The prevalence of valvular infective endocarditis (IE) is increasing and is burdened by high mortality and morbidity. Despite the higher risk, the surgical approach is superior to medical therapy alone, and over the years there has been a more aggressive attitude, with earlier indications for surgery. This article aims to review the available literature and the American and European guidelines in order to summarize the most appropriate surgical timing for valvular IE. Although there are discrepancies between the guidelines, an emergent indication (<48 h) should be considered in patients with e

Prosthetic valve replacement surgery is performed in approximately 210,000 patients annually worldwide. At least 3%, and possibly as many as 12.5%, of these valves will eventually demonstrate a clinically important paravalvular leak. Most of the significantly symptomatic leaks are currently repaired surgically, but the risks of repeat operation and re leak are significantly higher than the original valve-replacement surgical risks. Transcatheter closure of paravalvular leaks has been accomplished using a number of different devices in a relatively small number of centers over the last 16 years

Over a period of 10 years, 25 patients underwent reoperation for prosthetic valve endocarditis. The basic procedure for surgical treatment differed depending on the presence or absence of annular ring abscess. Standard valve replacement was employed in 15 patients without annular ring abscess (three aortic, eleven mitral and one tricuspid). The ten other patients who had had partial destruction of the annulus underwent complex surgical treatment (six aortic, three mitral and one aortic and mitral). Complex operative techniques consisted of three different procedures, depending on the anatomica

OBJECTIVE: The treatment strategy for aortic stenosis (AS) has been changing due to newly developed valvular prostheses and trans-catheter aortic valve implantation (TAVI). To determine the role of new modalities for AS with a small aortic root, papers using the concept of prosthesis-patient mismatch (PPM) were reviewed.
METHODS: First, to determine the cut-off value of the indexed effective orifice area (IEOA) for defining PPM, the studies of surgical aortic valve replacement (SAVR) with a follow-up longer than 5 years and a patient number larger than 500 were reviewed. Second, the papers co

Mucopolysaccaridosis syndromes are metabolic disorders that are inherited in an autosomal recessive pattern, and demonstrate cardiovascular involvement that, on rare occasions, results in surgery. The case is presented of a 12-year-old boy with symptoms and signs of congestive heart failure who was referred to the authors' department for severe mitral and aortic valvular stenosis. The patient was shown to have the somatic characteristics of mucopolysaccharidosis, while urinalysis indicated the presence of chondroitin sulfate B. Mitral and aortic valve replacements were carried out, using bilea

BACKGROUND: The effective orifice area (EOA) is utilized to characterize the hemodynamic performance of the transcatheter heart valve (THV). However, there is no consensus on EOA measurement of self-expanding THV. We aimed to compare two echocardiographic methods for EOA measurement following transcatheter self-expanding aortic valve implantation.
METHODS: EOA was calculated according to the continuity equation. Two methods were constructed. In Method 1 and Method 2, the left ventricular outflow tract diameter (LVOTd) was measured at the entry of the prosthesis (from trailing-to-leading edge)

The optimal treatment of congenital aortic valve lesions is a controversial issue. This study was performed to evaluate the outcome after surgical treatment of aortic valve lesions in congenital aortic valve disease. Between the years of 2000 and 2008, 61 patients (mean age: 12.6 +/- 9.6 years, range: 1 day to 40 years) underwent aortic valve surgery for congenital aortic valve disease. Twenty-four patients had undergone previous cardiovascular operations. Indications for surgery were aortic regurgitation in 14.7% (n = 9), aortic stenoses in 26.2% (n = 16), and mixed disease in 59.1% (n = 36).

Efficient function and long-term durability without the need for anticoagulation, coupled with the ability to be accommodated in many different types of patient, are the principal requirements of replacement heart valves. Although the clinical use of valves appeared to have remained steady for several decades, the evolving demands for the elderly and frail patients typically encountered in the developed world, and the needs of much younger and poorer rheumatic heart disease patients in the developing world have now necessitated new paradigms for heart valve technologies and associated material

Redo surgical aortic valve replacement should be favored over transcatheter aortic valve replacement, especially in patients who have prosthetic aortic valves that cannot be fractured, in whom implantation of a small transcatheter aortic valve replacement would lead to patient-prosthetic mismatch, adversely affecting the quality of life.

Valve-in-valve transcatheter aortic valve implantation (ViV TAVI) is rapidly arising as a safe and effective alternative to redo-surgery in the treatment of bioprostheses deterioration. While scientific community is currently focusing its attention on the most common limitations related to this procedure, such as the risk of coronary obstruction and patient-prosthesis mismatch, data regarding the first step of a ViV TAVI, the crossing of a degenerated bioprosthesis, are still lacking. The aim of this review is to analyze the available information about bioprosthesis crossing, to show the inher

Transcatheter valve implantation in patients with aortic stenosis has evolved as an acceptable alternative to surgical aortic valve replacement in a subset of patients at excessively high risk from surgery. The structural valve degeneration (SVD) is a known phenomenon in catheter-based valves too, which was hitherto seen with surgical bioprosthetic valves. Echocardiography plays a pivotal role not only in early detection but also in the management of SVD of Transcatheter valves. The goal of this report is to agglomerate our experience of an unusual case of SVD of a catheter-based valve implant

PURPOSE OF REVIEW: Repeat surgery for failed mitral valve prostheses and repairs are fraught with high rates of morbidity and mortality. Therefore, clinicians have evolved transcatheter technology as an alternative therapy. This review serves as an update as the field has moved out of the early learning curve of treating postsurgical mitral valve failures.
RECENT FINDINGS: Mitral valve-in-valve procedures have higher rates of technical success and better in-hospital and 1-year mortality rates than mitral valve-in-ring cases. The higher rates of complications, including left ventricular outflo

We report a case of transcatheter aortic valve implantation in a 79-year-old woman with a coronary anomaly who underwent surgical aortic valve replacement with a 23-mm Abbott Trifecta bioprosthesis. The procedure was performed in response to severe aortic stenosis caused by a bicuspid aortic valve. Computed tomography showed an anomalous origin of the right coronary artery from the left coronary sinus, with an interarterial course. Although the virtual transcatheter valve to coronary ostium distance-right coronary artery was short, the right coronary artery ostium was just behind the stent pos

Valve-in-valve repeat prosthetic reconstruction is a less invasive alternative to 'open' redo operation for degenerative dysfunction of an aortic bioprosthesis. At the beginning of the history of using this method it was resorted to only in cases of high surgical risk, involving virtually inoperable patients. Currently, the world experience numbers in thousands of such interventions, known to be also performed in patients not belonging to a high-risk group. Recent studies have demonstrated a decrease in the perioperative mortality and improved quality of life in the remote period after valve-i

Rheumatic mitral valve disease being common in India, valve replacement and hence thrombosis of prosthetic heart valves (PHV) are not uncommon. The diagnosis is commonly established by increased gradients across PHV during transthoracic echo and restricted leaflet movement during transesophageal echo or fluoroscopy. We demonstrated both the leaflets of a thrombosed PHV,of which one was stuck in closed position by real-time three-dimensional transesophageal echocardiography.

Despite significant advances in transcatheter tricuspid valve repair and replacement (TTVR), a substantial proportion of patients with severe tricuspid regurgitation (TR) remain ineligible for currently available therapies due to anatomical constraints, advanced disease, or prohibitive procedural risk. Heterotopic valve implantation strategies, in which prosthetic valves are positioned outside the native tricuspid annulus, have emerged as alternative approaches for this challenging population. The UNICA system represents a novel single-stent, double valve heterotopic platform designed for pati
