Food allergy.

Adverse clinical reactions to food associated with disturbed immunologic function (food allergy) affect 1-3% of the population and vary from life-threatening to a minor inconvenience. They must be differentiated from reactions caused by toxins, pharmacologic agents, enzyme deficiences and non-specific release of inflammatory mediator substances. Enteric absorption of food protein antigens which may occur despite an array of gastrointestinal protective mechanisms normally induces both a protective immune response and immunologic tolerance. Quantitative changes in absorption related to deficient

Methemoglobinemia caused by the ingestion of courgette soup given in order to resolve constipation in two formula-fed infants.

Methemoglobinemia is not a rare condition arising from the exposure to hemoglobin-oxidizing agents such as nitrates-nitrites present in well … d treated with methylene blue at 1%. After 12 h the syndrome was completely resolved. Home-prepared infant foods containing vegetables are potential causes of methemoglobinemia. It is important not to feed infants with vegetables having a high nitrate content (e.g., courgette, spinach, beets and green beans) to resolve constipation since, particularly in the …

[Serious neurological compromise due to vitamin B12 deficiency in infants of vegan and vegetarian mothers].

Vitamin B12 deficiency is one of the most serious complications of vegetarianism and its variants. Infants born to vegan mothers are at greater risk of serious deficiency, being more vulnerable to their effects. B12 deficiency is not usually suspected by the pediatrician in healthy infants with neurological symptoms. The manifestations are nonspecific: apathy, rejection of food and loss of maturational patterns. A nutritional history of the mother, mainly if she is vegetarian, to estimate her reserves is fundamental to detect risk of deficiency of this vitamin in the small child. The objective

Predictors for late tolerance development in food protein-induced allergic proctocolitis.

Background: Food protein-induced allergic proctocolitis (FPIAP) is a non-immunoglobulin E (IgE) mediated food allergy that typically presents with blood-mixed mucoid stool. Objective: To identify the predictors that affect the tolerance development in infants with FPAIP and laboratory as well as clinical differences between patients with early and with late tolerance. Methods: A total of 185 infants with FPIAP were included. The patients were grouped and analyzed based on laboratory tests and clinical characteristics. Results: The median (interquartile range [IQR]) age of onset of symptoms was

Household food production is positively associated with dietary diversity and intake of nutrient-dense foods for older preschool children in poorer families: Results from a nationally-representative survey in Nepal.

BACKGROUND: Nutrition-sensitive interventions supporting enhanced household food production have potential to improve child dietary quality. However, heterogeneity in market access may cause systematic differences in program effectiveness depending on household wealth and child age. Identifying these effect modifiers can help development agencies specify and target their interventions. OBJECTIVE: This study investigates mediating effects of household wealth and child age on links between farm production and child diets, as measured by production and intake of nutrient-dense food groups. METH

Effectiveness of two feeding methods in improving energy intake and growth of infants with cleft palate: a randomized study.

OBJECTIVE: To compare two feeding methods advocated for infants with cleft palate: (a) a squeezable plastic container with a narrow, long crosscut nipple (squeezable cleft palate nurser); and (b) a standard nipple with a crosscut (crosscut nipple). The effectiveness of a nutrition intervention protocol for these infants was also documented. DESIGN: Thirty-one infants (median age = 15 days) were randomized to one of two feeding methods (18 infants, squeezable cleft lip/palate nurser; 13 infants, crosscut nipple) within sex (21 boys, 10 girls) and palatal defect (22 cleft lip and palate, 9 isol

Presence of Escherichia coli O157:H7 in ground beef and ground baby beef meat.

A total of 114 beef and baby beef samples were examined. The samples included ground baby beef, mixed ground baby beef and pork, and chopped and shaped meat. The samples were analyzed from 30 different grocery stores in Zagreb, Croatia. The object of this study was to evaluate the prevalence of Escherichia coli O157:H7 in the samples that can enhance the potential risk of outbreaks of hemorrhagic colitis and hemolytic uremic syndrome. The results in all tested samples of E. coli O157:H7 were negative. A single sample was positive in a latex agglutination test using antiserum to O157:H7. It was

Feeding preterm infants after hospital discharge: a commentary by the ESPGHAN Committee on Nutrition.

Survival of small premature infants has markedly improved during the last few decades. These infants are discharged from hospital care with body weight below the usual birth weight of healthy term infants. Early nutrition support of preterm infants influences long-term health outcomes. Therefore, the ESPGHAN Committee on Nutrition has reviewed available evidence on feeding preterm infants after hospital discharge. Close monitoring of growth during hospital stay and after discharge is recommended to enable the provision of adequate nutrition support. Measurements of length and head circumferenc

Randomised trial of nutrition for preterm infants after discharge.

