Atrial septal defect with anomalous coronary: not always a contraindication for device closure.

The co-existence of an ostium secundum atrial septal defect (OS-ASD) and an anomalous circumflex (LCX) artery is rare, with limited literature addressing the device closure in such cases. A middle-aged woman presented to us with dyspnoea on effort which had progressively worsened from New York Heart Association (NYHA) class I to II over 2 years and evaluation revealed a 16 mm OS-ASD along with an anomalous LCX arising from the right coronary sinus with a retro-aortic course. We report this rare case of atrial septal defect (ASD) device closure in a patient with anatomical combination of ASD an

Catheter-based closure of a large atrial septal defect with inferior rim deficiency using pulmonary vein slide-out assisted implantation technique: a case report.

BACKGROUND: Transcatheter approach for large and complex atrial septal defects may represent a therapeutic challenge, particularly when the postero-inferior rim is deficient and floppy. CASE SUMMARY: Here, we describe a successful catheter-based closure of a large (>30 mm) secundum atrial septal defect associated with postero-inferior rim deficiency in a 35-year-old female with congestive heart failure using pulmonary vein slide-out assisted implantation technique. DISCUSSION: Inferior-posterior rim deficiency is a well-known risk factor for device instability or embolization. Transcatheter

Late Erosion of Percutaneous ASD Occlusion Device.

UNLABELLED: Percutaneous closure of atrial septal defects (ASDs) is an ever more common treatment modality in selected patients. Device erosion is a rare but often fatal complication of this therapy, which usually presents early in the post-procedure course. We describe the case of a patient presenting with chest pain and shock in whom early recognition and intervention prevented a negative outcome. Awareness of this potentially lethal complication is important as the population of patients undergoing percutaneous procedures for structural heart disease continues to grow. LEARNING POINTS: As

Familial ventricular aneurysms and septal defects map to chromosome 10p15.

AIMS: Although ventricular septal defects (VSD) are the most common congenital heart lesion, familial clustering has been described only in rare instances. The aim of this study was to identify genetic factors and chromosomal regions contributing to VSD. METHODS AND RESULTS: A unique, large kindred segregating various forms of septal pathologies-including VSD, ventricular septal aneurysms, and atrial septal defects (ASD)-was ascertained and characterized clinically and genetically. Eighteen family members in three generations could be studied, out of whom 10 are affected (2 ASD, 3 septal aneu

Late incompetence of the left atrioventricular valve after repair of atrioventricular septal defects: the morphologic perspective.

OBJECTIVE: The mortality following repair of atrioventricular septal defects has fallen dramatically in the last 4 decades, but reoperation for late regurgitation across the left atrioventricular valve has remained disconcertingly stagnant. Seeking potential structural causes, we compared the morphology of the surgically created septal leaflet of the left valve following repair of atrioventricular septal defects to the aortic leaflet of the normal mitral valve. METHODS: We compared the mitral valves of 92 normal hearts to the left ventricular components of the bridging leaflets of hearts with

Twin heart with a fused atria and separate ventricles in conjoined twins.

One of the most interesting congenital malformations is that of conjoined twins. We report echocardiographic features of twin heart in dicephalus, tribrachius, dispinous, thoracoomphalopagus twin. It showed two hearts fused at atrial level. Right-sided heart had single atrial chamber with a single ventricle. A single great vessel, aorta, originated from it. Left-sided heart was well developed with two atria and two ventricles. There was a small mid muscular ventricular septal defect and a small patent ductus arteriosus. Great arteries had normal origins.

A single bicaval venous cannula used for optimization of endoscopic robotically assisted procedures of the heart.

Robotically assisted heart operation is one of the minimally invasive approaches that have revolutionized many surgical procedures. Robotic procedures on the heart, however, are by no means routine, and their optimization requires development of new instruments and techniques. Described here is the robotically assisted use in a sheep model of a new peripherally inserted bicaval venous cannula in the totally endoscopic closure of atrial septal defect. The exposure of the surgical target area was successful with no obstructions in the surgical view. The cannula shows sufficient flow characterist

Major adverse cardiovascular events in adult congenital heart disease: a population-based follow-up study from Taiwan.

BACKGROUND: The aim of the present study was to identify the long-term major adverse cardiovascular events (MACE) in adult congenital heart disease (ConHD) patients in Taiwan. METHODS: From the National Health Insurance Research Database (1997-2010), adult patients (≥18 years) with ConHD were identified and compared to non-ConHD control patients. The primary end point was the incidence of MACE. Cox proportional hazards models were used to compute hazard ratios as estimates for multivariate adjusted relative risks with or without adjusting for age and sex. RESULTS: A total of 3,267 adult pati

2013 College of Diplomates of the American Board of Pediatric Dentistry Annual Meeting: pediatric cardiology: an overview.

