reconstruction of the First Metacarpal in Type IIIB Thumb Hypoplasia: A Radiographic Follow-Up Study.

PURPOSE: To present a radiographic follow-up study of the use of a nonvascularized iliac crest bone graft as a treatment for modified Blauth type IIIB thumb hypoplasia. METHODS: From January 2015 to December 2019, nonvascularized iliac crest bone grafts were used to reconstruct the first metacarpal in 23 cases with type IIIB thumbs. The average follow-up duration was 1.9 years (range 1.0-3.9 years). We evaluated the patients' serial x-rays and measured the width and length changes of the reconstructed first metacarpals. RESULTS: Survival of the graft, judged radiographically, was achieved in

Ring finger metacarpal fracture iatrogenic rotation using an ulnar gutter splint: A cadaveric study.

STUDY DESIGN: Cohort cadaveric study. INTRODUCTION: Ring finger metacarpal fractures are often treated with ulnar gutter orthoses incorporating the ring and small fingers. Iatrogenic pronation of the distal metacarpal fragment may occur from overzealous orthotic "molding", resulting in a crossover deformity of the ring finger over the small finger. PURPOSE OF THE STUDY: The goal of this cadaveric study is to determine whether including the middle finger in an ulnar gutter orthotic could lessen the chances of iatrogenic ring finger metacarpal fracture rotation. METHODS: Transverse ring finge

Complete pseudoepiphyses with associated enhanced growth in hands and feet: a report of 2 siblings--case report.

We present 2 siblings with multiple complete pseudoepiphyses in their hands and feet with associated symptomatic enhanced growth. Physical examination of the 6-year-old boy revealed long slender fingers and hyperplastic great toes. Radiography showed complete pseudoepiphyses in the first metacarpals, proximal and middle phalanges of the hands, and proximal phalanges of the feet. The patient's younger brother had a similar phenotype with slightly milder functional complaints. Genetic analysis did not reveal an underlying syndrome in these siblings.
