Minimally invasive femoral head fracture fixation using three-dimensional navigation: a technical note.

Femoral head fractures are rare traumatic injuries that are usually associated with hip dislocations. Open reduction and internal fixation are performed when indicated, but can be associated with a higher risk of avascular necrosis. We report the case of a 24-year-old patient with a Pipkin type II fracture dislocation of the femoral head fixed via a minimally invasive three-dimensional navigated internal fixation technique. This technique minimizes deep soft tissue dissection to the hip capsule and associated vascularity and allows for accurate implant positioning.Level of evidence: Therapeuti

Stress Fracture of the Sacrum.

A physical therapist evaluated a 25-year-old male military trainee, who reported 1 week of left hip pain exacerbated by running and sitting cross-legged. At follow-up, the patient reported unchanged hip pain and new left knee pain. Due to potential for multifocal stress injuries outside the visual field of a single MRI, bone scan and single-photon emission computerized tomography were chosen as imaging modalities. Imaging revealed a nondisplaced lateral left-sided sacral stress fracture and left calcaneal stress fracture. J Orthop Sports Phys Ther 2015;45(11):965. doi:10.2519/jospt.2015.0411.

The dancer's hip.

Conditions that occur in the dancer's hip fall into the following categories: poor training; conditions that occur as the result of normal use; overuse syndromes, including tendinitis and myositis; and conditions referring pain to the hip. Dancers are highly motivated and goal oriented and often suppress symptoms for long periods, making diagnosis and treatment difficult. Observing the dancer at work and understanding his art are emphasized, and a practical guide to therapy is presented. Development of proper dance technique and a proper flexibility program can decrease the incidence of injuri

A Storm, An Explosion, and Flying Rocks - An Unusual Injury due to a Lightning Strike in the Mountains.

BACKGROUND: Most injuries observed in victims of lightning strikes can be explained by electrothermal phenomena. Blast penetrating injuries caused by a lightning-strike-induced explosion of a nearby structure are rarely reported. CASE PRESENTATION: Here reported is the case of a patient with numerous mixed injuries caused by a lightning strike, including deep lacerations of both hips and thighs with rock fragments embedded in the wounds. Surgical removal of rock fragments from deep areas of the right hip and right lower leg was necessary. The cause of the formation of rock missiles was the li

Multicentre randomised controlled trial protocol in China comparing postless hip arthroscopy with conventional perineal post-assisted hip arthroscopy to reduce perineal complications in patients undergoing hip arthroscopy for femoroacetabular impingement: the POSTLESS-HIP trial.

INTRODUCTION: Perineal complications-including nerve injury (eg, pudendal neurapraxia), soft tissue injury and sexual or urinary dysfunction-are well-documented adverse effects of conventional hip arthroscopy using a perineal post. Postless hip arthroscopy avoids direct perineal pressure, but high-quality randomised evidence is lacking. METHODS AND ANALYSIS: This multicentre, prospective, parallel-group, randomised controlled trial will enrol 270 patients aged 15-60 years with femoroacetabular impingement syndrome requiring hip arthroscopy. Participants will be randomised 1:1 via a centralise

Secondary capsular laxity of the hip.

We describe a hip condition with a recognisable pattern of clinical signs and radiological findings thought to result from chronic capsular injury. Between June 2006 and October 2009, ten patients (11 hips), four men and six women, were identified with an abnormality of external rotation at the hip joint. A detailed history and clinical examination was undertaken for each patient. Dynamic magnetic resonance imaging of symptomatic and control hips were evaluated for bony and soft tissue appearances. The relative positions of the femoral head and the acetabulum were assessed through a range of h

Clinical anatomy of the musculoskeletal system in the hip region.

Although the hip joint is regarded as inherently stable, hip pain and injuries caused by traumatic/non-traumatic hip instability are relatively common in active individuals. A comprehensive understanding of hip anatomy may provide better insight into the relationships between hip stability and clinical problems. In this review, we present our recent findings on the hip morphological characteristics, especially focusing on the intramuscular tendon of the gluteus medius tendon and its insertion sites, hip capsular attachment on the anterosuperior region of the acetabular margin, and composition

Hip dislocation in a 3-year old girl after minor trauma: A rare case report.

Traumatic hip dislocations in children are rare injuries. They are mostly sustained after low-energy trauma in younger children, and after high-energy trauma in older children and adolescents. In 90 % of cases dislocation is posterior. Since this condition is an orthopedic emergency, it requires urgent reduction in order to avoid complications such as recurrent or habitual dislocation or avascular necrosis (AVN) of the femoral head. A 3-year old girl presented sustained posterior dislocation of the right hip after twisting injury. Clinically, the right leg was shortened, and in the position of

Hip Fractures in Patients with Dementia: An Emerging Orthopedic Concern.