In a randomised double blind trial, the effect on growth and clinical status of a nutrient enriched 'post-discharge' milk formula versus a standard term formula, was compared in 32 exclusively bottle fed preterm infants. The formulas were used as the sole milk intake up to a postnatal age of 9 months. Significant increases in linear growth and weight gain were observed in the infants who received the enriched diet. There were no differences in vomiting, posseting, or bowel habit between the groups. Formula volumes ingested were similar between diet groups, indicating that the difference in for

Calcium, phosphorus, and magnesium: how much is too much for infant formulas?

Despite the use of metabolic balance studies and measurements of bone mineral content, it is difficult to assess the requirements of Ca and P in growing infants. From observations made forty years ago with cow's milk formulas it is obvious what is too much calcium and phosphorus in infant formulas. These observations were utilized in the development of "humanized cow's milk formulas" in the 1950s. Though the total concentrations of Ca and P have been greatly reduced in these formulas, the Ca/P ratio imbalance, compared to human milk, persists. Infants fed these formulas still receive a relativ

[Weaning of infants in the Turin district. An epidemiological survey].

A cross section study was performed on 1060 healthy children living in the Torino district to examine the modalities followed for weaning during infancy. Mothers were asked to fill in a questionnaire which contained questions concerning the family, allergic and metabolic diseases in relatives, type of food given to infants before, during and after weaning, and source of dietary counselling. A score was given for each answer. Data indicate generally correct trends though major fields of error concerns prevention of food allergy and coeliac disease. Scores were correlated with family type and ar

Prevention of iron deficiency by milk fortification. II. A field trial with a full-fat acidified milk.

In a longitudinal study from age 3 to 15 mo, 276 term, healthy, spontaneously weaned infants received a full-fat acidified milk fortified with 15 mg of elemental Fe as ferrous sulfate and 100 mg of ascorbic acid/100 g of powder and 278 control infants received milk without additives. At ages 9 and 15 mo significant differences were encountered in all measures of Fe nutriture in favor of the fortified group (p less than 0.001). Anemia (Hg less than 110 g/L) was present in 25.7% of unfortified infants compared with only 2.5% in those fortified at age 15 mo. Saturation of transferrin less than 9%

[An acute vomiting infant with profuse diarrhoea: enterocolitis due to non-IgE-mediated cow's milk allergy].

BACKGROUND: Food protein-induced enterocolitis is a manifestation of non-IgE-mediated cow's milk allergy, characterized by acute vomiting and profuse diarrhoea. This reaction is often not recognized as cow's milk allergy. CASE DESCRIPTION: We present a 6-week-old, formula-fed girl with frequent vomiting, diarrhoea and failure to thrive. These symptoms disappeared after giving cow's milk-free formula. Forty-five minutes after the last dose in a non-blinded provocation test with cow's milk, she developed profuse diarrhoea and vomiting, resulting in hypovolemic shock. No specific IgE against cow

[Allergen skin tests. Asthmatic infants and young children].

Skin tests represent a major tool in the diagnosis of IgE mediated-allergy. The main indication is the measure of atopy during infancy and early childhood, as a predictive factor of the development of asthma. When atopic eczema is present, skin testing can provide a diagnosis of food allergy. Skin tests are easily done, without absolute contraindications. The epidemiological knowledge helps to choose the allergens to test. The skin-prick test method is well standardized and must be rigorous because of the hyporeactivity of the infant skin. Results are always confounded with positive and negati

Perspectives on lead toxicity.

Lead toxicity causes hematological, gastrointestinal, and neurological dysfunction in adults and children. Symptoms are usually noted with blood lead greater than 1.93 mumol/L. Severe or prolonged exposure may also cause chronic nephropathy, hypertension, and reproductive impairment. Lead inhibits enzymes; alters cellular calcium metabolism; stimulates synthesis of binding proteins in kidney, brain, and bone; and slows nerve conduction. Less severe exposure to lead, designated by blood lead levels of 0.48-0.96 mumol/L, has been implicated in poor pregnancy outcome, impaired neurobehavioral dev

Pesticide, metal, and other chemical residues in adult total diet samples. (XIV). October 1977-September 1978.

Residues of pesticides, herbicides, fungicides, and selected industrial chemicals and metals have been reported since 1964 through the Total Diet Study conducted by the Food and Drug Administration. This report, which is the fourteenth in the series, includes data for the adult diet market basket samples collected from October 1977 through September 1978 in 20 cities throughout 4 geographical areas. All items from each market basket were separated into 12 food classes for analysis, and the averages and ranges of the residues found are presented in various formats. In addition to the 240 compos
