Phosphate management in chronic kidney disease.

PURPOSE OF REVIEW: The review focuses on the rationale and evidence behind management strategies for hyperphosphatemia in patients with chronic kidney disease (CKD). RECENT FINDINGS: Optimal management of phosphate in CKD remains an area of uncertainty, but multiple studies now point to a clinical benefit from the use of phosphate binders. Evidence of improved survival is particularly strong with sevelamer, though it remains unclear whether the absence of calcium or other properties of sevelamer are responsible for this relationship. Newer agents, such as iron-based binders or niacin compound…

Stone, paper… seizure? A case report of hypocalcemic seizures worsened by anti-seizure medications.

Refractory seizures in the emergency department (ED) may result from overlooked metabolic causes. Primary hypoparathyroidism is a rare but treatable etiology, sometimes associated with Fahr's syndrome. We report a 27-year-old man with a known seizure disorder presenting with multiple daily generalized tonic-clonic seizures unresponsive to polytherapy. Examination revealed positive Trousseau's and Chvostek's signs. Laboratory tests demonstrated severe hypocalcemia, hyperphosphatemia, and markedly reduced intact parathyroid hormone, consistent with primary hypoparathyroidism. Non-contrast CT hea…

A Western Diet High in Phosphate Primes the Development of the CKD-Mineral Bone Disorder in an Alport Syndrome Model.

KEY POINTS: An animal protein high-phosphate diet decreased cardiac mitochondrial oxidative phosphorylation in mice with normal kidney funct … ases cardiac mitochondrial function independent of vascular disease and before cardiac hypertrophy. Hyperphosphatemia, a component of the CKD-MBD occurring in later stages of CKD, has been shown to stimulate vascular calcification (VC). In a mouse model of Alport syndrome CKD resistant to VC, we examine the effects of a high-phosphate Western-type diet (HP) on the …

Neonatal hypocalcemia caused by maternal hypercalcemia: clinical characteristics, etiology, treatment, and outcome.

BACKGROUND: Maternal hypercalcemia is a rare but potentially serious cause of late-onset neonatal hypocalcemia. Data on the clinical charact … eizure between 5 and 12 days of age. Laboratory findings revealed hypocalcemia (median: 5.3 mg/dL), hyperphosphatemia (median: 10.2 mg/dL), inappropriately intact parathyroid hormone (median: 18.4 pg/mL), and low 25-hydroxyvitamin D3 levels. Neonates born to mothers with severe hypercalcemia (≥11 mg/dL) had lower serum calcium and higher phosphorus levels compared …

The impact of accessibility to non-calcium-based phosphate binders and calcimimetics on mineral outcomes in patients receiving maintenance hemodialysis: A 10-year retrospective analysis of real-world data.

INTRODUCTION: Hyperphosphatemia and hyperparathyroidism are common in end-stage kidney disease and are associated with poor outcomes. In addition to adequate dialysis, medications are usually required for optimum control of serum phosphate and parathyroid hormone (PTH) levels. The use of calcium-based phosphate binders (CBPBs) and active vitamin D is associated with an increase in serum calcium and worsening vascular calcification. To overcome these limitations, non-calcium-based phosphate binders (NCBPBs) and calcimimetics have been developed. However, the coverage for these new medications r…

Hypocalcaemic Tetany and Seizures Secondary to Vitamin D Deficiency.

PRESENTATION: A female adolescent presented with a generalised tonic-clonic seizure preceded by hands stiffness. She presented with a sudden history of being unresponsive with head deviation to right side, followed by up-rolling of eyes, and whole body shaking which self-terminated after five minutes. On examination, she was in postictal state, with inward clenching of both hands and fingers. The rest of her systemic examination was normal. DIAGNOSIS: Laboratory results showed severe hypocalcaemia, hypomagnesaemia, hyperphosphatemia, elevated parathyroid hormone (PTH), and critically low vita…

Primary Ηypoparathyroidism in a Patient With Sarcoidosis: A Case Report.

Sarcoidosis is a complex, multisystem granulomatous disorder with a variable clinical presentation, commonly involving the lungs but potenti … -converting enzyme (ACE) levels. Surprisingly, the patient also exhibited significant hypocalcemia, hyperphosphatemia, and low parathyroid hormone levels, which led to the diagnosis of primary hypoparathyroidism, an unusual finding in the context of sarcoidosis. Acute Refractory Hypocalcemia in a 51-Year-Old Male With a History of 1,1-Difluoroethane Inhalation.

Hypocalcemia is a common electrolyte derangement that is most associated with parathryoid hormone or vitamin D abnormalities. Less common causes that most providers are aware of include hyperphosphatemia, acute pancreatitis, chronic kidney disease, and sepsis. However, certain populations are at risk for less common, but no less dangerous, causes. One such cause is 1,1-difluoroethane, an organofluorine that is used as a propellant in aerosol sprays and is commonly abused. 1,1-Difluoroethane has been noted to cause severe hypocalcemia by accumulation of the metabolite fluorocitrate in tissues. …

A novel homozygous variant in exon 10 of the GALNT3 gene causing hyperphosphatemic familial tumoral calcinosis in a family from North India.

Hyperphosphatemic familial tumoral calcinosis (HFTC) is an extremely rare autosomal recessive disorder caused by variants in the GALNT3 (N-acetylgalactosaminyltransferase 3), FGF23 (Fibroblast Growth Factor-23) and αKL (α-Klotho) genes, which results in progressive calcification of soft tissues. We describe the case of a 9-year-old girl who presented with recurrent hard nodular swellings on her feet and knees which intermittently discharged chalky white material. Her younger brother also had a similar condition. Both siblings showed hyperphosphatemia, but the parentsbiochemical parameters were…

Sanjad-Sakati syndrome with macrocytic anemia and failure to thrive: a case from South Jordan.

UNLABELLED: Backgorund: Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disease caused by a deletion mutation (155-166del) in exo … ciency, which has not been described in previous SSS cases. It was difficult to treat the patient's hyperphosphatemia and we ultimately selected sevelamer treatment, which was tolerated well and improved his hypocalcemia. CONCLUSIONS: SSS should be considered in the differential diagnosis of any infant with hypocalcemia, dysmorphism and failure to thrive.

Ileus and Intra-Abdominal Hypertension due to Phosphate- Containing Enema.

The most well known complications of fleet enema solution are cardiac insufficiency, renal failure, water-electrolyte imbalance, and ileus. A 7-y-old girl with phenylketonuria and long-term constipation was admitted to the emergency department with symptoms of seizure, vomiting and abdominal distention. Laboratory results revealed hypocalcemia and hyperphosphatemia. ECG findings showed normal sinus rhythm and prolonged QT interval. At the follow-up, the patient's abdominal distention was markedly increased. She was evaluated for a surgical pathology and, this was considered unlikely. Intra-abd…

INTRODUCTION: Hypoparathyroidism is an entity associated with hypocalcemia, more frequently a consequence of neck surgery. An autoimmune etiology is rare and its diagnosis difficult to establish. Besides hypocalcemia (4.6 mg/ dL), hyperphosphatemia (8.7 mg/dL), undetectable parathyroid hormone and low urinary calcium, phosphorus and magnesium were present. Molecul…