Hip fractures are a very common injury in the elderly population associated with an increased mortality rate. Currently, more and more elderly patients are diagnosed with dementia. Demented patients are more prone to falls and hip fractures compared to the general population because of conditions related to their disease such as instability, osteoporosis, poor muscle control and weakness. The coexistence of dementia and hip fractures is a difficult situation for both the patients and the treating physicians because of postoperative complications in this frail subgroup of patients and their ina

The use of a modified posterior approach (SPAIRE) may be associated with an increase in return to pre-injury level of mobility compared to a standard lateral approach in hemiarthroplasty for displaced intracapsular hip fractures: a single-centre study of the first 285 cases over a period of 3.5 years.

BACKGROUND AND PURPOSE: A tendon-sparing modification of the posterior approach to the hip joint was introduced in the specialist hip unit at our institution in 2016. The SPAIRE technique-acronym for "Saving Piriformis And Internus, Repair of Externus" preserves the insertions of gemellus inferior, obturator internus, gemellus superior and piriformis intact. We compare the results of the first 285 hip hemiarthroplasty patients, unselected but preferentially treated by our hip unit surgeons using the SPAIRE technique, with 567 patients treated by all orthopaedic surgeons (including the hip unit

Influence of hip and knee positions on gluteus maximus and hamstrings contributions to hip extension torque production.

BACKGROUND: Diminished gluteus maximus muscle strength has been proposed to be contributory to various lower-limb injuries. As such, it is of clinical importance to perform hip extensor strength testing in a position that biases torque contribution of the gluteus maximus relative to the other hip extensors (i.e. hamstrings). OBJECTIVES: To determine the relative torque contributions of the gluteus maximus and hamstring muscles in various hip extensor strength testing positions. METHODS: 13 Young, healthy participants performed maximum isometric hip extension on a dynamometer in 4 different p

Psoas release at the pelvic brim in ambulatory patients with cerebral palsy: operative technique and functional outcome.

Seventeen patients with cerebral palsy (29 hips) underwent psoas recession at the pelvic brim. The operative technique was a direct anterior approach, lateral to the femoral sheath. There were no infections or nerve or arterial injuries. After surgery, clinical examination revealed that fixed hip-flexion contractures decreased significantly in all patients. All of the subjects retained the ability to flex the hip against gravity and against manual resistance. All of the subjects underwent pre- and postoperative gait analysis. Stance-phase dynamic minimum hip flexion decreased significantly. Dy

Postoperative sciatic and femoral nerve palsy with reference to leg lengthening and medialization/lateralization of the hip joint following total hip arthroplasty.

The radiographs and prospective records of 1284 (1152 primary and 135 revisions) Charnley low friction arthroplasties performed by one surgeon were studied in reference to postoperative elongation of the limb and lateralization or medialization of the center of rotation of the hip joint and their effect on postoperative nerve palsy. Displacement of the center of the hip joint in relation to fixed points on the pelvis was measured. In primary low friction arthroplasties, leg lengthening ranged from 0.4 to 4 cm; in the revision group, they ranged from 0.04 to 5.8 cm. Sixty-six hips were lengthen

Modified Shelf Acetabuloplasty Endoscopic Procedure With Allograft for Developmental Hip Dysplasia Treatment.

Hip dysplasia has been identified as one of the leading causes of osteoarthritis. However, hip arthroscopy alone, in the setting of hip dysplasia, remains controversial. In borderline hip dysplasia, with lateral center-edge angle (LCEA) between 18° and 25°, good outcomes have been reported with appropriate capsular and labral management. However, in severe hip dysplasia, with LCEA below 18°, there is an acetabular bony structural deficiency that must be addressed. Even with the potential benefit of hip arthroscopy in addressing intra-articular injuries related to the instability, it cannot be

Posterior hip dislocation associated with posterior wall acetabular fracture and ipsilateral intertrochantric fracture: a very rare case report.

Traumatic hip dislocations are common in high-energy motor vehicle accidents. We present a case of a 43-year old man who sustained posterior hip dislocation with posterior wall acetabular fracture and ipsilateral intertrochantric fracture following a motorcycle accident. Urgent open reduction and internal fixation of the hip fracture-dislocation and fixation of intertrochantric fracture with a dynamic hip screw were done. To our knowledge, such an injury has been rarely reported in the literature. Possible mechanisms of injury and operative procedures are discussed. Radiographic follow-up afte

Intra-articular anterior inferior iliac spine avulsion with hip instability: A retrospective study.

BACKGROUND: Intraarticular anteroinferior iliac spine (AIIS) avulsion fractures combined with hip dislocation are infrequently reported in medical literature. This study aims to present the outcomes of surgical treatment for this rare type of injury. METHODS: We retrospectively analyzed six cases of AIIS avulsion fracture with hip dislocation treated surgically. All patients underwent emergency hip reduction under sedation followed by open reduction and internal fixation using 4 mm cannulated screws through the iliofemoral approach. RESULTS: After an average follow-up period of 50 months, al