Congenital heart defects can be classified as those characterized by a left to right shunt, a right to left shunt, obstruction to flow from the heart, or abnormalcies of myocardial contraction and relaxation. Left to right shunts include atrial and ventricular septal defects, and patent ductus arteriosus. Right to left shunts include transposition of the great arteries, tetralogy of Fallot, tricuspid atresia among others. These defects result in cyanosis. Lesions causing obstruction to blood flow from the heart include aortic and pulmonary valve stenosis and coarctation of the aorta. Disorders

Breaking the Barriers: Transseptal Puncture in Patients with Atrial Septal Defect Closure Devices.

Atrial septal defect and patent foramen ovale are common congenital heart defects in adults, often treated with atrial septal occluders (ASOs) via percutaneous closure. Transseptal puncture (TSP) for left atrial interventions in patients with ASOs presents procedural challenges, requiring detailed pre-procedural planning and imaging. Transoesophageal echocardiography and cardiac CT help identify optimal puncture sites, either through the native septum or across the ASO. Advanced techniques, such as balloon-assisted tracking and radiofrequency energy delivery, aid in navigating the device. TSP

Teratogenic effect of acetohydroxamic acid in clinically normal beagles.

Acetohydroxamic acid (AHA), a potent urease inhibitor used for treatment of infection-induced struvite urolithiasis, was teratogenic after administration of 25 mg of AHA/kg of body weight/day orally to 5 clinically normal Beagles from the onset of proestrus until parturition. Thirty pups exposed to AHA in utero developed anomalies of the skeletal system, heart, and ventral midline. Cardiac anomalies included atrial septal defects (20%), ventricular septal defects (3%), and atrial and ventricular septal defects (3%). Skeletal anomalies included coccygeal hemivertebrae and fused coccygeal verteb

[Histopathological analysis of neonatal mouse hearts with connexin43 gene defects].

OBJECTIVE: To investigate the characteristics of heart morphology in neonatal mice with connexin43 gene defects. METHODS: Two mouse lines were used in this study, which included connexin43 knockout (Cx43KO) mice and CMV43(CT) transgenic mice. PCR analysis was carried out to identify the genotypes of two transgenic mice. Sections with HE staining were analyzed to display the morphologic structures of the hearts in neonatal mice. C57BL6/SJ mice were used as control. RESULTS: All 11 homozygous Cx43KO mice died within 24 hr after birth showing severe right ventricular outflow tract obstruction (

[Atrial septal defect with pulmonary hypertension in infancy (author's transl)].

Four cases of atrial septal defect, "ostium secundum", associated with pulmonary hypertension and congestive heart failure in children under the age of two years are reported. A correct diagnosis could be made before surgery in only two of the case. In one case partial anomalous pulmonary venous connection was incorrectly diagnosed, while ventricular septal defect could not be excluded in the fourth case. The systolic pressure in the pulmonary artery was higher than 50 mm. Hg in all cases. One patient died after catheterization, and total surgical correction was performed in the other three ca

Endovascular closure of MINOCA-caused ventricular septal defect (VSD).

Myocardial infarction with non-obstructive coronary arteries (MINOCA) is a rare form of acute myocardial infarction and it is an even rarer cause of ventricular septal defect (VSD). We present a case of successful endovascular closure of post-MINOCA VSD with the use of an atrial septal defect (ASD) occluder. The patient improved from intra-aortic balloon pump dependent to New York Heart Association (NYHA) I in a matter of days. 18-month follow-up period is event free and symptom free.

[Lower mid-line skin incision and minimal sternotomy in pediatric open heart surgery--an alternative cosmetic approach].

Full median sternotomy has been the standard approach for open heart surgery. However, it leaves an unsightly surgical scar. We therefore designed a lower mid-line skin incision and minimal sternotomy approach as a cosmetic alternative. Since February 1993, 78 pediatric patients with congenital heart disease have undergone this open heart surgery procedure. Their ages ranged from 1 month to 13 years (median: 2 years and 7 months) and body weights ranged from 2.4 to 43 kg (median: 11.5 kg). Thirty-one patients had atrial septal defect, 34 perimembranous ventricular septal defect, 9 subpulmonary

RXR alpha deficiency confers genetic susceptibility for aortic sac, conotruncal, atrioventricular cushion, and ventricular muscle defects in mice.

Retinoid-dependent pathways play a central role in regulating cardiac morphogenesis. Recently, we characterized gene-targeted RXR alpha -/- embryos, which display an atrial-like ventricular phenotype with the development of heart failure and lethality at embryonic day 14.5. To quantitate the frequency and complexity of cardiac morphogenic defects, we now use microdissection and scanning electron microscopy to examine 107 wild-type, heterozygous, and homozygous embryos at embryonic day 13.5, 14.5, and 15.5. RXR alpha -/- embryos display complex defects, including ventricular septal, atrioventri
